John Day Project has various types of FE in multiple locations. The bulk of FE equipment is located on project in the various offices/crews, powerhouse, vehicles, boats and cranes. FE inventory is identified by specific locations. The COR will coordinate date/time that contractor will be on-site, in an effort to make mobile units (vehicle/crane) reasonably available to the contractor. Annual Maintenance shall include all of the monthly inspection specs, plus: change out annual tag, re-inventory all extinguishers and provide the government an updated service record within 30-days of annual inspection. Annual maintenance service is intended to give maximum assurance that an extinguisher will operate effectively and safely.
Annually, the Contractor shall perform maintenance procedures that include a thorough examination listed above and additionally the three basic elements of an extinguisher:
• Examination of mechanical parts
• Extinguishing agents
• Expelling means.
All service shall be performed in accordance with the most current NFPA 10 Standard. Any FE found to be out of compliance with NFPA 10 shall be immediately replaced with a compliant FE of the same type.

• Operating instructions on nameplate legible and face outward.
• Seal and tamper indicators not broken or missing.
• Determine fullness by weighing or “hefting” (for extinguishers without pressure gages).
• Examine for obvious physical damage, corrosion, leakage, or clogged nozzle.
• Marked with their letter (class of fire) and numeric (relative extinguishing effectiveness) classification
• Tags, pins, and tamper indicators shall be replaced as necessary
• With each inspection, extinguisher tags shall be dated with month/day/year and initialed by the inspector.
• No less than 25 days or no more than 40 days may pass between inspections, with an inspection required and documented in all 12 months.
All service shall be performed in accordance with the most current NFPA 10 Standard. Any fire extinguisher found to be out of compliance with NFPA 10 shall be immediately replaced with a compliant fire extinguisher of the same type.

Hydrostatic Test Interval for Fire Extinguishers
Extinguisher Type Test Interval (Years)
Carbon Dioxide 5 Dry Chemical, Stored Pressure with Mild Steel Shells
NOTE: By January 15th of the base year and option year, the Contractor is responsible for providing the Project COR a list of upcoming hydro testing quantities to be performed during the next option year.

5.2 Task 2 – Carbon Dioxide (CO2) 75 lbs Bank Services
Main Unit (MU) generators are protected by Kidde – Fenwal, Cardox fixed CO2 high-pressure cylinder bank systems. A CO2 bank is composed of pilot and slave cylinders. Pilot cylinders are equipped with “squibs” that explosively activate a spring-loaded valve to release the stored gas. As the high-pressure gas is released from the pilot cylinders into the header it causes valves on slave cylinders to open and discharge the remaining gas into the generator barrel.
NOTE: Squibs are provided by the government.
5.2.1 Carbon Dioxide (CO2) 75 lbs System Bank Semi-annual Service
Per NFPA 12, CO2 cylinders must be weighed semi-annual to verify that proper volume of gas is available. Containers showing a loss in net content of more than 10% percent must be refilled or replaced. Each bank system is set on a semi-annual critical preventative maintenance schedule.
Contractor shall obtain hard-copies of all critical preventative maintenance from COR and perform inspection/documentation as described under job task service procedure. Contractor is required to arrange Safe Clearance Lock-Out Tag-Out through project Operations as per critical job tasks.
Upon successful completion of critical preventative maintenance, Contractor shall sign and date all critical PM’s and return to the COR.
5.2.2 Carbon Dioxide (CO2) 75 lbs Emergency Bottle Replacement
All emergency replacement shall occur under the same process as critical preventative maintenance in reference to Safe Clearance Lock-Out Tag-Out procedures and coordination with project Operations. In case of a bank discharge, the Corps of Engineers employees and/or contractor can change the bottles. Contractor shall be notified by COR of replacement necessity. Replacement bottles shall be installed within 48 hours, from time of notice, if not previously agreed upon by the Contractor, COR and Project Operations. Discharged bottles shall be removed, serviced, recharged and returned to project within ten (10) working days.

All service shall be performed in accordance with the most current NFPA 12 Standard and 49 Code of Federal Regulations (CFR). All services shall be performed by trained/certified operator and conducted offsite at an approved facility.
Safe handling procedures must be followed when transporting CO2 cylinders. Co2 cylinders should be constructed, and marked in accordance with U.S. Department of Transportation (DOT) specifications. All services should be followed to Federal, State, and Local regulations.
NOTE: By January 15th of the base year and option year the Contractor shall be responsible for providing the Project COR a list of upcoming hydrostatic testing quantities to be performed during the next option year.
5.2.4 Carbon Dioxide (CO2) 75 lbs I-Valve
During the hydrostatic testing service that is conducted offsite, the I-valve needs to be inspected for damage, which can include but not limited to; safety disc washer, pilot check, gasket, and sleeves.
Any damage to the thread obsolete models, and/or if you deem unsafe need to be replaced. All removed valves shall be turned into the COR.
5.2.5 Carbon Dioxide (CO2) 75 lbs Bottle Recharge Service
Any bottle found undercharged or discharged shall be recharged/refilled. Consecutive recharge is based on use of CO2 bottles. Based on manufacturing date the Contractor shall provide, a minimum of six (6) months prior, notice of End-of-Service life for each Emergency Breathing Apparatus to the COR to ensure funding is allocated for replacement equipment.
5.3.1 Emergency Breathing Apparatus (EBA) Annual Service
Contractor shall disassemble each EBA and escape pack and inspect each component (face piece, hose, regulator, and air bottle) to ensure they are in good working order. Inspection tags shall be replaced during annual maintenance. The annual maintenance will be conducted according to Manufacturer Guidelines as listed in product service manual. Contractor shall provide written and accredited Breathing Air Certificates from certified test facility at least semi-annually.
5.3.2 Emergency Breathing Apparatus (EBA) Monthly Service
Contractor shall inspect EBA apparatus to ensure:
• All inspections performed according to manufacturer service manual or representatives requirements. They are filled to capacity – pressure gauges indicate full pressure.
• They are located as directed by the COR.
• Access to EBA is unobstructed.

If a deficiency is found, this information shall be noted in writing and sent to the COR. Each bottle shall be isolated and pressure checked. If a particular bottle is less than 2000 psi it shall be removed from the bank and replaced with a full bottle. Each bottle shall be individually tagged with the date of inspection and pressure of the bottle monthly. All pressures shall be recorded on the service records and forwarded to the COR.
5.5 Task 5 – Medical Equipment Service
There are numerous medical equipment items throughout the project which include: Automated External Defibrillator (AED), Emergency Shower (ES), and Emergency Eye Wash Stations (EW).

The contractor is required to train the Operations section in 3 separate sessions to cover all shifts. Contractor shall provide documentation to POC/COR for all of the above noted training sessions.
6.0 Performance Requirements Summary (PRS)
The PRS is intended to ensure that the Government receives the services for which it contracted. The Government will only pay for services received. The Performance Requirement Summary (PRS) determines if the contractor meets the performance standards of the contract, as well as provides guidelines for how and when surveillance will be performed. It ensures timeliness, effectiveness, and that the contractor is delivering the results specified in the contract. Government contract quality assurance will be performed at such times and/or places as may be necessary to determine the services conform to contract requirements.
The contractor service requirements are summarized into performance objectives that relate directly to mission essential items. The performance threshold briefly describes the minimum acceptable quality levels (AQL) of service required for each requirement. These thresholds are critical to mission success.
