PERFORMANCE WORK STATEMENT

1. GENERAL REQUIREMENT:

a. The Contractor shall furnish, all necessary labor, transportation, equipment, materials/supplies, and supervision to perform non-personal custodial services in the manner, location and frequency set forth in the following paragraphs and schedules. Daily work shall be performed at the Sam Rayburn Power Plant (SRPH), located at the Sam Rayburn Dam, approximately 15 highway miles from the town of Jasper, Texas and one afternoon per week at the R.D. Willis Power Plant (RDW), located approximately 14 miles to the west of the town of Jasper, Texas. The distance between the Sam Rayburn Power Plant and the R.D.Willis Power Plants via Road 255 / State Hwy 63 / FM1747 / FM777 is approximately 28 road miles.

b. The work is to be performed at the Power Plants and adjacent, parking areas and access roads. Since many of the areas to be serviced are frequented by the public, very high standards of performance shall be required of the Contractor.

c. The R.D. Willis Power Plant is an unmanned and normally unattended facility, therefore to gain access to perform janitorial services at RDW. The contractor shall coordinated for access one afternoon per week ether on Wednesday or Thursday. Arrangements must be made to work at RDW on ether day of the week. Coordination will need to be done on a case by case basis. All work at RDW shall be performed between the hours of 1:00 p.m. and 4:30 p.m. unless otherwise approved. The Contractor shall be required to provide sufficient personnel to perform all scheduled work at RDW within the 3.5 hour time frame allowed each week. Government personnel will be available at the RDW site, on the date service is to be performed, and to provide access to the power plant and perform contract inspection.

d. Work at Sam Rayburn Power Plant shall be performed five days per week normally, (Monday thru Friday) except on Government Holidays and weekends.

e. These specifications are written to identify both generic and specific work items. Where conditions are noted to be unique to a specific power plant, a site identifier is provided to that location by SRPH or RDW. Much of this specification is of a general nature and applies to all work performed by this contract, regardless of location. Unless a specific site identifier is shown, (SRPH or RDW) the contractor must consider the work is to be performed at both power plant locations.

2. PERFORMANCE TIMES:

a. The Contractor shall perform all work on SRPH between the hours of 8:00 a.m. and 2:00 p.m. Monday through Friday except on Federal holidays. The Contractor shall plan the Daily and weekly work schedules in such a manner as to complete all work within the SRPH conference room, its associated restrooms, and administrative (clerk’s) office and entrance foyer prior to 10:00 a.m. each day.

b. All services to be performed at RDW shall be performed each Wednesday (or Thursday upon approval by COR) between 1:00 p.m. and 4:30 p.m. of each week except on Federal holidays. This time frame shall be strictly limited due to unavailability of access to RDW at other days and times.

c. The Contractor shall provide and maintain an up-to-date list of all employees who are working under this contract. All employees shall “sign in” their names and arrival time in the Sam Rayburn Control Room before starting work and “sign out” with their names and departure time when the work is completed each day at the SRPH. On the day when work is performed at RDW the Contractor shall also “sign in” and “sign out” of the Janitorial log located on the Control Room desk at the RDW. In addition to signing in and out at RDW, the Contractor shall notify Government personnel who are within the facility when arriving and when departing the facility

3. SUPERVISOR AND EMPLOYEE QUALIFICATIONS:

a. The Contractor shall have a representative on-site (i.e. On-site Supervisor), at all times work is in progress, who has authority to act on his behalf. The Contractor shall be required to designate, in writing, to the QA the name of the individual who has authority to act on his behalf.

b. The Contractor shall ensure that all employees are capable and demonstrate adequate knowledge of chemicals, tools, equipment and techniques necessary to perform the work. The Government may require the Contractor to discontinue using any employee determined by the Government to be unsatisfactory. In the acceptance or rejection of work by the Government, no allowance will be made for lack of skill of personnel. Loitering or unnecessary interference by the contract personnel with the work of Government personnel shall be sufficient cause for the Government to require the discontinuance of use of any specified persons in the performance of this contract.

c. The Power Plants are “SMOKE FREE” environments and no smoking is permitted within the power plants. A designated “SMOKING AREA” is located outdoors near the main entrances of each Power Plant.

d. The work requires that Contractor personnel be physically able to climb multiple floor levels using stairs.

a. The Contractor shall immediately report to the KO or his authorized representative any unusual and/or potentially hazardous conditions which are observed during the performance of work.

b. It shall be the responsibility of the Contractor to notify the KO or his representative of facilities that require maintenance beyond the scope of the contract, i.e., electrical, carpentry, and plumbing repairs. The Contractor shall initiate and deliver to the KO or his representative completed Trouble Report Forms (Government furnished) on such troubles within 8 hours of noting the problem. If a problem is noted which may damage equipment, materials or building in a short time frame, immediate notification of the KO or his representative is required with follow-up of a trouble report in writing.

14. PROTECTION OF EXISTING STRUCTURES, UTILITIES, AND WORK:

The Contractor shall:

a. Protect against accidental and/or intentional operation of any system.

b. Protect all existing structures, utilities, and work of any kind against damage or interruption of Service which may result from operations of the Contractor.

c. Allow only persons directly involved with accomplishment of the Contractor's operations (i.e.

no friends, family, guests, children, or pets) in the Power Plant.

d. Notify the KO or his representative immediately of damage to Government and private property and injury to any person resulting from his operation.

1) None of the kitchen facilities located in SRPH or RDW will be available for use by the contract personnel;

however, the contractor may provide a microwave and/or coffee makers for their employee use at locations approved by the KO or his representative.

f. "NO TOUCH" POLICY FOR CONTROLS WITHIN THE POWER PLANT:

There are numerous areas which will require the exercise of caution while cleaning. The control room has many control knobs and levers which must not be touched by anyone except those authorized to do so. This is of great importance for the protection of Power Plant equipment.

When doing any cleaning of areas which have control panels, the Contractor must exercise caution and assure that the controls are not accidentally operated by bumping the control panel.

The Contractor is directed to not touch any controls on the control boards for any purpose. The fronts of control panels will be cleaned by Government personnel familiar with the control circuitry. The relay panels at the rear of the control board shall be dusted by the Contractor per Part II, Para.1q. The "NO TOUCH" policy shall extend to the distribution centers throughout the Power Plants.

g. During any wet mopping operations, the Contractor shall assure that all access to wet or potentially slippery areas are posted with temporary and portable free standing signs and that they are placed appropriately to warn personnel and visitors of potentially hazardous areas. Signs appropriate for the hazard such as "CAUTION-WET FLOOR" for wet floor areas shall be used. These signs shall be furnished by the Contractor and shall remain in place until the floor is dry. The Contractor shall remove and properly store all signs prior to leaving the facility.

15. DISPOSAL OF WASTE

a. Wastewater Disposal Requirements:

1) The SRPH has a self-contained sewage disposal system which is capable of handling typical detergents and wastewaters. Clear and soapy waste water may be poured into the drains in the Janitors closets and other specially designated drains. Plant personnel will identify the appropriate drains for disposal of mop and rinse waters within the SRPH at the beginning of contract work. Use of inappropriate drains could result in contamination of the river waters and is prohibited. Any waters which contain cleaning agents, soaps, waxes or strippers shall be poured into storage barrels as directed at the SRPH for disposal by the Government.

2) The RDW does not have a sewage disposal system available for disposal for detergents, therefore, all mopping operations conducted at RDW shall be performed using “HOT” water and using no detergents and no waxes. On infrequent occasions when RDW floors must be cleaned using detergents, Government personnel will perform the “spot cleaning” and transport the associated detergent wastewater to the SRPH facility for disposal. The Contractor is specifically prohibited from using detergents in mop water used at the RDW facility. Based upon the prohibition against use of detergents, any mop water used for the cleaning of the RDW floors will be uncontaminated “soiled water” and will be directly poured into the drains of the RDW power plant and subsequently discharged directly into the river without contamination.

b. Solid Waste Disposal Requirements:

1) SRPH has a solid waste dumpster located approximately 2500 feet from the Sam Rayburn Powerhouse located adjacent to the Project Office. These dumpsters are for use by the Contractor. These dumpsters shall be used by the Janitorial Contractor for all solid waste except for any materials separated by Government personnel for recycling. The Contractor shall perform transportation in order to properly dispose of solid waste into the dumpsters.

Any future requirements pertaining to trash separation/recycling transportation will be performed by separate contract and are not included in this contract.

2) The RDW facility has trash barrels located outside of the Power Plant as well as inside the facility, which are for use in disposal of the solid waste and debris. Each day that work is performed at the RDW facility, the Contractor shall remove all trash and debris from the facility and transport it to the nearby Sandy Creek Park (approximately 2 miles from Plant) and enter the park to dispose of the trash in the Government provided dumpster located near the restroom facility. In the event that debris is too large for the barrels, the Contractor shall notify GQAR or Government personnel to arrange for removal of the large objects.

16. KEY CONTROL

The Contractor shall establish and implement adequate methods of ensuring that all keys issued to the Contractor by the Government are not lost or misplaced and are not used by unauthorized persons. All keys (and access cards) shall be issued by the Government upon signature of each contract employee. No keys issued may be duplicated.

The Contractor shall report the occurrence of a lost key immediately to the KO or his authorized representative.

Cost of replacing lost or destroyed keys issued to the Contractor shall be paid by said Contractor. It is the responsibility of the Contractor to prohibit the use of keys issued by the Government by any other persons other than the Contractor's employees. The Contractor shall prohibit the opening of locked areas by the Contractor's employees to permit entrance of persons other than the Contractor's employees engaged in the performance of assigned work in those areas. No access to the Power Plants will be permitted except during approved work hours.

All keys issued to Contractor personnel shall be returned to the Government issuing office before final payment will be made on this contract.

17. SUPPLIES, UTILITIES AND EQUIPMENT FURNISHED BY THE GOVERNMENT

The Government shall furnish, at no cost to the Contractor, the following materials and usage of equipment:

a. Water and electric power necessary to the performance of the work required.

b. Storage space, as designated, for storage of Contractors’ materials and equipment. Additional storage locations shall be provided, as the need arises, for ladders and other items which may be needed on the job site. The Contractor's equipment shall be stored in specified areas and not left out after duty hours.

c. Disposal dumpster for all collected garbage at SAM RAYBURN POWER PLANT. This dumpster shall be located at the Project Office near the outer parking area. Contractor shall transport waste from the Power Plant up to the dumpster adjacent to the Project Office on a daily basis. R. D. WILLIS POWER PLANT trash shall be placed in waste container barrels located outside of the R. D. WILLIS POWER PLANT facility then transported to the closed available dumpster in Sandy Creek Park.

18. MATERIAL AND EQUIPMENT:

a. The Contractor shall provide all cleaning supplies, materials, and equipment necessary to comply with the provisions of this contract (i.e., mops, dust mops, clean mop heads, mop buckets, scrub brushes, brooms, germicidal disinfectants, scouring powders, bowl cleaners, deodorizers, detergents, dust mop treatments, window cleaning supplies, waxes, buffers and buffing pads, carpet shampoo machines, fiberglass ladders, vacuum cleaners) as required. No government owned equipment/property will be made available for use by the contractor.

b. All supply items which are provided by the contractor shall be of high quality. Any supplies which are deemed inferior by the KO or his representative shall be brought to the attention of the Contractor and subsequently replaced by the Contractor with a suitable product.

c. A list of all material and equipment to be used, along with the SDS sheets, shall be submitted for approval by the KO at the pre-work meeting. Following are definitions for the major types of cleaning agents and supplies. It is not the intent of the following definitions to imply that these are the only materials necessary to comply with the provisions of the contract. All equipment provided by the Contractor shall be clearly marked to identify ownership prior to being brought on-site.

1) CLEANING AGENT - Biodegradable soap, soap compounds, and/or biodegradable detergents shall be used. If the Contractor uses a cleaning compound containing disinfectants and/or deodorants in lieu of applying these materials separately, the label must state that the compound is biodegradable and contains disinfectants and/or deodorants. The manufacturer's instructions shall be followed in mixing the proper strength solution for application. Carpet cleaning solutions to be used in Carpet Shampoo machines shall be specifically formulated for such use by a commercial carpet shampoo manufacturer.

2) GERMICIDAL DISINFECTANTS - Only standard commercial germicidal disinfectants shall be used. The manufacturer's instructions shall be followed in mixing the proper strength solution for application. The disinfectants may be mixed with the cleaning solution if the manufacturer's instructions on the disinfectant's label do not prohibit it.

3) DEODORIZER - Deodorant shall be a material packaged and marketed for use as a deodorizer. The manufacturer's instructions shall be followed in mixing the proper strength for application. The deodorant may be mixed with the cleaning solution of the manufacturer's, if instructions on the deodorant's label do not prohibit it.

4) TOILET TISSUE - Toilet tissue shall be no less than 2 ply tissue.

5) DEODORANT CAKES - Cake shall not contain paradichlorobenzene. The cake shall be furnished in non-rusting hangers, if hangers are required.

6) WASTE RECEPTACLE LINERS - Liners must be polyethylene of 0.002 inch minimum thickness for wastebasket size and 0.005 inch thickness for 30 gallon size.

7) LIQUID HAND SOAP - Liquid hand soap shall be manufactured for use in mechanical Soap dispensers and shall be biodegradable and phosphate free.

8) PAPER TOWELS - Paper towels required shall be provided to fit the two types of paper towel dispensers presently installed in the facilities. Single fold dispensers as designed and Roll paper towel dispensers are also installed adjacent to the single fold towel dispensers; therefore, the Contractor shall also assure both types of dispensers are re-filled with the proper style of paper towel refills.

9) FLOOR FINISH REMOVER - Floor finish remover shall be a cleaner and stripper appropriate for use with the floor finish the Contractor uses.

10) FLOOR FINISH - Floor finish shall be a self shine floor finish suitable for buffing for restoration of floor finish. Experience in floor care in the Power Plant has been that floor care systems formulated for high speed buffers have provided satisfactory results.

11) FLOOR SEALER - Floor sealer shall be a clear, high luster, scuff resistant finish suitable for use on quarry tile and composite tile floors which shall be compatible with the floor finish used.

12) VACUUM CLEANERS - The vacuum cleaner must be equipped with HEPA Filtration, does not require replacement bags, and be designed to provide reduced noise levels in the office environments where they will be used.

13) FLOOR BUFFERS - The Contractor shall provide a minimum of two (2) floor buffing machines for use in the Power Plant, (one for use on the Control Room/Office level and one for the use on the lower level floors on which buffing is required). This Contract requires buffing of several floor areas on differing levels in the Power Plant. Manually carrying of a buffer between floors via the stairwell is impractical and unsafe and will not be permitted. Experience in care of the composite tile, ceramic tile and quarry tile floors has shown that High Speed Buffers (greater than 1200 RPM) used in conjunction with compatible high speed floor finish systems provide satisfactory results with substantial reductions in maintenance labor over conventional speed buffers and floor finish systems.

14) LADDERS- The Contractor shall provide a sufficient number of fiberglass ladders, of proper height, for use on each floor of the Power Plant to fulfill the requirements for high dusting, lamp replacement and other cleaning. No wooden or aluminum ladders shall be permitted within the Powerhouses. Transporting or carrying of ladders greater than six (6) feet via the stairwells will not be permitted due to safety considerations. The Government will provide the services of a crane on infrequent occasions for use of the overhead crane for lowering ladders and other equipment through the floor hatches upon advance notification. The Contractor shall be required to notify the KO or his representative of the request for use of the overhead crane 24 hours in advance of anticipated need.

15) AEROSOL DEODORANT SPRAY- Spray deodorants shall be a commercial type aerosol can of pleasant odor to remove unpleasant odors.

16) WET VACUUM CLEANERS - The Contractor shall provide a large capacity wet vacuum for use in vacuuming of stripper water and wax water from the floors during stripping operations and for use in the vacuum cleaning of upholstery, under furniture and for use in the elevator cab. A small portable vacuum may also be required for the cleaning of overhead light fixtures and cabinet tops. Feather dusters are not to be used in the powerhouses since they do not adequately capture dust.

17) CARPET SHAMPOO MACHINES- The Contractor shall provide one or more carpet shampoo machines in order to shampoo all carpets in the Power Plant as directed.

19. INSPECTION AND ACCEPTANCE:

a. Work will be inspected regularly by Government personnel and the Contractor will be advised as soon as practical of any deficiencies found. Upon being verbally advised of a deficiency, the Contractor shall take immediate action to correct such deficiency and to ensure the deficiency does not re-occur. Deficiencies that cannot be corrected prior to the next scheduled service for the area involved, or for which a satisfactory explanation has not been presented by the Contractor, will be subject to a reduction in payment. Inspections will be based on the daily work schedule to assure that inspection closely follows the actual maintenance of the facility or area.
