PROJECT TDVG 182002

Construct Asphalt Walking Path South of River Club Rd.

A. PART I - General A1. Purpose: In general terms, the intent of this project is to construct an asphalt walking path at the Alpena CRTC to include but not limited to:

- The base bid shall include site work, aggregate subbase, gravel base course, and 2” HMA surface for a 16,800 SF walking path (approximately 2100’ x 8’) area.

- The walking path shall be constructed to support light vehicular traffic to include sedans pickups and vans.

The walking path shall be sloped to drain with MDOT requirement for positive drainage. The walking path shall be as level as possible and sloped transversely to prevent ponding. The asphalt walking path shall have mixture design and constructed using current MDOT standard specifications for construction.

- The base bid shall include site work aggregate subbase, gravel base course, and 2” HMA bituminous surface for a 16,800 SF (approximately 2100 LF), 8 FT wide. To connect the existing walking path north of River Club Rd. with parking lot and sidewalk at Building 132. The walking path shall be created to support typical vehicles to include sedans, pickups and vans. The walking path shall be crowned and sloped to drain with the minimum MDOT requirement for positive drainage. The roadway shall have a mixture design and constructed using current MDOT standard specifications for construction.

- The base bid shall include 4” wide solid, traffic white, lines for walking path designation at the north end of Building 132 parking lot and roadway crossings at River Club Rd.

- Tree, vegetation and spoil removal/grubbing.

- See the attached proposed plan view with area dimensions. The 2” HMA surface and 23A aggregate are references. But the final product shall be determined by the Contractor to meet all the requirements of this SOW.

- Contractor is responsible in obtaining a Soil Erosion Permit and implementing required management practices.

- The wage schedule shall be based on Alpena County.

- No design is due at the time of the bids. The contractor shall submit the lump sum costs for FOR BOTH

THE BASE AND OPTIONAL LINE ITEMS AND PROVIDE A NARRATIVE OF HOW THE CONTRACTOR

INTENDS TO MEET THE REQUIREMENTS OF THIS CONTRACT AND TIMELINE. The offeror shall use a either the GANTT chart or an equivalent critical path scheduling approach and the schedules shall be graphically represented.The Contractor shall demonstrate how they will meet the deadline for the project completion date.

- The final product shall be based on an allowable soil bearing pressure of 1,500 psf. If suitable soils are not found at the elevations, excavate and replace with engineered fill as directed by the design engineer. No borings shall be required.

- Known elevations for this proposed area are not known other than data through the USGS. Surveying shall be the responsibility of the contractor. There will be some earthwork (cut and fill) and grading for the contractor to perform. Quantities are unknown and will be determined by the Contractor to meet the requirements of this SOW.

A1b. Option : Furnish and install (17) exterior bollard lights along the campground fence. Bollard lights to be comparable to type and light levels of the existing bollard lights along River Club Rd.

A2. Goals: Contractor shall provide all plant, labor, materials, and supervision associated with the drafting and execution of the construction for the above titled project. The final product must conform to good engineering practice and employ techniques readily available in the local construction industry. Project shall be constructed as a complete and usable facility and be within the anticipated cost range as noted in paragraph A5 below. Existing site and facility as-built drawings can be provided by the government at the contractor’s request.

1. PLACE AND PERIOD OF PERFORMANCE: Work will be performed at the Alpena-CRTC, Alpena, Michigan. The work shall be construction completed no later than (60 days from notice to proceed). Normal work hours are 0730 to 1630 Monday through Friday. Work outside these hours, days and on federal holidays shall be approved by the Contracting Officer.

A.3 Access and General Protection/Security Policy and Procedures The Alpena-CRTC is a closed facility, with controlled access. The contractor shall comply with all applicable installation/facility access and local security policies and procedures, which may be obtained from the Contracting Officer's Representative (COR). The contractor shall also provide all information required for background checks to meetInstallation, access requirements to be accomplished by the local installation's Security Forces. The contractor shall ensure compliance with all personal identity verification requirements as directed by DoD, HAF and/or local policy. Should the Force Protection Condition (FPCON) change, the Government may require changes in contractor security matters or processes.

Contractor and all associated sub-contractors employees shall comply with adjudication standards and procedures using the National Crime Information Center Interstate Identification Index (NCIC-III) and Terrorist Screening Database (TSDB) (AFI 31-101 and AFI 10-245), applicable installation, facility and area commander installation/facility access and local security policies and procedures (provided by government presentative), or, at OCONUS locations, in accordance with status of forces agreements and other theater regulations.

The contractor will submit Contractor EAL’s for all contractor employees and subcontractor employees, requiring access to Government Installations, Facilities and Controlled Access areas shall complete AT Level I awareness training. AT Level I Awareness Training will be conducted when personnel report to Pass and ID to receive their base access credentials. Personnel will sign the awareness and a copy will be maintained with the Contractor EAL.

. All contractor employees, to include subcontractor employees, requiring access Government Installations, Facilities and Controlled Access areas shallcomplete AT Level I awareness training within 10 calendar days after contract start date or effective date of incorporation of this requirement into the contract,whichever is applicable. AT level I Awareness training is conducted at the Pass and ID office upon receipt of access credentials for the installation

A4. Governing Standards

All work shall conform to the following standards:

1. Safety: USACE 385-1-1 standards apply. Prior to beginning work, the Contractor shall submit a site safety plan specifically tailored to this project.

2. The Contractor shall refer to and follow specifications set forth in UFC 1-200-01 and ANGETL 15-01 (available only by e-mail), ANGETL 10-03 and ANGETL 32-1018 shall be followed. 2012 MDOT standards shall be adhered to. If there is a question as to use state or federal codes, the strictest code applies. This project is on federal property.

A5. Definitions A. Definitions

1. Contracting Officer: The services to be performed under this contract are subject to the general supervision, direction, control and approval of the Contracting Officer.

2. Contracting Officers Representative (COR): The Contracting Officer's Representative responsible for administering contracts under the immediate direction of the Contracting Officer.

3. Contractor: This term, as used herein, refers to the contractor under this contract.

B. Selection Procedure - During the review of offers the owner may ask for additional information. The owner may initiate action to award a contract at any point after review of the offers. Therefore, offers should reflect the offeror’s best terms both from a technical and cost standpoint. See FAR 52. 215-1, Instructions to Offeror’s - Competitive Acquisition (Jan 2004.)

C. Schedule Objectives – Performance period 60 days from NTP

A5. Cost Range The anticipated cost range for this project is between $25,000.00 and $100,000.00.

A6. Pre-Proposal Site Visit A pre-proposal site visit will be held on a date determined by the Contracting Officer at the Alpena-CRTC, Alpena, Michigan. All contractors, consultants, subcontractors, manufacturers and suppliers are invited to attend.

The purpose of the meeting is to allow prospective bidders to view the project site. A base escort will meet bidders and present the site, answer general questions about the installation, but will NOT answer questions regarding the project. All technical questions shall be submitted in writing, by general contractors only, in the form of an RFI to the Contracting Officer, MSgt Steven Stocking via e-mail at steven.a.stocking3.mil@mail.mil. Courtesy copy RFI’s to the Contracting Officer’s Representative, Capt. Brian Blumline, via e-mail at brian.g.blumline.mil@mail.mil.

The Contracting Officer will issue the suspense date for RFI submissions.

A7. Selection Criteria and Weightings A. Evaluation Factors The Best Value offer will be determined on the basis of the lowest priced proposal meeting the acceptability standards for non-priced evaluation factors.

Non-Priced Evaluation Factors listed below were evaluated on an acceptable /unacceptable basis:

B. Cost Proposal - Offerors are advised that if the technical proposals are essentially equal, award will be made on the basis of the lowest cost.

B. PART II - RESPONSIBILITIES

B1. Contractor:

The RFP documents are intended to define existing conditions, certain required items, and parameters to be included in the project. It is the contractor’s responsibility to complete the documents and construction in a manner consistent with the intent of the RFP documents within the required time period.

C. PART III PROPOSAL REQUIREMENTS: Refer to "Instructions to Offerors".

D. PART IV - POST AWARD REQUIREMENTS

D1. Construction Document Preparation:

A. Submissions:

1. The contractor shall prepare and submit complete construction documents to include final drawings and specs for review and comment for approval by the government in accordance with standard professional practice, Unified Facilities Criteria, ANG Engineering Technical Letters and MDOT.

2. The contractor shall allow a minimum of five (5) working days for government reviews

B. Electronic Media:

1. Submit technical proposals, drawings, as-builts, sketches, etc. on CD after the project is constructed.

2. Record drawings shall be completed in AutoCAD version 2007 or later and in Acrobat PDF.

C. Approved Construction Documents:

1. The final construction document submission package will be submitted by the contractor for approval by the government after completion of the 95% review for the final package to be submitted by the contractor. The government will have 5 days to take approval action. The shop drawings will be certified by a Registered Architect (R.A.) and/or Professional Engineer (P.E.) licensed in Michigan.

D. Construction Specifications: Project specifications shall include specifications for all products, materials, equipment, methods, and systems shown on the construction drawings in accordance with standard professional practice and this RFP. The specification submitted for review shall include:

1. The name of the manufacturer, the product name, model number, or other identification as appropriate to clearly identify the product that will be used in the construction of this project.

2. Other data as appropriate to clearly identify the product that will be used in the construction of the project i.e. shop drawings, product data, and samples as required by the RFP documents; and E. Requirements:

1. Project shall be in accordance with standards and codes listed in Section A-3 of this RFP.

2. The contractor shall provide prompt, written notification to the Contracting Officer concerning conflicts with, or recommended deviations from codes, laws, regulations, standards, and opinions of review officials as described above. No work altering the scope of this contract shall be undertaken prior to receipt of written approval from the Contracting Officer.

D2. Construction Period Submittals A. Upon 100% shop drawing approval, the contractor shall distribute a total of 3 sets of the approved construction documents prepared by the contractor to the government. One full size set and two half size sets are required. Two sets of bound specifications shall be delivered on 8 1/2” x 11” paper. Any supplemental information and calculations shall be turned into the government.

B. Other submittals - The contractor shall submit all products, test results, certificates, manufacturer’s instructions, manufacturer’s field reports, etc. as required by the approved specifications. A waste management plan, safety plan, hazardous material usage form, AF Form 103, AF Form 3064 and AF Form 3065 shall be submitted. All material and product submittals shall be accompanied by an AF Form 3000.

C. Shop drawings and submittals - The contractor shall check subcontractor's shop drawings, detail drawings, schedules, descriptive literature and samples, testing labor-laboratory reports, field test data and review the color, texture and suitability of materials for conformity with the RFP Documents and construction documents.

D3. Project Close-Out The contractor shall provide the following project close-out documents:

1. As-built record drawings in AutoCAD and PDF formats of actual work in place; O&M documents, and warranties. Submittals shall meet the requirements of ANG ETL 32-1018

2. DD Form 1354, Real Property Information

3. The contractor shall have a closeout preparation meeting at the construction 75% point to ensure the project is on track for proper closeout.
