SECTION 1
EXECUTIVE SUMMARY
Tank 2 (Facility 20005) is located at Hancock ANG in Mattydale, New York. Tank 2 is a welded, steel, vertical aboveground Jet-A (F-24) storage tank that was fabricated and erected by Pittsburg Tank &
Tower Company in 1996 in accordance with (IAW) American Petroleum Institute (API) 650, 9th Edition, Appendix E, F and H. Tank 2 is 27’-0” in diameter and 28’-3” high with a nominal capacity of 2,500 barrels (BBL). The in-service inspection was performed on 22 July 2015. Results of this inspection will provide recommended repairs and upgrades required to comply with applicable industry and military standards.
Testing and inspection were performed IAW applicable sections of API 653, 5th Edition. Contained in this report is a detailed account of the tank inspection, a summary of the non-destructive testing (NDT), and conclusions derived from the inspection and testing. Findings from this inspection are further detailed in Section 4 of this report. Repairs and upgrades identified in this section are further classified and listed in Section 5 of this report.
Foundation and Containment
Tank 2 is located in a concrete containment dike lined with a thick-mil coating. Approximately 118 linear feet of the thick-mil coating exhibits cracking along the concrete dike perimeter. The tank perimeter rests on a 27” high concrete ring wall foundation. The foundation is equipped with thirteen (13) equally spaced seismic anchors and six (6) grounding lugs and cables. All grounding cables are in good condition with no signs of damage or detachment.
The tank bottom is provided with a leak detection system via four (4) equally spaced tell-tale pipes along the concrete ring wall.
Tank Shell
The tank shell consists of four (4) butt-welded A-36 steel shell courses. External coating on the tank shell is in good overall condition with no indications of coating failure. The tank is provided with standardized product designation IAW Military Standard (MIL-STD)-161G; however, the tank is not provided with a National Fire Protection Agency (NFPA) emergency responder diamond.
Shell nozzles consist of three (3) 36” manways, one (1) 10” product receipt, one (1) 12” main suction, one
(1) 4” low suction, two (2) 4” aqueous film-forming foam (AFFF) discharge penetrations, six (6) 1” penetrations for the high-high level alarm (HHLA) sensor, low level alarm (LLA) sensor, high level alarm
(HLA) sensor, and high level control valve (HLCV) shut-off switch chamber, one (1) 1” threaded plug, and one (1) 1” water draw-off. Tell-tale holes for manway (MW) 1, MW2, and MW3 reinforcement pads are plugged. All three (3) tell-tale holes should be unplugged to aid in determining if weeping has occurred between the tank shell and reinforcement pad.
External coating for the HLCV body and the product receipt, main suction, and low suction double block and bleed (DB&B) valves exhibit approximately 4 ft2 of flaking, staining, and surface corrosion. Transfer
1-2 piping for the AFFF discharge chamber is capped off. No action is required as Tank 2 is provided with a full contact, aluminum honeycomb internal floating roof (IFR). The IFR provides adequate fire protection for tanks storing Jet-A.
Thermal relief valves (TRV) for the product receipt and main suction lines are mounted horizontally with non-standard crossover piping. At this current configuration, there is a possibility that both TRVs will not provide adequate relief in the event of over pressurization.
Fixed Roof
The fixed roof is of lap-welded, steel, cone-up construction. External coating on the fixed roof is in good overall condition with no indications of coating failure. The fixed roof is equipped with six (6) perimeter fiberglass patches of various sizes.
Fixed roof nozzles consist of two (2) 3” penetrations for the IFR anti-rotation cable, one (1) 8” automatic tank gauge (ATG)/gauging hatch, one (1) 30” manway, one (1) 4” blind flange, one (1) 72” x 24” inspection hatch, two (2) 4” tie-offs, one (1) 12” center vent, and one (1) 3” penetration for the IFR grounding cable.
External coating at the 4” blind flange exhibits approximately 1 ft2 of flaking, staining, and surface corrosion.
The tank is provided with an aluminum honeycomb IFR. The IFR is provided with a secondary peripheral wiper seal. Visually from the fixed roof manway, the secondary peripheral wiper seal is in good overall condition with no signs of damage or compromise. The primary seal type and condition is unknown due to limited access.
Gauges and Alarms
The tank is equipped with a roof mounted Cameron ATG system for automatic tank gauging. The tank is equipped with an 8” manual gauging hatch for tank gauging operations. The 8” gauging hatch is provided with a fully slotted stilling well and the Cameron ATG system is integrated with the 8” gauging hatch.
The tank is provided with a shell mounted LLA sensor, HLA sensor, and HHLA sensor. For overfill protection, there is a shell mounted HLCV shut-off switch chamber and a HLCV at the product receipt.
The tank is not provided with a liquid level gauge for monitoring liquid level from the ground level.
Cathodic Protection
The tank bottom is provided with an impressed current cathodic protection (ICCP) system. The ICCP rectifier was on during the inspection; however, the panel voltmeter and ammeter read zero voltage and amperage indicating that the tank bottom is not cathodically protected.

SECTION 5
RECOMMENDATIONS
5.1 Repair and Upgrade Recommendations
The following subsections outline the itemized tank repairs/upgrades.
5.1.1 Mandatory Repairs Required to Remove Tank from Service
Mandatory repairs are repairs required to maintain hydraulic and structural integrity over the required 5-year in-service (external) inspection interval.
No mandatory repairs required to remove tank from service.
5.1.2 Repairs Required to Fully Comply with API 653 / 650
Recommendation 1: Unplug tell-tale holes at MW1, MW2, and MW3 reinforcement pads. Perform a pressure test at each reinforcement pad to verify internal weld integrity. Provide the unplugged tell-tale holes with heavy grease to prevent water ingress between the reinforcement pad and shell plate.
5.1.3 Repairs Required to Fully Comply with UFC 3-460-01 / 3-460-03
Recommendation 2: Troubleshoot the ICCP system.
Recommendation 3: Provide the tank with a liquid level gauge calibrated in 1/16” increments. The liquid level gauge should be mounted approximately 60” above walking surface.
Recommendation 4: Mount two (2) TRVs at the product receipt and main suction lines vertically per
UFC and manufacturer’s requirements.
Recommendation 5: Recoat approximately 118 linear feet of thick mil coating along the containment dike perimeter.
Recommendation 6: Provide approximately 5 ft2 of spot coating repair at the HLCV body, the product receipt, main suction and low suction DB&B valves, and the fixed roof 4” blind flange.
Recommendation 7: Replace the Cameron ATG system with an Enraf ATG system and VITO temperature/water combination probe. The Enraf ATG and VITO combination probe should be provided with fully slotted stilling wells. The VITO combination probe should be located directly above the tank sump, or at the lowest feasible point.
Recommendation 8: Provide the tank with an emergency responder diamond IAW NFPA 704.
5-2
5.1.4 Recommended Repairs As Best Management Practice
Repairs required as Best Management Practice are recommended to extend the tank service life and maintain product quality, but are not necessarily part of a design or inspection standard.
No recommended repairs are required as BMP.
5.2 Inspection Intervals
The interval between tank inspections (both internal and external) should be determined by the tank service history. Inspection intervals can be adjusted based on known corrosion rates. Corrosion rates can only be determined when a comparison is made between measurements taken over an interval of time. The previous API 653 inspection report was not available for review and comparison.
Listed below are the recommended inspection intervals per API 653:
• Routine monthly inspections to monitor the external condition of the tank and associated piping.
Gross defects, leaks, shell distortions, signs of settlement, and corrosion should all be monitored along with the foundation condition (includes tank chime to foundation interface), coatings, and appurtenances. The fixed roof should be visually inspected for signs of damage.
• Formal visual external inspection by an API certified inspector every 5 years or RCA/4N years
(where RCA is the difference between the measured shell thickness and the minimum required thickness and N is the shell corrosion rate), whichever is less. Based on the review of shell and roof thickness measurements, the 5-year interval will govern the next formal external visual inspection.
• Ultrasonic thickness measurements of the shell plates can be performed with the tank in service and can be scheduled at a maximum interval of fifteen (15) years or RCA/2N, whichever is less.
Based on shell thickness measurements, the recommended interval for the UTT measurements on the tank shell is fifteen (15) years.
• Bottom corrosion rates govern the internal inspection interval. Per API 653, the next internal inspection should be performed within twenty (20) years of the previous inspection; however, UFC
3-460-03 requires a maximum internal inspection interval of ten (10) years. With no previous inspection records, it is recommended to perform the next internal inspection immediately. In the event that the previous inspection records are found, the next internal inspection should be performed within ten (10) years of the previous internal inspection. MW1 is stenciled with a cleaning date of 13 May 2009; however, no inspection date was identified.
The recommended inspections and intervals are:
Routine In-Service Inspection by Facility Personnel: Monthly
External Inspection by Authorized Inspector: Five (5) Years; July 2020
External Inspection by Authorized Inspector w/ UTT: Fifteen (15) Years; July 2030
Internal Inspection by Authorized Inspector: Immediately or Ten (10) Years

Results
The maximum measured out-of-plane deflection is 0.031 inches and the maximum allowable deflection is 0.027; therefore, the tank is not within API 653 acceptable limits. The allowable deflection is not accurately represented by the above analysis due to the small tank diameter and arc length. It is recommended to monitor the tank settlement in future inspections.
