The location of any conditions that may prevent proper installation of lining materials into the pipeline must be immediately reported to the COR and the representative of Olympia Fields, with follow-up documentation provided within 24 hours so that these conditions can be corrected or properly addressed. The Contractor for later reference by the Owner and Engineer must produce a videotape/DVD showing pre and post lateral lining with a written log detailing the lateral lining length, depth and corresponding address. The Contractor must provide the Engineer with three (3) copies and the Army Corps one (1) copy of the written log and videotape/DVD.

It is the Contractor's responsibility to clear the line of all obstructions such as solids, corrosion on inner pipe walls, dropped joints, protruding branch connections or broken pipe that will prevent the insertion of the liner. If inspection reveals an obstruction that cannot be removed by conventional cleaning or internal cutting equipment, or that it would be inadvisable to rehabilitate the service lateral by CIPP, then the Contractor must inform the Government in writing with copies to the Owner and Engineer in writing as to the reasons for point repairs by excavation, recommended abandonment of lining, or other circumstances.

PART 2 PRODUCTS

2.1 GENERAL

The resins, fabric tube, tube coatings, or other materials, such as a calibration hose when combined as a composite structure, produce CIPP that meets the requirements of ASTM F1216, ASTM F1743, and this specification.

2.2 LATERAL LINER

The tube must consist of one or more layers of absorbent fabric capable of carrying resin, and capable of withstanding installation pressures and curing temperatures. The tube material must be able to stretch to fit irregular pipe sections and negotiate bends up to 90o. The outside layer must be plastic coated with a material compatible with the resin system used.

The liner must conform to the minimum structural standards as listed below:

CURED-IN-PLACE PIPE LINER STANDARD RESULTS

Tensile Stress ASTM D638 3,000 psi Flexural Stress ASTM D790 4,500 psi Modulus of Elasticity ASTM D790 250,000 psi

Fabricate liner to a size that when installed will neatly fit the internal circumference of the service lateral. Verify the lengths and internal pipe diameters in the field before installation. The minimum length must be that necessary to effectively span the distance from the connection at the sewer main to the property line, unless otherwise specified.

SECTION 33 01 99 Page 4

2.2.1 Resin

The resin must be a thermoset resin system that is compatible with the cured-in-place pipe installation. The resin must be able to cure in the presence of water and the initiation temperature for curing must be less than 180oF. The bond between all CIPP layers must be strong and uniform, with no part of the tube left unsaturated by resin. The finished liner in place must be fabricated from materials which when cured will be chemically resistant to withstand internal exposure to domestic sewage.

The length of the product will be based on the plans and field measurements, taken in the field by the Contractor. Product will be suitable for use with raw untreated wastewater, including being resistant to sewage having a pH range of 5 to 11 and temperatures up to 150 degrees F.

Minimum service life of 50 years.

2.2.2 Cleanouts

For lateral lining systems which require a cleanout, and where existing cleanouts are not available, the Contractor must excavate and construct a cleanout through which the lateral pipeliner can be positioned. The cleanouts will be PVC and installed from the sewer service line to the existing grade. The cleanout be capped with a screw on cap. The clean out locations will be laid out by the Village while the contractor is locating the cleanouts. The location f the proposed cleanouts are shown on the plans.

PART 3 EXECUTION

3.1 PREPARATIONS

Complete the following preparations, unless approved otherwise by the Contracting Officer. The Contracting Officer makes no guarantee regarding the information, data, and physical condition of underground facilities or existing pipe lines. a. The Contractor make every effort to maximize service usage throughout the duration of the project. The time each service is out of service be minimized; but the maximum amount of time a customer is out of service be 8 hours. A public notification program be implemented, and as a minimum, require the Contractor to be responsible for determining each home or business connected to the sanitary sewer, contacting them, and informing them of the work to be conducted, when the sewer will be off-line, and when the sewer is put back on-line. The Contractor must also provide the following:

1. Written notice to be delivered to each home or business the day prior to the beginning of work being conducted on the section, and a local telephone number of the Contractor they can call to discuss the project or any problems which could arise.

2. Personal contact with any home or business which cannot be reconnected within the time stated in the aforementioned written notice.

b. Remove all internal debris out of the sewer line that will interfere with the installation of CIPP, such as solids, mineral deposits, and roots. All lines will be cleaned using high pressure jetting equipment.

Obstructions not removed using the high pressure jetting equipment be identified and the Contracting Officer and Owner be notified of all obstructions within 48 hours and before any CIPP lining work is to commence on that pipe segment.

c. If pre-installation inspection reveals an obstruction such as a protruding service connection, dropped joint, or a collapse that will prevent the installation of CIPP that was not evident on the pre-bid video and it cannot be remedied by conventional sewer cleaning, notify the Contracting Officer and Owner of such lines. The Owner may elect to authorize a point repair with the Contractor or use other means to repair the obstruction, including deleting the work from the project.

The Owner may elect to award a point repair to repair the sewer line

d. CIPP installation be in accordance with ASTM F1216, Section 7, or ASTM F1743, Section 6 or ASTM F2019-03 for Ultra Violet cured pipe.

e. All CIPP be installed in accordance with the manufacturer's recommendations.

3.3.1 CIPP TERMINATION

CIPP terminations be done in a smooth and uniform manner without jagged edges, irregular shapes or protruding material. CIPP be sealed to the pipe using a flexible sealant suitable for sealing all edges of the CIPP and concrete, PVC or clay tile. The invert of the CIPP have a smooth transition from the invert of the manhole to edge of the CIPP.

3.3.2 Installation of Cleanout

The cleanout installation must be made at a point just outside the property line within the right of way, or as determined by the Engineer, and constructed such that the finished lid is flush with the existing ground elevation.

3.3.3 Point Repairs

Work must consist of the removal and replacement of the existing sanitary sewer lateral sections to the line and grade as directed by the Government.

Connections with existing pipes must be made by means of mission couplings.

Existing service connections within the limits of the repair must be replaced with a new pipe section.

All replacement sanitary sewers located in areas where proper horizontal and vertical separation from existing water main is achieved must be semi-rigid PVC SDR 26 conforming to ASTM D3034 with elastomeric gasketed joints meeting the requirements of ASTM D3212. The repair length must be 8' long.

The Contractor must provide and operate all necessary trench dewatering equipment, sheeting and trench boxes to sufficiently keep the trench dry and safe during all sewer replacement operations.

Following excavation and verification of the damaged pipe location and size, the existing sewer must be removed. Then, the trench bottom must be reshaped so that the replacement pipe must be at line and grade otherwise installed at a minimum slope, or as directed by the Engineer. The existing material at the bottom of the trench must be removed and replaced with Grade CA-7 course aggregate. Material added to the trench bottom as a pipe bedding must be tamped to the Engineer's satisfaction. All sewers, must have a Grade CA-7 stone bedding of a minimum thickness of six inches (6").

The Contractor must ensure that the bedding is properly placed under the pipe haunches and is in full contact with the new pipe.

The bell and spigot areas of PVC pipe joints must be thoroughly cleaned, making certain that the gasket is properly seated.

The existing exposed sewer must be cut at the ends within the limits of removal as directed by the Engineer. The replacement pipe must also be cut using the above equipment, as required for abutting plain square ends at the points of connection. The length of the replacement pipe after cutting must be one-half inch to one inch (1/2" to 1") less than the length of the removed pipe. Temporary blocking should be provided as required. The band coupling must be centered over each joint and clamp bolts and shear ring

Unlisted classifications needed for work not included within the scope of the classifications listed may be added after award only as provided in the labor standards contract clauses (29CFR 5.5 (a) (1) (ii)).

The body of each wage determination lists the classification and wage rates that have been found to be prevailing for the cited type(s) of construction in the area covered by the wage determination. The classifications are listed in alphabetical order of "identifiers" that indicate whether the particular rate is union or non-union.

Union Identifiers

An identifier enclosed in dotted lines beginning with characters other than "SU" denotes that the union classification and rate have found to be prevailing for that classification. Example: PLUM0198-005 07/01/2011. The first four letters , PLUM, indicate the international union and the four-digit number, 0198, that follows indicates the local union number or district council number where applicable , i.e., Plumbers Local 0198. The next number, 005 in the example, is an internal number used in processing the wage determination.

The date, 07/01/2011, following these characters is the effective date of the most current negotiated rate/collective bargaining agreement which would be July 1, 2011 in the above example.

Union prevailing wage rates will be updated to reflect any changes in the collective bargaining agreements governing the rates.

0000/9999: weighted union wage rates will be published annually each January.

Non-Union Identifiers

Classifications listed under an "SU" identifier were derived from survey data by computing average rates and are not union rates; however, the data used in computing these rates may include both union and non-union data. Example: SULA2004-007 5/13/2010. SU indicates the rates are not union majority rates, LA indicates the State of Louisiana; 2004 is the year of the survey; and 007 is an internal number used in producing the wage determination. A 1993 or later date, 5/13/2010, indicates the classifications and rates under that identifier were issued as a General Wage Determination on that date.

Survey wage rates will remain in effect and will not change until a new survey is conducted.

WAGE DETERMINATION APPEALS PROCESS

1.) Has there been an initial decision in the matter? This can be:

* an existing published wage determination

* a survey underlying a wage determination

* a Wage and Hour Division letter setting forth a position on a wage determination matter

* a conformance (additional classification and rate) ruling

On survey related matters, initial contact, including requests for summaries of surveys, should be with the Wage and Hour Regional Office for the area in which the survey was conducted because those Regional Offices have responsibility for the Davis-Bacon survey program. If the response from this initial contact is not satisfactory, then the process described in 2.)

and 3.) should be followed.

With regard to any other matter not yet ripe for the formal process described here, initial contact should be with the Branch of Construction Wage Determinations. Write to:

Branch of Construction Wage Determinations Wage and Hour Division U.S. Department of Labor 200 Constitution Avenue, N.W.

Washington, DC 20210

2.) If the answer to the question in 1.) is yes, then an interested party (those affected by the action) can request review and reconsideration from the Wage and Hour Administrator (See 29 CFR Part 1.8 and 29 CFR Part 7). Write to:

Wage and Hour Administrator U.S. Department of Labor 200 Constitution Avenue, N.W.

Washington, DC 20210

The request should be accompanied by a full statement of the interested party's position and by any information (wage payment data, project description, area practice material, etc.) that the requestor considers relevant to the issue.

3.) If the decision of the Administrator is not favorable, an interested party may appeal directly to the Administrative Review Board (formerly the Wage Appeals Board). Write to:

Administrative Review Board U.S. Department of Labor 200 Constitution Avenue, N.W.

Washington, DC 20210

4.) All decisions by the Administrative Review Board are final.
