PERFORMANCE RISK ASSESSMENT QUESTIONNAIRE

The Army Contracting Command, Aberdeen Proving Ground requires past performance information as part of an evaluation process for Request for Quote W91CRB-18-R-0027. On the following pages, “Offeror” refers to the firm using your company as a reference. Handwritten responses are sufficient. Your input is important to us and responses are needed for inclusion in our evaluation. Once completed, please e-mail to sabrinna.r.cosom.civ@mail.mil and gerald.a.simmons2.civ@mail.mil.

Please return the completed Questionnaire within twenty-five (25) calendar days of the release of the RFP. Thank you.

PART 3: EVALUATION

For the purposes of this survey the government will receive an overall assessment of what the contractor can perform in accordance with the solicitation, using the ratings below. In responding to the survey, please justify and explain exceptional, very good, marginal, and unsatisfactory ratings. Please include comments to explain all ratings given. Respondent may be contacted to clarify comments or ratings. An individual within your organization that is knowledgeable of the contractor’s day-to-day operations and overall performance should complete this questionnaire.

The ratings below are to be used to evaluate the performance of the contractor identified above, NOT the offeror.

5 - Exceptional- Performance meets contract requirements and significantly exceeds contract requirements to the Government’s benefit. For example, the contractor implemented innovative or business process reengineering techniques, which resulted in added value to the Government. The contractual performance of the element or sub-element being assessed was accomplished with few minor problems for which corrective actions taken by the contractor were highly effective.

4 - Very Good- Performance meets contractual requirements and exceeds some to the Government’s benefit. The contractual performance of the element or sub-element being assessed was accomplished with some minor problems for which corrective actions taken by the contractor were effective.

3 - Satisfactory- Performance meets contractual requirements. The contractual performance of the element or sub-element contains some minor problems for which proposed corrective actions taken by the contractor appear satisfactory, or completed corrective actions were satisfactory.

2 - Marginal- Performance does not meet some contractual requirements. The contractual performance of the element or sub-element being assessed reflects a serious problem for which the contractor has submitted minimal corrective actions, if any. The contractor’s proposed actions appear only marginally effective or were not fully implemented.

1 - Unsatisfactory- Performance does not meet contractual requirements and recovery is not likely in a timely or cost effective manner. The contractual performance of the element or sub-element contains serious problem(s) for which the contractor’s corrective actions appear or were ineffective.

NOTE: If you have rated this contractor on this particular contract or project in CPARS or some other evaluation system accessible to the Government, you may indicate that past performance information on this contract/project is available in the appropriate system and you may include a copy of that rating in lieu of the following questionnaire.
