DESCRIPTION OF SERVICES

This statement of work describes contractor requirements for the project titled “FACILITY 205– ASPHALT REPAIR AND WIDEN ROAD -FLIGHTLINE”. All work will occur at the Fresno Air National Guard Station, Fresno, California. The contractor is responsible to furnish and install all labor and materials to complete the items listed in this Statement of Work. The contractor shall provide the government a complete and usable product that will be inspected/accepted by the government at the completion of the project. All information included in this document, along with the instructions and standards referenced in the document, should be considered as part of the Statement of Work.

SITE OF WORK

All work will occur at the Fresno Air National Guard Station 5323 McKinley Ave, Fresno, CA. 93727

STATEMENT OF WORK

1. Objectives: The purpose of this project is to repair 1' - 5' wide upheaval of asphalt caused by movement of the apron, and widen road access over the SE corner of BLDG 100 by 5’.

2. Asphalt Repair

2.1. Demolition

2.1.1. Demo asphalt specified on map. The map will indicate how wide and length of demo asphalt required. Properly dispose of all debris off 144th FW property. Will need disposal ticket. (Map will be provided on solicitation or at site visit)

2.2. Directions

2.2.1. Will be using existing subgrade. If more subgrade is needed contractor will provide base rock 1/2- or 3/4-inch

2.2.2. Level to grade

2.2.3. Compaction test will meet 95% compaction and be tested every 100’ with documentation

2.2.4. Apply tack coat to walls of the asphalt that was demoed specified on map. (Map will be provided on solicitation or at site visit)

2.2.5. Asphalt should be placed in 2 layers at 1.5” of asphalt per layer resulting in 3” thick asphalt.

2.2.6. Asphalt should be AR-8000 grade. A vacuum steam refined straight run asphalt for use in hot mixes in warmer temperature climates or under heavier load conditions.

3. Widen Road

3.1. Demo base to widen road by 5’ and replace 4” of subgrade using base rock 1/2- or 3/4inch rock

3.2. Level to grade to existing grade with proper drainage

3.3. Compact. Compaction test will meet 95% compaction and be tested every 50’ with documentation

3.4. Apply tack coat to walls of existing walls of pavement

3.5. . Asphalt should be placed in 2 layers at 1.5” of asphalt per layer resulting in 3” thick asphalt.

3.6. Asphalt should be AR-8000 grade. A vacuum steam refined straight run asphalt for use in hot mixes in warmer temperature climates or under heavier load conditions.

CONTRACTOR LICENSE REQUIREMENTS

1. The contractor will possess a valid California “C-12” license and be able to provide proof of competence in the area of work. The contractor may self-perform sections of the project.

2. All listed sub-contractors working for the contractor will possess the applicable California Contractors license. Refer to the Project Specific Requirements section for the required license from the Contractor.

PROJECT SPECIFIC REQUIREMENTS

1. All subcontractors working for the contractor will possess the applicable California Contractors license. For reference, the contractor digs, moves, and places material forming the surface of the earth, other than water, in such a manner that a cut, fill, excavation, grade, trench, backfill, or tunnel (if incidental thereto) can be executed, including the use of explosives for these purposes. This classification includes the mixing, fabricating and placing of paving and any other surfacing materials C-12 licenses.

2. The project will be completed no later than 60 working days post Notice to Proceed (NTP). The contractor and government will negotiate the final phasing (if applicable), and schedule during a review of the project at the pre-construction conference.

3. Materials. The following lists out materials requirements for this project. Contractor may request substitutes for an approved equal during the pre-construction conference. All substitution requests shall be made in writing on the AF Form 3000. Requests for material substitutions made after the pre-construction conference may not be granted by the Government.

c. Base Rock for subgrade

GENERAL REQUIREMENTS

1 When required, the Contractor will attend a Pre-Construction Conference. The PreConstruction Conference will generally be scheduled after award but prior to the NTP. At a minimum, the contractor shall have their project manager and site superintendent attend the meeting. The Contractor will provide the following documents at the Pre- Construction Conference

a. Project Schedule

b. Contract Schedule of Values

c. Safety Plan

d. Quality Control Plan

e. Material Submittals

f. Material Safety Data Sheets

g. Draft DD 1354 – Final draft to be submitted at conclusion of project 2 Government Furnished Property and Services. The government will provide water, sewer and electrical connections. Unsecure outdoor storage area at the base is available and must be requested through the project manager prior to utilizing. Secured storage is the responsibility of the contractor.

3 Quality Control. Contractor shall develop and maintain a quality program to ensure work is performed as described in this Statement of Work and in accordance with all local, state and federal rules.

4 Quality Assurance. The Government will maintain a Quality Assurance Program to ensure work is performed as described in this Statement of Work and in accordance with all local, state, and federal rules. The Project Manager will manage the Government’s QA program.

5 Hours of Operation. The hours of work shall be between 7:00 a.m. and 5:00 p.m. Monday through Friday, and rotating Mondays unless otherwise prescribed by this statement of work. Work shall not be performed on Weekends and Federal Holidays without prior coordination from the Project Manager.

6 Base Access. Base security personnel require that all personnel entering the installation are cleared for access. The contractor will provide first name, last name, driver’s license number and applicable vehicle data for all personnel accessing the base. The contractor shall submit this information to the base Contracting Office NLT two working days prior to the date of required access. Personnel must maintain a valid driver’s license, vehicle registration and proof of insurance at all time while on the base.

7 The Contractor, and all listed sub-contractors, must be licensed within the State of California. Refer to the Project Specific Requirements section for the required license from the Contractor.

8 Technical Competence and Ability. If requested by the Government, the Contractor shall provide proof of technical competence and experience in the area of expertise required by this Statement of Work. The contractor shall be prepared to submit documents providing proof of successful competence such as training certificate or accreditations from recognized industry leaders. Contractors shall also be prepared to provide proof of successful project experience through references of former clients in related project types.

9 Dimensions and Quantity. All dimensions and quantities provided in the Statement of Work are estimates only. They are provided as a courtesy for contractors to estimate the magnitude of work or materials necessary to complete a task. They are not intended to be final measurements. The Contractor is responsible for verifying all dimensions and quantities in the field prior to starting work.

10 Submittals. All submittals will be transmitted with an AF Form 3000. When submitting on a finish, texture or other material characteristic where a photograph is unacceptable, the Contractor will provide a sample of the material for review and approval along with the product cut-sheet.

11 Warranty. The contractor shall guarantee all workmanship and materials covered under this statement of work for a period of no less than one (1) year from the date of project acceptance. This contractor’s warranty shall be included with any manufacturer’s warranties that accompany materials or systems installed as part of this statement of work.

SUBMITTALS

All submittals will be transmitted with an AF Form 3000. When submitting on a finish, texture or other material characteristic where a photograph is unacceptable, the Contractor will provide a sample of the material for review and approval along with the product cut-sheet.

CLOSEOUT DOCUMENTATION

10% Retainage will be released upon receipt of the following items:

· Contractor workmanship letter of warranty up to one year.

· Certificates of no debts to suppliers and vendors.

· Electronic pictures showing the area before and after repair.

· Recycling or Landfill proof of disposition document (if available).

· All applicable O&M manuals "Contractor Manpower Reporting The Contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the Air Force collection site. The Contractor is required to completely fill in all required data fields using the following web address: http://www.ecmra.mil/.

Reporting inputs will be for the labor executed during the period of performance during each Government fiscal year (FY), which runs October 1 through September 30. While inputs may be reported any time during the FY, all data should be reported no later than October 31 of each calendar year.
