STATEMENT OF WORK

Statement of Work for placement of Stone for the Left Dam Abutment

General Requirements The statement of work includes excavating and placement of stone riprap protection on the left abutment (looking down stream) of the dam. Green River Lake Dam is located approximately 6 miles south of Campbellsville, Kentucky in Taylor County, Kentucky.

a. The contractor shall provide all equipment, materials, supplies, and labor to perform excavation of existing materials and placement of number 4 (#4) or number 23 (#23), KYTC Class I, and KYTC Class III Rip Rap protection on the left abutment of the dam.

b. Access to the site and coordination for construction is through the Green River Lake Corps of Engineers (COE) office with Lori Brewster, Project Manager, at (270) 465-4463. It is incumbent upon the contractor to visit the site and work out details of equipment storage and staging of materials with the Project Manager or representative. The road entering from highway 55 closest to the dam is capable of handling the full load of 25 tons from the tri-axle trucks. The stone will be dumped from the top of the dam and not carried over the stilling basin conduit. The gravel service road extending from the tail-water parking lot over the stilling basin conduit and along the foot of the Dam is capable of withstanding debris loads from the excavated area on the dam. It will be to the contractor’s discretion which route they choose for hauling out the debris, but the stone will be brought to the top of the dam and not over the conduit.

c. Once the contractor has selected a quarry to supply stone for the subject project a qualitative field investigation shall be conducted by a Louisville District Dam Safety Engineer of the proposed quarry. Any available and current test and performance data conducted by State Agencies, to determine the suitability for production ledges and/or stockpiles to be utilized for the project shall be provided for review. If the quarry has testing information conducted specifically on riprap, then it should be provided for review. Aggregate testing results will be reviewed; however, aggregate results should only be used as a relative indicator of stone quality, not as a replacement to riprap testing, as aggregate testing is not indicative of larger scale stone performance. Louisville District Dam Safety Engineer will determine if the quarry can be used for this contract or if the contractor will need to find a different quarry.

d. The contractor will clear eroded area of any woody debris, trash, or any vegetation. There is no requirement for a silt fence in this excavation process. Prior to placing the base stone and riprap, the contractor will excavate the described eroded area of the Dam abutment with 3:1 pitched slopes and two foot (2’) wide bottom in accordance to the attached drawings. The prepared base shall be inspected by the Project Manager’s representative or Technical POC and no material shall be placed thereon until that area has been approved.

e. The contractor is responsible for all debris and unacceptable material excavated from area and will be taken off site and disposed of in accordance with State and Local Laws. Excavated materials that meet the specified gradations, as determined by the Louisville District Dam Safety Engineer, can be stockpiled to be reused.

f. The Corps of Engineers has established repair limits for this project with driven stakes (not to be mistaken with pool elevation markers). The described slope of the eroded area of the left abutment (looking down stream) is approximately 20 feet (20’) wide, between COE driven stakes approximately 40 feet (40’) from road edge from top of Dam (careful consideration will be taken to not damage existing PZ piping at Dam road edge), down approximately 280 feet (280’) to existing KYTC Class I and KYTC Class III riprap.

g. The contractor will bed the entire newly excavated area with a minimum of six inches (6”) of number 4 (#4) or number 23 (#23) stone. There is no need for installment of the geo fabric material. The contractor will overlay the base stone with a combination of Class I and Class III riprap protection at a minimum depth of two foot (2’) in the center and tapering out to a minimum depth of one foot (1’) on each side to match the existing ground. If the depth of the stone is greater than (1’) or is slightly above grade at the taper that is acceptable, however; in no case will less than ground level be approved. The contractor shall place the combination of Class I and Class III riprap in such a manner as to produce a reasonably well graded mass of rock with the minimum practicable percentage of voids. The inclusion of objectionable quantities of dirt, sand, clay or rock fine coatings shall not be permitted as this reduces the ability of the field personnel to observe the delivered stone. The stone fill must be placed starting at the existing repaired slope. The fill will be placed to full thickness and progress up the slope. This will be done to prevent destabilizing the slope. Protection stone shall be placed to its full course thickness in one operation. The desired distribution of the various sizes of stones throughout the mass shall be obtained by selective loading of the material at the quarry or other source; by controlled dumping of successive loads during the final placing; or by other methods of placement which will produce the specified results. Great care shall be taken during placement to reduce impact or mechanical induced fracturing. Great care shall be exercised when placing stone adjacent to existing concrete ditch by protecting them using hand placement until there is adequate cover for protection. Rearranging of individual stones by mechanical equipment or by hand will be required to the extent necessary to obtain a reasonably well graded distribution of stone sizes as specified above. Dropping of the riprap shall be kept to a minimum, and no tracked equipment shall be allowed on the newly placed riprap. The contractor will maintain the stone protection until accepted and any material displaced by any cause shall be replaced at contractors’ expense to the lines and grades shown on the drawings.

h. All disturbed areas shall be restored to original condition upon completion of the project. In no way shall the contractor disturb or maneuver over the previously repaired slope that the stone placement will be tying into.

i. At no time shall the contractor start work until Louisville District Dam Safety Engineer has approved the rock from the contractors selected quarry.

GEOTECHNICAL REQUIREMENTS

a. Suitable Riprap Material Suitable riprap materials shall be comprised of materials classified as Class I and Class III Channel Lining as stated in Section 805 of the Kentucky Transportation Cabinet Standard for Roads and Bridge Construction 2008. Samples which are acceptable for use on this project should not contain unacceptably high amounts of deleterious materials or structures, including shale, clay, claystone, ironstone, weak bedding planes, thick carbonaceous stylolitic seams, open fractures, and numerous nodule or massive beds of chert, chalcedony, flint, etc. Riprap stones shall be approximately equal-dimensional in cross-section and be relatively free from thin slabby pieces having a flat or elongation ratio greater than three. In no case shall the quantity of riprap stones having a flat or elongation ratio greater than three exceed five (5) percent by weight of any one load or area. For these specifications the following definitions shall apply: Flat Stone: Stone with a ratio of width to thickness greater than 3; Elongated Stone: Stone with a ratio of length to width greater than 3. The KYTC size standards for channel lining and the KYTC standards for coarse aggregate are attached to this document and sizes and description are as follows:

805.13.03 Channel Lining, Class IA. Provide crushed stone meeting the general requirements of this section. Use a crusher, grizzly, or sieve with openings to produce a grading that 100 percent passes the 5 inch sieve, no more than 20 percent of the finished product passes through square openings 1 1/2 by 1 1/2 inches. Please refer to Exhibit A.
