2. SCOPE OF WORK – BASE CONTRACT

The work consists of furnishing all plant, labor, materials, supplies, and accessories required to accomplish the sampling and well installation, together with all other operations incidental to the work, in strict accordance with this Scope of Work and subject to the terms and conditions of the contract. All equipment and supplies as specified herein are subject to approval by the Contracting Officer’s Representative (COR) prior to issuing Notice to Proceed. No portion of this contract may be subcontracted without prior approval from the Government. The Contractor shall have a copy of this Scope of Work while performing the contract work and shall be familiar with the applicable provisions governing the contract work.

The Contractor shall provide qualified personnel to complete the following major work items:

Prepare an Accident Prevention Plan (APP) Retrieve groundwater and soil samples Install and develop two deep monitoring wells

Work items and required submittals are described in detail below. The Contractor shall perform all work as specified.

2.1 Submittals

2.1.1 Accident Prevention Plan

The Contractor shall develop a site-specific Accident Prevention Plan (APP) meeting the requirements of EM 385-1-1 01.A.11 (http://www.publications.usace.army.mil/). The APP shall be submitted for approval prior to starting work. The plan shall include the following, as a minimum: project point-of contact list, emergency telephone numbers, directions to the nearest hospital(s), a list of emergency equipment to be maintained at the work site (e.g., first aid kit, eye wash bottle, fire extinguisher, cell phone, etc.), and activity hazard analyses (AHA) for all major work items. Major work items include, but are not limited to, mobilizations, drilling and sampling operations. It is not expected that the Contractor will encounter any free-phase contamination during drilling or well installation. It is anticipated that OSHA Level D protective equipment will be adequate.

The APP shall identify on-site personnel who shall ensure that all contractor and subcontractor personnel perform all work in accordance with the site APP. A minimum of one personnel with this responsibility shall be onsite at all times that field work is ongoing, by either the contractor or any subcontractors. The USACE will provide a copy of an existing approved APP for drilling at the DSCP site for reference purposes. No work shall be performed until this plan has been accepted by the Government and no deviation from the accepted plan will be permitted without prior approval by the Government.

2.1.2 Drilling Log

The Contractor shall submit complete, legible copies of DRILLING LOG, ENG FORM 1836 (or approved equivalent) upon completion of the work. All logs shall be recorded during the actual performance of the work and shall be preserved in good condition and order by the Contractor until they are delivered and accepted. Geological classification of the soils are not required on the drilling logs; this will be completed by a USACE geologist. Surveyed coordinates of the drilling location are also not required.

2.2 Soil Borings and Well Installation

2.2.1 Prior to Boring Placement

Utility Markout USACE will contact Pennsylvania One Call/811 prior to any earth-disturbing activities.

Pennsylvania One Call will notify the owners of underground facilities of the scheduled drilling.

Additionally, USACE will conduct an independent utility markout to avoid conflicts with underground utilities.

Site Access Since portions of the site are open and accessible to members of the public, site security cannot be guaranteed by USACE. Contractor personnel are responsible for security of their machines, equipment, supplies, etc. The Contractor shall also limit public access to the work site during field activities using cones, tape, or other means. Right of Entry (ROE) will be established legally and in writing by the government.

Permits, Certifications and Licenses The contractor shall comply with all Federal, State and local laws, regulations, ordinances and permits relating to the performance of this work. Bidders shall be licensed water well drillers in the Commonwealth of Pennsylvania. Street closure permits have been provided by the City of Philadelphia for the boring and well installations. Park permits, if needed, will also be acquired by the government. The contractor shall, at their own expense, procure any additional required permits, certifications and licenses required by Federal, State, and local law for the execution of this work. All monitoring well installation and related work shall be in accordance with Pennsylvania Department of the Environment, City of Philadelphia Department of Public Health and the Philadelphia Streets Department.

Figure 2. Generalized geologic section, South Philadelphia (USGS, 1991)

2.2.2. Sample Collection and Well Installation

The Driller shall install two new groundwater monitoring wells near Pattison Avenue and 20th Street in Philadelphia, Pennsylvania. The Contractor shall select either mud rotary or sonic drilling methods. The approximate locations of the proposed wells are shown on Figure 1. The Contractor shall coordinate the final location with the USACE Point of Contact and provide notification 5 days prior to the commencement of the well installation operations.

The Driller shall drill a pilot borehole at each of the proposed well locations. The purpose of the pilot borehole is to establish the stratigraphy, locate the desired aquifer and screening interval, and collect soil and groundwater samples. The Contractor may drill the pilot borehole for sample collection, and later ream to 6.5 inches to accommodate a two-inch well. Potable water shall be used in the construction and development of the wells.

It is expected that the driller shall advance through the Trenton Gravel and encounter the lower sand unit at approximately 70 feet BGS. Once the top of the aquifer is identified, a soil sample shall be collected with split spoons to confirm the elevation of the top of the aquifer. The driller shall retrieve the soil sample and USACE staff will bottle it and ship it to the lab for analysis.

Soil sample collection shall be repeated on ten (10) foot intervals until the bottom of the deep aquifer or refusal is reached. It is expected that the driller shall encounter bedrock at approximately 120 ft BGS, as shown in Figure 2. The driller shall not proceed into solid rock.

After the first soil sample is collected, a Hydropunch® (or equivalent instrument) shall be advanced two feet further into the borehole to collect a groundwater sample for analysis.

Groundwater sample collection shall also be repeated on ten (10) foot intervals until the bottom of the deep aquifer or refusal is reached. USACE will provide all bottle ware and handle sample shipping and analysis.

The results of the groundwater analysis will be used to select the well screen locations. Once all the samples from the first borehole are collected, USACE staff will immediately sent them to the laboratory for characterization on an expedited 24-hour turnaround from the lab. After reviewing the lab results, a 15-foot long well screen shall be installed at the aquifer level where the highest VOC concentrations are detected. This screened interval will be determined in the field by the USACE personnel who will be on-site and/or available by phone. Table 1 indicates the proposed designation and approximate depths for the wells, however the actual final depth of the well and screened interval will be determined in the field based upon the information obtained from the pilot borehole and sample analysis. Bidders shall assume a 120 foot depth for cost estimating purposes.

The riser shall be 2-inch diameter, PVC Schedule 40 material with flush threaded joint fittings.

The driller shall have on hand or readily available, a selection of commonly used filter pack materials and screen slot sizes that are compatible with the naturally occurring aquifer stratigraphy. Previous well installations in the same formation have utilized a 0.02 inch screen slot size. The secondary filter pack (choker sand), just above the primary sand filter pack, is required and shall consist of #00 filter sand.

Filter pack and a choker sand separation layer shall be placed as indicated on the typical monitoring well installation detail shown in Figure 3. In accordance with ASTM D 5092, all sand shall be added via tremie pipe to control the speed of placement and prevent bridging.

Placement by gravity or free fall methods is not allowed. Neat cement grout shall be placed in the annular space above the very fine choker sand separation layer. Grout shall also be injected under pressure through a tremie pipe to reduce the chance of voids. Neat cement grout shall cure a minimum of 48 hours before well development begins. Wells shall be installed within 8-inch diameter, flush-mounted, H-20 traffic load rated vaults. Locking well caps shall be provided and the well vault covers shall be secured with screws. The Contractor shall repair and revegetate any ruts or other damage caused by the drill rig in accordance with City of Philadelphia specifications. If a well is placed in a city sidewalk, the concrete sidewalk shall be restored to the original condition following well installation.

A tape measure shall be used to verify and document the final dimensions (top and bottom) and thickness of all sand, grout, and other materials added during the construction of the well. The driller shall also verify, using a tape measure, the depth of the bottom of the borehole and the bottom of the well after it is installed.

2.2.3 Well Development

The development shall be accomplished by surging and pumping methods, in accordance with ASTM D 5521 Standard Guide for Development of Groundwater Monitoring Wells in Granular Aquifers. Air surging and jetting are prohibited. The method of development shall be approved by the USACE Point of Contact. The well development shall be conducted until the groundwater obtained from the well has a sufficiently low turbidity (less than 50 NTU) using the low-flow procedure, or until a maximum of 4 hours of total redevelopment pumping time has been incurred. The minimum time of development shall be two hours of continuous surging.

The turbidimeter used for this shall be calibrated properly each day that it is used. Development water shall be containerized. The USACE POC shall be notified immediately if any well does not attain the required turbidity or if any well does not provide sufficient amounts of water to be developed. The Contractor shall complete a record of the well development operations conducted at each well location and present the results on the Well Development Record.

2.2.4 Investigation Derived Waste (IDW)

All cuttings, drilling fluids/mud, development water, decontamination water and other investigation derived waste shall be containerized and exported from the project site for disposal.

The contractor is responsible for transportation and disposal of all cuttings, drilling fluids/mud, development water, decontamination water and other investigation derived waste (IDW). No IDW shall be placed on the site grounds near the borehole nor in surrounding streets and sewers.

USACE staff will collect a sample of the IDW for waste characterization and furnish analytical results to the Driller. Limited space is available for temporary storage of containerized IDW at the DSCP project office, located at 3000 South 20th Street, Philadelphia, PA. USACE will furnish analytical results of waste sample within three weeks of well installation. The Driller shall completely dispose of all IDW within three weeks of receiving the analytical results.

2.2.5 Decontamination

All augers, tooling and other down-hole equipment that has contacted subsurface soil shall be decontaminated between drilling locations by hot water pressure washer. All downhole equipment shall be decontaminated prior to mobilization to the site. If decontamination is conducted onsite, the Contractor shall construct a temporary decontamination pad. The Contractor shall assume that potable water is not available at the site and provide a decontamination water source. The Contractor shall dispose of decontamination water, residual soil and decontamination pad materials upon completion of work as described above.

3. SCOPE OF WORK – OPTIONS

52.212-1 INSTRUCTIONS TO OFFERORS--COMMERCIAL ITEMS (OCT 2016)

(a) North American Industry Classification System (NAICS) code and small business size standard. The NAICS code and small business size standard for this acquisition appear in Block 10 of the solicitation cover sheet (SF 1449). However, the small business size standard for a concern which submits an offer in its own name, but which proposes to furnish an item which it did not itself manufacture, is 500 employees.

(b) Submission of offers. Submit signed and dated offers to the office specified in this solicitation at or before the exact time specified in this solicitation. Offers may be submitted on the SF 1449, letterhead stationery, or as otherwise specified in the solicitation. As a minimum, offers must show--

(1) The solicitation number;

(2) The time specified in the solicitation for receipt of offers;

(3) The name, address, and telephone number of the offeror;

(4) A technical description of the items being offered in sufficient detail to evaluate compliance with the requirements in the solicitation. This may include product literature, or other documents, if necessary;

(5) Terms of any express warranty;

(6) Price and any discount terms;

(7) "Remit to" address, if different than mailing address;

(8) A completed copy of the representations and certifications at FAR 52.212-3 (see FAR 52.212-3(b) for those representations and certifications that the offeror shall complete electronically);

(9) Acknowledgment of Solicitation Amendments;

(10) Past performance information, when included as an evaluation factor, to include recent and relevant contracts for the same or similar items and other references (including contract numbers, points of contact with telephone numbers and other relevant information); and

(11) If the offer is not submitted on the SF 1449, include a statement specifying the extent of agreement with all terms, conditions, and provisions included in the solicitation. Offers that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration.

(c) Period for acceptance of offers. The offeror agrees to hold the prices in its offer firm for 30 calendar days from the date specified for receipt of offers, unless another time period is specified in an addendum to the solicitation.

(d) Product samples. When required by the solicitation, product samples shall be submitted at or prior to the time specified for receipt of offers. Unless otherwise specified in this solicitation, these samples shall be submitted at no expense to the Government, and returned at the sender's request and expense, unless they are destroyed during preaward testing.

(e) Multiple offers. Offerors are encouraged to submit multiple offers presenting alternative terms and conditions or commercial items for satisfying the requirements of this solicitation. Each offer submitted will be evaluated separately.

(f) Late submissions, modifications, revisions, and withdrawals of offers:

(1) Offerors are responsible for submitting offers, and any modifications, revisions, or withdrawals, so as to reach the Government office designated in the solicitation by the time specified in the solicitation. If no time is specified in the solicitation, the time for receipt is 4:30 p.m., local time, for the designated Government office on the date that offers or revisions are due.

(2)(i) Any offer, modification, revision, or withdrawal of an offer received at the Government office designated in the solicitation after the exact time specified for receipt of offers is “late” and will not be considered unless it is received before award is made, the Contracting Officer determines that accepting the late offer would not unduly delay the acquisition; and--

(A) If it was transmitted through an electronic commerce method authorized by the solicitation, it was received at the initial point of entry to the Government infrastructure not later than 5:00 p.m. one working day prior to the date specified for receipt of offers; or

(B) There is acceptable evidence to establish that it was received at the Government installation designated for receipt of offers and was under the Government's control prior to the time set for receipt of offers; or

(C) If this solicitation is a request for proposals, it was the only proposal received.

(ii) However, a late modification of an otherwise successful offer, that makes its terms more favorable to the Government, will be considered at any time it is received and may be accepted.

(3) Acceptable evidence to establish the time of receipt at the Government installation includes the time/date stamp of that installation on the offer wrapper, other documentary evidence of receipt maintained by the installation, or oral testimony or statements of Government personnel.

(4) If an emergency or unanticipated event interrupts normal Government processes so that offers cannot be received at the Government office designated for receipt of offers by the exact time specified in the solicitation, and urgent Government requirements preclude amendment of the solicitation or other notice of an extension of the closing date, the time specified for receipt of offers will be deemed to be extended to the same time of day specified in the solicitation on the first work day on which normal Government processes resume.

(5) Offers may be withdrawn by written notice received at any time before the exact time set for receipt of offers.

Oral offers in response to oral solicitations may be withdrawn orally. If the solicitation authorizes facsimile offers, offers may be withdrawn via facsimile received at any time before the exact time set for receipt of offers, subject to the conditions specified in the solicitation concerning facsimile offers. An offer may be withdrawn in person by an offeror or its authorized representative if, before the exact time set for receipt of offers, the identity of the person requesting withdrawal is established and the person signs a receipt for the offer.

(g) Contract award (not applicable to Invitation for Bids). The Government intends to evaluate offers and award a contract without discussions with offerors. Therefore, the offeror's initial offer should contain the offeror's best terms from a price and technical standpoint. However, the Government reserves the right to conduct discussions if later determined by the Contracting Officer to be necessary. The Government may reject any or all offers if such action is in the public interest; accept other than the lowest offer; and waive informalities and minor irregularities in offers received.

(h) Multiple awards. The Government may accept any item or group of items of an offer, unless the offeror qualifies the offer by specific limitations. Unless otherwise provided in the Schedule, offers may not be submitted for quantities less than those specified. The Government reserves the right to make an award on any item for a quantity less than the quantity offered, at the unit prices offered, unless the offeror specifies otherwise in the offer.

(Applies to all offers exceeding $3,500, and offers of $3,500 or less if the solicitation requires the Contractor to be registered in the System for Award Management (SAM) database.) The Offeror shall enter, in the block with its name and address on the cover page of its offer, the annotation ``Unique Entity Identifier'' followed by the unique entity identifier that identifies the Offeror's name and address. The Offeror also shall enter its Electronic Funds Transfer (EFT) indicator, if applicable. The EFT indicator is a four-character suffix to the unique entity identifier. The suffix is assigned at the discretion of the Offeror to establish additional SAM records for identifying alternative EFT accounts (see subpart 32.11) for the same entity. If the Offeror does not have a unique entity identifier, it should contact the entity designated at www.sam.gov for unique entity identifier establishment directly to obtain one. The Offeror should indicate that it is an offeror for a Government contract when contacting the entity designated at www.sam.gov for establishing the unique entity identifier.

(k) System for Award Management. Unless exempted by an addendum to this solicitation, by submission of an offer, the offeror acknowledges the requirement that a prospective awardee shall be registered in the SAM database prior to award, during performance and through final payment of any contract resulting from this solicitation. If the Offeror does not become registered in the SAM database in the time prescribed by the Contracting Officer, the Contracting Officer will proceed to award to the next otherwise successful registered Offeror. Offerors may obtain information on registration and annual confirmation requirements via the SAM database accessed through https://www.acquisition.gov.

52.212-2 EVALUATION--COMMERCIAL ITEMS (OCT 2014)

(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The Government intends to award to the lowest priced offeror that conforms to the Statement of Work.

(b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).

(c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
