STATEMENT OF WORK
PN: WEFM172100 REPAIR MULTIPLE FACILITIES
GENERAL: The Contractor is responsible for providing all materials, equipment, tools and labor to complete the items described in each Contract Line Item (CLIN) in accordance with applicable local and national codes, UFC’s (Unified Facilities Criteria), and ANG ETL’s (Engineering Technical Letter). Ensure all waste is properly recycled, if possible, or disposed of. The contractor shall provide a 1 year warranty on all items below.
CONDITIONS FOR CONTRACT AWARD: The basis of award for this project shall be ALL OR NONE OF THE CLINS.
CLIN #1 REPAIR WATER INFILTRATION ON BULDING 12 – STANLY COUNTY
The Contractor shall include the work herein but not limited to the installation of new through-wall flashing and counter flashing on the NE facing wall above roof level. Installation of two new gravel-stop closures and backer rod and sealant where missing at the NE facing wall above roof level. Installation of new trench drain to address ponding problems near Building 12 in area #1 (note report photographs 10 & 11) and area #2 (note report photographs 12- 14). Trench drain will connect to a new storm water pipe that will discharge to grade as shown in the Conceptual Site Plans attached with this report. In addition, demolish existing pavement and install new pavement to slope to trench drain. See Attachment A for report and drawings regarding water infiltration areas on building 12.
The Contractor shall include the work herein but not limited to the cleaning and preparation of areas that are in need of repairs. Prime and install 5” cover strip over all field seams that loose or have shown signs of pulling loose. Install patches to 7 corners of curbs wall corners that have holes or voids that are leaking. Repair or replace and seal 6 pipe boots that appear to have failed over the years. Check 7 roof drains for loose bolts and install new target patch around each drain. Provide owner with a 1 year workmanship warranty. See Attachment B for these basis of design quote for roof repair.
LOCATION: The project is located at the 145th Airlift Wing, North Carolina Air National Guard, Stanly County Regional Airport, New London, NC.
WORK TO BE ACCOMPLISHED: The Contractor is responsible for furnishing all materials, equipment, labor, testing and permitting necessary to perform work as outlined in project specifications and drawings and to complete the items described in each Contract Line Item (CLIN) in accordance with applicable local and national codes, UFC’s (Unified Facilities Criteria), and ETL’s (Engineering Technical Letter). All material/equipment to be used will be submitted to the Government for approval/coordination prior to contractor procurement. The contractor is also responsible for the proper disposal of all waste, to include materials being removed as defined herein. For all CLINs: The contractor shall work in a manner to ensure that daily operations are not impeded. This includes and is not limited to the daily flow of traffic; on and off base.
GOVERNMENT ERRORS AND OMMISIONS: The Contractor is responsible for providing the Government with a complete and usable product/facility. It is the Contractor’s responsibility to ensure that all applicable industry codes and standards are adhered to. Should apparent issues arise – the Contractor shall submit a Request for Information (RFI) to the Contracting Office. It is unacceptable for the Contractor to knowingly “look-over” an apparent issue, simply because it was not specifically called out in the Statement of Work. The Contractor and Contracting Officer’s Representative (COR) will review the provided Statement of Work and Drawing Set. The scope of work depicted on the drawings and Statement of work is intended to be inclusive. However, work to replace/reposition existing equipment /items; such as, but not limited to, repositioning existing HVAC fixtures back into a new lay-in ceiling; are considered required work to make the facility "Complete and Usable," even if not directly called out in the SOW or drawings.

Submission of a bid by a contractor shall be accepted as prima fascia evidence that they have examined the job and specifications and is satisfied as to the nature, location, and scope of the work and all other matters which can in any way affect the work or cost thereof under this contract. Any failure of the contractor to acquaint himself with all available information including a physical survey of the construction site of the proposed work, will not relieve him/her from successfully performing all the required work in the plans and specifications to a complete, finished, acceptable job.
The above outline of principle features of the work in no way limits the responsibility of the contractor to perform all work and furnish all tools, labor, and equipment required by the specifications referred to herein.
The Contractor is responsible for ensuring and demonstrating all work meets the performance standards set forth in the plans and specifications and in accordance with Federal Acquisition Regulations, Part 52.246.

HAZARDOUS MATERIALS MANAGEMENT: Contractor shall maintain and submit monthly hazardous materials usage in accordance with NCANG requirements. All Material Safety Data Sheets shall be maintained on site and submitted on the AF Form 3000 electronically. Any unused hazardous material shall be removed from the job-site unless required as excess stock.
GREEN PROCUREMENT: Contractor shall follow all Federal mandates for green procurement, and the EPA Comprehensive Procurement Guidelines where applicable. Where purchase of recycled content is not available or practical, the Contracting Officer shall be notified immediately.
WASTE REPORTING: Contractor shall provide a monthly waste report indicating the amount (weight) of construction debris that was landfilled and diverted from landfill (recycled).
SECURITY REQUIREMENTS: Contractor personnel shall obtain pass and individual picture identification items in accordance with NCANG Base and Air Force regulations. All contractors working on base will be required to submit a background check and complete the required access paperwork before obtaining an access badge. "All contractors/sub- contractor personnel will follow the vetting process for the NC Air National Guard as outlined in the 145th Airlift Wing Instruction 31-101 and Contractors Handbook. Access will not be permitted until all personnel are vetted and approved for unescorted entry to the installation".
For contractors that do not require CAC, but require access to a DoD facility or installation. Contractor and all associated sub-contractors employees shall comply with adjudication standards and procedures using the National Crime Information Center Interstate Identification Index (NCIC-III) and Terrorist Screening Database (TSDB) (Army Directive 2014-05/AR 190-13), applicable installation, facility and area commander installation/facility access and local security policies and procedures (provided by government representative), or, at OCONUS locations, in accordance with status of forces agreements and other theater regulations.
CONTRACTOR MANPOWER REPORTING: The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the AIR FORCE collection site. The contractor is required to completely fill in all required data fields using the following web address: https://afcmra.dmdc.osd.mil/. Reporting inputs will be for the labor executed during the period of performance during each Government fiscal year (FY), which runs October 1 through September 30. While inputs may be reported any time during the FY, all data shall be reported no later than October 31 of each calendar year. Contractors may direct questions to the help desk by email at dod.ecmra-af-support-desk@mail.mil.
QUESTIONS ARISING DURING PROPOSAL PREPARATION: If questions arise during the Contractor’s development of their proposal, they shall provide those questions in writing to the Contracting Officer. This includes general and technical clarifications. No telephone questions shall be answered. Questions can be emailed to the Contracting Officer at: SSgt Zachary Leddy (Lead Contracting Officer): zachary.d.leddy.mil@mail.mil All questions and answers shall be sent to all responsive contractors unless the Contractor asking the question specifically stipulates this is a Best Value Alternative question and thereby requests privacy pertaining to his/her best value initiatives.
