1. DESCRIPTION OF SERVICES

The Contractor shall provide all labor, supervision, equipment, tools, expendable parts/materials, transportation and services necessary to perform inspection, test, maintenance and repair of U.S. Government-owned roll-up (high speed), overhead and shutter doors at the Kunsan AB in accordance with this Performance Work Statement (PWS) and equipment manufacturer’s operation and maintenance (O&M) manuals. The workload estimates requiring these services are listed in the Appendix A. All type of doors generally called ‘overhead or overhead coiling doors’ are here in the PWS grouped into 3 categories, rollup, overhead and shutters by specific technical requirements. Comparison of these overhead doors is presented in the Appendix B, Comparison Chart of Doors. Typical ITM (inspection, test, and maintenance) work tasks are described in the Appendix D, only for reference purpose.

1.1. INITIAL SURVEY AND PRE-INSPECTION SERVICE (BASE YEAR ONLY)

1.1.1 The Contractor shall perform an initial survey and inspection of each roll-up, overhead and shutter door to collect technical data/quantities from respective buildings, ascertain the status of each door, and to prepare reports for any repairs necessary to restore the doors to operable condition. In addition the findings will be incorporated into the contract. Recommended survey data are, but not limited to, those displayed on the Appendix C. Reasonable care shall be exercised to preclude damaging the door and its components during disassembling, inspection and testing operations. This survey and inspection shall be completed within 45 days after the Notification to Proceed (NTP) from the Contracting Officer (KO) and shall be performed along with 1st maintenance work as described in the paragraph 1.2.

1.1.2 The Contractor shall determine level of maintenances and repairs required and shall prepare an inspection report for each door. This report shall include building number; location of each door, door type (overhead, rollup or shutter) and itemized list of parts & cost to replace, paint or repair door parts.

1.1.3 When an initial survey and inspection of an (overhead, rollup or shutter) door indicates that an economical repair is not feasible and (overhead, rollup or shutter) door needs entire replacement and installation the Contractor shall coordinate with the COR to obtain written approval from the KO for a DO. The request shall include a description of the defective component being replaced and the estimated cost with at least three quotes for (overhead, rollup or shutter) door defective components and submit to COR for approval. A report shall be submitted to the COR indicating the building number and (overhead, rollup or shutter) door location along with a description of the work performed and the warranty provided.

If a building is manned 24 hours a day, 7 days a week, an exterior rollup, overhead and shutters door that will not lock is not an emergency. The Contractor shall respond to emergency repair calls within two hours from the time of notification during normal business hours and within three hours from the time of notification at times other than normal business hours that the (overhead, rollup or shutter) doors are out of service. A call of this type would suggest that there are security issues, or the roll up door condition could result in life, or property endangerment, or could have an adverse effect upon personnel, equipment and mission accomplishment. Emergency service shall be provided, 24 hours a day, seven days a week including holidays. If this type of call requires emergency repair, the Contractor shall prepare and submit to the Government a detailed written report within two business days after the emergency repair. The report shall include the date and time of the service call, the location of the door, the repairs performed, and the name of the technician performing the repairs. On call repair will be requested by the Contracting Officer or his/her designated representative based on customer needs and deficiencies identified on initial survey or preventative maintenance.

1.3.2.1 After restoring the Exterior rollup, overhead and shutters doors to an operable condition, the contractor shall submit a new condition report of service call to the COR within three (3) business days. The condition report shall specify the types of work performed, completed time, and either recommended ELINs needed or a list of repair parts provided, if furnished. Report shall list any other additional information important to ensure proper door functioning and operation.

1.3.2.2. If the service call cannot be accomplished within four (4) hours and less than KRW50, 000 ($50.00) for consumable/repair parts, the contractor shall inform the COR in writing. This may be done verbally at first, but must be provided in writing within one week. The written communication will identify all relevant aspects including but not limited to root cause and or corrective actions necessary. The Contractor shall evaluate whether the repairs may continue to be accomplished or performed as unscheduled repair/replace of parts. If the COR concurs with the Contractor’s rationale for unscheduled repair/replace of parts, the proposed repair shall be accomplished via DO.

1.3.3 UNSCHEDULED REPAIR/REPLACE OF PARTS: The contractor shall provide all labor, personnel, equipment, tools, materials, vehicles, and supervision to perform ELINs repair and replacement of parts service. The COR shall provide a summation of unsatisfactory inspections and customer complaints and provide insight into any identified trends.

APPENDIX A-9

2.2.1.10 The minutes of these meetings will be recorded by the COR, signed by the COR and any other signatures as deemed appropriate, distributed to the functional area and the contractor. Should the contractor not concur with the minutes, the contractor shall provide a written notification to the Contracting Office identifying areas of non-concurrence for resolution.

2.3 QUALITY CONTROL.

The Government will make review and acceptance of the QCP and any subsequent changes within 10 days after receipt of the revised QCP. After acceptance of the QCP the contractor will receive the Contracting Officer's acceptance in writing of any proposed change to his QCP system.

At a minimum the QCP should address:

Overall contract management and administration.

Describe a method acceptable to the Government of identifying deficiencies in the quality of service performed under this contract before the level of performance becomes unacceptable and address processes for corrective actions without dependence upon Government direction.

Establish an inspection system covering all services required by this contract. This plan shall specify areas to be inspected on either a scheduled or an unscheduled basis and the title of the individual who will do the inspection.

The contractor shall keep records of inspections and make them available to the Government throughout the contract performance period and for the period after contract completion until final settlement of any claims under this contract. Periodic validations will be conducted on a scheduled basis (daily, weekly, monthly, quarterly, semi-annual or annually) and may be adjusted, based on quality trends. This will be based on contractors QCP that is submitted that describe work performed (daily, weekly, monthly, quarterly, semi-annual or annually) that is required within 10 days of start of contract. Any unsatisfactory inspection (defect) result shall be recorded within that day, and the Contractor shall re-perform the service after notification by the KO or COR. Failing to meet the performance threshold as outlined in the SS for any of these performance objectives in any one month period shall result in a warning or letter of concern from the KO. Failing to meet the performance threshold as outlined in the SS of these performance objectives in any combination for any two, or more consecutive or non-consecutive months during a contract period shall constitute an immediate Progress Meeting with the Multi-functional Team.
