PERFORMANCE WORK STATEMENT

GREASE TRAPS CLEANING SERVICES

Fort Huachuca, Arizona

As of 30 January 2018

A. PURPOSE

A.1. The purpose of this contract is to remove all grease, dirt, or any other foreign material from Grease Traps. The term of the contract is for one year, with the option for renewal for each of the following four years.

B. THE CONTRACTOR SHALL PROVIDE TRAINED EMPLOYEE(S) TO:

B.1. Provide a competent person to oversee all aspects of the work being preformed, safety, quality control, disposal of waste materials, and clean-up.

B.1.1. The manager, competent person and alternate must be able to read, write, speak, and understand the English language.

B.2. Operations Security (OPSEC). The Contractor shall comply with DoD Directive 5205.02, Army Regulation 530-1, and the requiring activity OPSEC program. The Contractor shall ensure all contractor employees and subcontractors performing work under this contract complete Level I OPSEC training within 30 calendar days of start of employee performance on this contract and annually thereafter. The Contractor shall maintain all OPSEC training records and shall provide copies to the COR upon request.

B.2.1. AT Level I Training. AT Level I Training. All contractor employees, to include subcontractor employees, requiring access to Army installations, facilities, and controlled access areas shall complete AT Level I awareness training within 60 calendar days after contract start date or effective date of incorporation of this requirement into the contract, whichever is applicable. The contractor shall submit certificates of completion for each affected contractor employee and subcontractor employee to the COR or to the KO, if a COR is not assigned within 30 calendar days after completion of training by all employees and subcontractor personnel. The contractor is provided the following information: CAC HOLDERS: Level I Antiterrorism Awareness Training course (JS-US007-14) is now hosted on https://Jkodirect.jten.mil. Login and Search for the course on the Course Catalog tab via the number or key word, enroll, and Launch. FOR NON-CAC HOLDERS: A standalone version of the course for Non-CAC holders is also available at this link http://jko.jten.mil/courses/atl1/launch.html.

B.2.2. iWATCH Training. This standard language is for contractor employees with an area of performance within an Army controlled installation, facility or area. The contractor and all associated sub-contractors shall brief all employees on the local iWATCH program (training standards provided by the requiring activity Anti-Terrorism Officer). This locally developed training will be used to inform employees of the types of behavior to watch for and instruct employees to report suspicious activity to the COR. This training shall be completed within 45 calendar days of contract award and within 10 calendar days of new employees commencing performance with the results reported to the COR NLT 10 calendar days after contract award.

B.2.3. Contractor and all associated sub-contractors employees shall comply with applicable installation, facility and area commander installation/facility access and local security policies and procedures (provided by government representative). Contractor and all associated sub-contractors employees shall provide all information required for background checks to meet installation access requirements to be accomplished by installation Provost Marshal Office, Director of Emergency Services or Security Office. Contractor workforce must comply with all personal identity verification requirements (FAR clause 52.204-9 or NAF Clause BI.142, Personal Identity Verification of Contractor Personnel) as directed by DOD, HQDA and/or local policy. In addition to the changes otherwise authorized by the changes clause of this contract, should the Force Protection Condition (FPCON) at any individual facility or installation change, the Government may require changes in contractor security matters or processes.

B.2.4. Contractor and all associated sub-contractors employees shall comply with adjudication standards and procedures using the National Crime Information Center Interstate Identification Index (NCIC-III) and Terrorist Screening Database (TSDB) (Army Directive 2014-05/AR 190-13), applicable installation, facility and area commander installation/facility access and local security policies and procedures (provided by government representative).

C. GENERAL

C.1. Scope of Work. The contractor shall furnish all labor, materials, equipment, personnel, supervision and transportation necessary to provide cleaning service in accordance with paragraph G during normal work hours for all grease traps listed at Section G.1. All work areas shall be left clean and all debris shall be removed after completion of work. Performance shall be in accordance with the terms and conditions of this contract and with the current local, state, and federal regulations.

C.2. Damaged Items. All defective components, parts, and materials shall be reported in writing to the Contracting Officer’s Representative (COR). Any items damaged by the Contractor shall be replaced by the Contractor at no cost to the Government.

C.3. Location of work is at Fort Huachuca, Arizona.

C.4. Occupational Safety and Health. The Contractor shall comply with the provisions in Occupation Safety and Health Program, as well as the Occupational Safety and Health Administration (OSHA) Act, Part 1960.

C.5. The Contractor shall contain, clean up, decontaminate and dispose of hazardous/toxic spill(s) waste occurring on the site. The Contractor shall be required to divert his work force as necessary to support spill cleanup operations in the event of a large spill (over 5 gallons or in excess of 100 square feet of surface coverage). The Contractor shall report to the Contracting Officer and Fort Huachuca Environmental Compliance Management Branch by telephone immediately after notification or discovery of a large spill and within 30 minutes after notification or discovery of small spills (less than 5 gallons or less than 100 square feet of surface coverage) that contain hazardous or toxic substances.

D. DEFINITIONS

D.1. Normal Work Hours. All work shall be conducted during the hours of 7:00 a.m. to 3:30 p.m., Monday through Friday, excluding federal holidays, as set forth in Title 5, United States Code (USC) 6103 (a). Work accomplished outside this time frame shall require prior approval of the Contracting Officer.

D.1.1. Federal Holidays.

D.2. Quality Assurance. These actions taken by the Government to assure it is receiving the services for which it has contracted. The Contracting Officer’s Representative (COR) or designated representative shall evaluate these services acting as Quality Assurance Evaluator (QAE).

D.3. Quality Control. Those actions taken by the contractor to assure the Government receives the services for which it contracts and those services meet the requirements of the contract.

E. GOVERNMENT-FURNISHED ITEMS

E.1. The Government will furnish water and electricity as required for the work to be performed.

F. CONTRACTOR-FURNISHED ITEMS

F.1. Contractor shall provide all equipment or material required to complete job as stated in paragraph G.

F.2. Certification. Contractor must provide proof that the firm has been engaged in inspection and maintenance services for the aforementioned items and has the qualifications both in terms of expertise and having proper and adequate equipment to accomplish the work as required.

F.3. Personnel. The Contractor shall provide the name and phone number of the Project Manager to the Contracting Officer within ten (10) calendar days after award of the contract, and no later than five (5) calendar days prior to effective date of change. The Project Manager shall be responsible for the supervision, overall administration, and coordination of the required services and be available during normal hours of operation. The Project Manager shall be empowered to act with full authority to enable him to make decisions to resolve the issues that arise in the day-to-day operations without undue delay.

F.4. Quality Control Plan (QCP). Quality Control is the responsibility of the contractor. The contractor is responsible for the delivery of quality services/supplies to the Government (see FAR 52.246-1 Contractor Inspection Requirements). The Contractor shall develop, implement and maintain an effective Quality Control System which includes a written QCP. The QCP shall implement standardized procedure/methodology for monitoring and documenting contract performance to ensure all contract requirements are met. The Contractors’ QCP must contain a systematic approach to monitor operations to ensure acceptable services/products are provided to the Government.

The QCP as a minimum shall address continuous process improvement; procedures for scheduling, conducting and documentation of inspection; discrepancy identification and correction; corrective action procedures to include procedures for addressing Government discovered non-conformance; procedures for root cause analysis to identify the root cause and root cause corrective action to prevent re-occurrence of discrepancies; procedures for trend analysis; procedures for collecting and addressing customer feedback/complaints. The contractor shall upon request provide to the Government their quality control documentation. QCP is to be delivered electronically within 10 days after contract award. Electronic copies of a comprehensive written QCP shall be submitted to the KO and COR within 5 working days when changes are made thereafter. After acceptance of the quality control plan the contractor shall receive the Contracting Officer’s acceptance in writing of any proposed change to their QC system.

F.5. Environmental Protection Plan (EPP). The Contractor shall establish and maintain an EPP that is in compliance with local, state, and federal environmental protection laws, regulations and standards. The EPP shall be submitted to the Contracting Officer for review and acceptance ten (10) calendar days prior to contract start date. Any changes to the EPP shall be submitted to the Contracting Officer for review and acceptance five (5) calendar days prior to any changes.

F.6. Material Safety Data Sheets (MSDS). Contractor shall provide MSDS to the COR or designated representative 10 days prior to start of work. MSDS are required for all hazardous material. Contractor is responsible for complying with all federal and state environmental and safety requirements. A specific requirement is hazardous communication and MSDS in accordance with 20 Code of Federal Regulations Part 1910. Contractor is also responsible for proper management of hazardous materials and petroleum products under Arizona Revised Statutes, Title 49, and the Resource Conversation and Recovery Act, 49 Code of Federal Regulations Parts 260 through 279.

F.7. Disposal Receipts. Contractor shall provide copy of all disposal receipts to the Government with monthly invoice.

G. SPECIFIC TASKS

G.1. The contractor shall pump and clean on all grease traps located at buildings listed below and at the frequency indicated. All grease traps shall be cleaned by the third Friday of the month due. Contractor shall contact the QAE two (2) days prior to service being performed and, in addition, will contact the QAE when contractor physically arrives on Fort Huachuca to pump grease traps. The contractor will also be required to provide emergency on call services for all facilities listed.

G.1.2. The contractor shall pump out all liquid and solid waste contents of the grease traps and inspection chambers until empty.

G.1.3. The contractor shall (where possible) scrape the walls of the tank to remove building-up of solids.

G.1.4. The contractor shall secure all lids to the tank and make sure that they are fitted property before departing from the area.

G.2. The contractor when finished shall clean the area of all trash and grease spills before departing from the work area.

G.3. The contractor shall properly dispose of all grease laden water in a site permitted and approved by the Arizona Department of Environmental Quality for the disposal or transfer of solid waste or a site that meets the criteria for an “agricultural landfill” under A.R.S. § 49-766 and provide copy of disposal receipts as required in paragraph F.7.

H.1. When it is determined by the COR there is a valid need for emergency cleaning of grease traps, the contractor shall respond within (two) 2 hours of callout.

I. SAFETY

I.1. Procedures. The work performance shall be in accordance with the following current publications: “Corps of Engineers Manual, General Safety Requirements, EM 385-1-1; Occupational Safety and Health Act (OSHA) Public Law 91-596 and the standards as set by the National Board of Fire Underwriters (National Fire Codes) and NFPA National Fire Code, No. 91. Flammable solvents or other flammable cleaning aids shall not be used. During cleaning, proper ventilation shall be provided and safety precautions taken when cleaning is done inside Grease Trap tanks.

I.2. Contractor personnel shall use proper safety equipment at all times during the performance of this contract and shall perform all tasks in a safe manner.

I.3. Fire Protection. Contractor shall equip motor vehicles used in performance of this contract with a fire extinguisher with a minimum listed rating of 10B:C in accordance with National Fire Protection Association (NFPA) 10-1984, Paragraph 4.

I.4. Vehicle Maintenance. Contractor shall maintain the outside of pumping vehicles and keep them free of grease and oil.

I.5. The Contractor shall not drive any vehicle on the grass or sidewalks.

I.6. Pumping requirements may require the Contractor to use approximately one hundred twenty (120) feet of additional suction lines. These lines may extend from the nearest parking lot, road, or sidewalk to the grease traps.

Contractor shall ensure these lines are protected so as to not pose a tripping hazard.

I.7. The Contractor shall be required to remove manhole covers on grease traps. These covers will vary in size and shape and may weigh as much as three hundred fifty (350) pounds.

I.8. Confined Space Entry. If Contractor must enter the grease trap for any reason, the Contractor shall perform all operations IAW OSHA standard 29 CFR 1910.146 (c) (4), Confined Space Entry.

J. INSPECTION OF SERVICES

J.1. Inspection. The work shall be subject to inspection at any and all times. The presence or absence of the inspector shall not relieve the contractor from strict compliance with these specifications nor shall the presence of an inspector relieve the contractor from any deficiency that may be noted in the final inspection. Upon completion of work on each grease trap or other item required to be cleaned in each building, the contractor shall notify the COR or designated representative and a joint inspection shall be made with the contractor.

J.2. Unsatisfactory Work. The Contractor shall re-perform work that was not satisfactorily accomplished in accordance with contract requirements within four (4) working hours after notification from the Contracting Officer.

If there is a problem beyond the Contractor's control that does not permit the Contractor to accomplish the work, the Contractor shall report such instances to the Contracting Officer the same day the Contractor receives notification of the unsatisfactory work.

K. CONTRACTOR MANPOWER REPORTING REQUIREMENTS

K.1. The Contractor shall report ALL Contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for Department of Public Works (DPW) for Fort Huachuca, AZ via a secure data collection site. The Contractor is required to completely fill in all required data fields using the following web address: http://www.ecmra.mil/.

K.2. Reporting inputs will be for the labor executed during the period of performance during each Government fiscal year (FY), which runs October 1 through September 30. While inputs may be reported any time during the FY, all data shall be reported no later than October 31 of each calendar year. Contractors may direct questions to the help desk by clicking on "Send an email" which is located under the Help Resources ribbon on the right side of the login page of the applicable Service/Component's Contractor Manpower Reporting website.

L. CONTRACT PAYMENTS

*All vendors who need access to the military installation must come through the Main Gate (Van Deman) of Fort Huachuca, located at the corner of State Highway 90 Bypass and State Highway 90.

*Please allow time for travel from the Main Gate to the Site Visit location.

*If applicable, vendors will also need to allow themselves ample time to obtain a pass at the Main Gate.
