The Contractor shall furnish to the Contracting Officer the electronic data in Mircostation V8i compatible files (DGN) and a comma delimited ASCII file (containing at a minimum the Point Number, Northing, Easting, Elevation, and Point Descriptor) with all submitted files and drawings being at full scale in same datum as the contract drawings (provided in the general notes). Additionally, provide all CAD files to include but not limited to .dtm, .tin, .alg, (raw data point clouds) and .ASCII or .XML file types used to generate the

SECTION 01 11 00 Page 8 submitted .dgn electronic drawings such as contours, .dtms, plan views, profiles, and cross sections, along with any accompanying field notes (written or electronically collected) to faciliate USACE QA/QC. Terrain model .tin files shall be translated into .XML files using the XML translater function available in AutoCAD or online to facilitate terrain model import on the USACE side. Upon completing surveys, or portions thereof, the Contractor shall not disturb the ground surface until such time that the Contracting Officer has completed Quality Assurance checks of the Contractors' survey data. The Contracting Officer shall disapprove any survey information submitted by the Contractor that lacks sufficient data.

3.3.1.1 Quantity Surveys

Quantity surveys as used in this clause mean a topographical survey accomplished by ground methods with the display output recorded and stored in an electronic field book for further calculations in a computer.

3.3.1.1.1 Quantity Survey Method for Cubic Yard Items

Unless otherwise authorized by the Contracting Officer, use the same means of data collection for Quantity and post-constuction surveys as used for pre-construction surveys. For cubic yard (CY) pay items where the Contractor shall capture pre-construction survey cross sections, capture a post-construction cross section within ten (10) longitudinal feet along the alignment in which each pre-construction cross section was taken so that the end area method of volume calculation can be accurately applied.

The cross sections should accurately display the topography of constructed features including, but not limited to: height, width, grades, and slopes of excavation or fill for all areas requiring cubic yard (CY) unit price measurements and payments. The Pre-Construction Survey and the Post-Construction Surveys shall be submitted in Microstation (.dgn) format. Observations shall be recorded at all breaks in slopes with a maximum distance of 25 feet between observations. Cross sections shall be taken at 90-degrees to baseline or the linear constuction item and at sufficient width beyond the limits of work to delineate the work in place.

The Contracting Officer reserves the right to mandate additional cross sections be taken at locations of his or her choosing for quality assurace purposes. For cubic yard (CY) pay items, other than channel excavation, where the Contractor elects to capture a terrain model (.dtm or .tin) depicting pre-constuction conditions, capture a post construction terrain model that accurately displays all post construction topography of constructed features including, but not limited to: height, width, grades, and slopes of excavation or fill and all breaklines necessary for a level of accuracy to plot two-foot contours. Observations shall be recorded at all breaks in slopes with a maximum distance of 25 feet between observations.

3.3.1.1.2 Quantity Survey Method for Square Yard Items

Unless otherwise authorized by the Contracting Officer, use the same means of data collection for Quantity and post-constuction surveys as used for pre-construction surveys. For square yard (SY) pay items, the Contractor shall, to the satisfaction of the Contracting Officer, take boundary shots, defining the areas of the square yard (SY) pay items with a maximum distance between points being 25 feet.

SECTION 01 11 00 Page 9

3.3.1.2 Post-Construction Surveys

The Contractor shall use the the cross-section method for depicting the final post construction conditions. Cross sections shall be taken the same as directed for the Pre-Construction Survey and depict the topography of constructed features including, but not limited to: elevation, width, grades, and slopes of all items in this contract. The Contractor shall verify acceptable location of at least one random, additional section between the provided/Pre-Construction sections. These intermediate sections shall be verified and approved by the Contracting Officer and shot by the Contractor for quality assurance purposes. Post-Construction Surveys shall be submitted in Microstation (.dgn) format. Section observations shall be recorded at all breaks in slopes with a maximum distance of 25' between observations. At a minimum, the cross sections shall extend ten feet (10') beyond the construction footprint or each item. The annotated cross-sections shall indicate the baseline; the theoretical grades and slope-lines as specified; and the actual grades and slope lines as constructed. The Contractor shall plot cross sections from the survey data to visually depict the grade and sections obtained. The Contractor shall use this survey data to develop the as-built drawings.

In addition to this, survey shall adequately capture, at a minimum, the perimeter(s) for all items paid for by the unit of Square Yard (SY) excluding the following three pay items: Channel Clearing, Grubbing, and Stripping, Burn Site Stripping, and Herbicide Spraying, Mowing, and Vine Treatment which are to be determined as part of the Pre-Construction Survey.

Post-Construction survey shall also verify satisfactory placement to include height, boundaries, and offset from adjacent top of bench slope at the disposal site. Post-Construction survey of the Disposal site shall include full topographic survey of the benches used for disposal adequate for producing a terrain model wiht a level of accuracy to plot two-foot contours to verify satisfactory placement of material. Boundaries of material placement at the disposal site shall be used for quantities of item Disposal Area Seeding, measured by Square Yard (SY).

Post-Construction survey shall also be conducted for the levee according to the same requirements as described for the Pre-Construction Survey, including a high resolution video.

The government will not be responsible for any input into the process of developing Post-Construction surveys. The survey shall be used to verify the quantity of excavation , quantities for items measured by square yard (SY), and other non-channel clearing Option items measured by the Cubic Yard (CY). after all work is complete.

CAD generated cross sections and average end area shall be used to compute the total quantity of material removed from the channel. Final surveys shall be the basis from which any disputes over changes in conditions shall be made. Promptly after completing the survey, the Contractor shall furnish all originals of all field notes and all other records relating to the survey to the Contracting Officer. The Contractor shall retain copies of all such material furnished to the Contracting Officer. The Contracting Officer must agree to any deviations from the template section prior to payment. Progress payments on partially completed work shall not be made without the data required by this paragraph. Determination of such quantities will be made by the Contracting Officer.

3.1 GENERAL REQUIREMENTS

Establish and maintain an effective quality control (QC) system that complies with FAR 52.246-12 Inspection of Construction. QC consist of

SECTION 01 45 00 Page 3 plans, procedures, and organization necessary to produce an end product which complies with the Contract requirements. The QC system covers all construction operations, both onsite and offsite, and be keyed to the proposed construction sequence. The project superintendent will be held responsible for the quality of work and is subject to removal by the Contracting Officer for non-compliance with the quality requirements specified in the Contract. In this context the highest level manager responsible for the overall construction activities at the site, including quality and production is the project superintendent. The project superintendent maintains a physical presence at the site at all times and is responsible for all construction and related activities at the site, except as otherwise acceptable to the Contracting Officer.

3.2 CONTRACTOR QUALITY CONTROL (CQC) PLAN

Submit an outline for the Contractor Quality Control (CQC) Plan, proposed to implement the requirements FAR 52.246-12 Inspection of Construction, no more than 30 calendar days after contract award. Submit the full quality control plan so that Government approval is obtained no more than 60 calendar days after issuance of the NTP. Construction will be permitted to begin only after acceptance of the CQC Plan or acceptance of an interim plan applicable to the particular feature of work to be started. Work outside of the accepted interim plan will not be permitted to begin until acceptance of a CQC Plan or another interim plan containing the additional work.

3.2.1 Content of the CQC Plan

Include, as a minimum, the following to cover all construction-operations, both onsite and offsite, including work by subcontractors fabricators, suppliers and purchasing agents:

a. A description of the quality control organization, including a chart showing lines of authority and acknowledgment that the CQC staff will implement the three phase control system for all aspects of the work specified. Include a CQC System Manager that reports to an individual in the corporate quality control chain of commmand and at least one level above the site superintendent (See specific requirements in Paragraph 3.4.2 CQC System Manager).

Letters of direction to all other various quality control representatives outlining duties, authorities, and responsibilities will be issued by the CQC System Manager. Furnish copies of these letters to the Contracting Officer.

d. Procedures for scheduling, reviewing, certifying, and managing submittals, including those of subcontractors, offsite fabricators, suppliers, and purchasing agents. These procedures must be in accordance with Section 01 33 00 SUBMITTAL PROCEDURES.

e. Control, verification, and acceptance testing procedures for each specific test to include the test name, specification paragraph

SECTION 01 45 00 Page 4 requiring test, feature of work to be tested, test frequency, and person responsible for each test. (Laboratory facilities approved by the Contracting Officer are required to be used.)

f. Procedures for tracking preparatory, initial, and follow-up control phases and control, verification, and acceptance tests including documentation.

g. Procedures for tracking construction deficiencies from identification through acceptable corrective action. Establish verification procedures that identified deficiencies have been corrected.

h. Reporting procedures, including proposed reporting formats.

i. A list of the definable features of work. A definable feature of work is a task which is separate and distinct from other tasks, has separate control requirements, and is identified by different trades or disciplines, or it is work by the same trade in a different environment. Although each section of the specifications can generally be considered as a definable feature of work, there are frequently more than one definable features under a particular section. This list will be agreed upon during the coordination meeting.

3.2.2 Acceptance of Plan

Acceptance of the Contractor's plan is required prior to the start of construction. Acceptance is conditional and will be predicated on satisfactory performance during the construction. The Government reserves the right to require the Contractor to make changes in the Contractor Quality Control(CQC) Plan and operations including removal of personnel, as necessary, to obtain the quality specified.

3.2.3 Notification of Changes

After acceptance of the CQC Plan, notify the Contracting Officer in writing of any proposed change. Proposed changes are subject to acceptance by the Contracting Officer.

3.3 COORDINATION MEETING

After the Preconstruction Conference, before start of construction, and prior to acceptance by the Government of the CQC Plan, meet with the Contracting Officer and discuss the Contractor's quality control system.

Submit the CQC Plan a minimum of 15 calendar days prior to the Coordination Meeting. During the meeting, a mutual understanding of the system details must be developed, including the forms for recording the CQC operations,, control activities, testing, administration of the system for both onsite and offsite work, and the interrelationship of Contractor's Management and control with the Government's Quality Assurance. Minutes of the meeting will be prepared by the Government, signed by both the Contractor and the Contracting Officer and will become a part of the contract file. There can be occasions when subsequent conferences will be called by either party to reconfirm mutual understandings or address deficiencies in the CQC system or procedures which can require corrective action by the Contractor.
