STATEMENT OF WORK

Burnsville Lake Surge Chamber, Sludge Holding Tank, and Control Panel for Bulltown Campground Sewage Treatment Plant

1. SCOPE.

The Contractor shall furnish materials, including pumps, floats, blower, electrical controls, and steel tanks for a surge chamber with integral sludge holding tank for Bulltown Campground Sewage Treatment Plant. The total contract price will include the cost of transportation of all materials to the construction site in Braxton County, West Virginia.

Offloading the tank and installation of this system will be performed by the Government.

2. SPECIFICATIONS.

2.1. General. The aerated surge chamber shall serve to equalize the flow to the aeration ditch of the existing sewage treatment plant. The sewage shall enter the surge chamber, shall be stored during peak flow periods, and shall be pumped to the aeration ditch at a consistent flow by means of duplex progressing cavity pumps so as to not overload the plant.

The aerated sludge holding tank shall hold sludge that accumulates in the plant until it is removed by a pumping truck. Sewage will be manually pumped into this tank using pumps presently installed in the existing sewage treatment plant. The contractor shall provide the tank and all necessary motors, aerator, controls, floats, weirs, and other parts necessary for this system to operate automatically.

2.2. Steel Tank. The sewage surge chamber shall be Economy Tank Company Model SC-50-50 or equal. The surge chamber shall hold a minimum of 25,000 gallons and shall have an integral sludge holding tank with a minimum capacity of 4,100 gallons attached. The tank shall be constructed of ¼” welded steel plate and shall be filleted on each side at the bottom to prevent sludge accumulation and at the top to enhance rotation of the tank contents.

The tanks shall be water tight, and all welding shall be done in accordance with AWS specifications. The steel chamber shall be suitable for burial and shall have hold down lugs to prevent flotation. The exterior measurements of the tank (including both surge chamber and integral sludge holding tank) shall be approximately 12’ wide, 35’ long and 11’high. Six 17 lb.

magnesium anodes shall be added to the tank for corrosion prevention.

An equalizing box with adjustable v-notch weirs and side weirs shall be located inside the surge chamber where sewage exits the surge chamber to equalize flow to the aeration ditch.

Flow shall be controlled by these weirs to allow the surge chamber to fill during peak flow periods. In the event more influent is coming into the plant and the pumps are not keeping the volume of the sewage from increasing, sewage will exit the surge chamber using the 6” overflow line mentioned elsewhere.

All steel plate shall be pickled, dry, and shall have all mill scale removed prior to fabrication. Mill scale removed by blasting will not be acceptable. After fabrication and welding, the inside and outside surfaces of the tank shall be blasted with Black Beauty #1240 grit. All surface areas shall be coated with Sherwin Williams TarGuard Coal Tar Two Part Epoxy Paint or equal, colored black, which is resistant to soil stress and to attack by alkalis, acids, salts, moisture, gasoline and oil. This coating is designed for use as a superior tank coating.

Enamels, asphalts or other coatings will not be acceptable.

Strongwell or equal fiberglass open grip safety grating panels shall be provided for all tank openings, with securely welded brackets to hold the grating. The grating panels shall be 12” maximum in width and constructed of fiberglass formed, perforated, and extruded so as to permit maximum open area. The maximum weight of each panel shall not exceed 27 pounds, and each panel shall have a safe uniform load carrying capacity of 135 pounds/sq. ft. without excessive deflection. The grating brackets shall likewise have a safe uniform load carrying capacity of 135 pounds/sq. ft. Panels larger than or heavier than that previously specified will not be acceptable.

All inlets and outlets shall be on the two ends of the tank. A 6” threaded Schedule 80 PVC overflow outlet shall be installed on the end of the surge chamber at the maximum height of the liquid so in the event the tank becomes full, sewage will gravity flow to the sewage plant.

The submersible pumps shall connect to an outlet on the end of the sludge holding tank to connect to piping that will extend to the aeration ditch. Piping shall extend from the partition between the two tanks over the liquid level in the sludge holding tank and under the grating to the outlet referenced in the previous sentence. This shall be a threaded fitting and shall accept 2” HDPE pipe. The connection on the partition that connects to the pumps shall be threaded on the inside so it can be disconnected from the wall of the tank for easy removal in the event a pump will need removed. The connection between the two pumps shall be accessible (above high liquid level) but under the grating for easy access for repair. Three inlets shall be installed on the end of the surge chamber to connect to incoming piping from the campground. These inlets shall be threaded fittings that accepts 2” HDPE pipe. An inlet shall be installed on the end of the sludge holding tank to accept piping from the aeration ditch for incoming sludge. This shall be a threaded fitting and shall accept 2” HDPE pipe.

Any exterior piping will be supplied by and installed by the Government.

2.3. Sewage Pumps. Two sewage pumps shall be located in the equalizing flow box to pump sewage to the aeration ditch in the sewage treatment plant. These pumps shall be Tsurumi Model 50C2.75S-CR-61 or equal and shall be self-priming positive displacement cutter pumps. They shall each be driven by 220 volt, 60 cycle, 1 phase, 1 HP motors. The discharge rate from the pumps shall be adjustable by regulating the speed of the pumps. The pumps shall be capable of pumping 12 GPM to the plant. The discharge line shall be 2” diameter.

2.4. Aeration. The surge chamber and sludge holding tank shall both be aerated. The blower shall be a Roots Universal RAI-J Dual Splash Lubricated or equal minimum 5 hp rotary positive blower. It shall be a lower noise than standard blower and shall have no grease fittings.

The entire unit shall be oil lubricated only to reduce maintenance. Standard blowers will not be acceptable due to high noise levels and increased maintenance. The header and diffuser assemblies shall be as specified for the main plant aeration chamber.

The blower shall have a 24 hour time clock to vary the air supply in 15 minute intervals and shall deliver a minimum of 80-100 C.F.M. @ at a minimum of 4.5 p.s.i. The motor drive shall be a 5 HP, 1750 RPM, 220 volt, 1 phase, 60 cycle unit with a V-belt drive. The motor shall be in a weatherproof motor / blower housing. For reduction of noise and the filtration of the inlet air, there shall be supplied a Solberg filter-silencer or approved equal. The blower and motor shall be mounted on a molded fiberglass slide base to maintain proper belt tension. The slide base shall be mounted on rubber shock absorbers firmly mounted to the base section. Rigid mounting of slide base will not be acceptable. The base structure shall be adequately reinforced to support the blower/motor unit. Air for the blower shall be admitted to the enclosure section through a vent of adequate size in the base section. Mounting of the unit on the top of any section of the tank will not be allowed. The blower/motor shall be enclosed within a molded fiberglass weatherproof hood mounted to the base. The housing shall consist of a channel shaped base section and fiberglass enclosure section of an inverted box design. One end of the enclosure shall be permanently fastened to the base section with a stainless steel continuous hinge.

Diffuser air shall be supplied by the blower to both the surge chamber and sludge holding tank.

Diffusers shall be EDI FlexAir or equal non-clog disc diffusers sized to create air flow to adequately aerate both the surge chamber and the sludge holding tank. They shall be spaced on 16” centers and located parallel to and near the base of the bottom fillet. All piping and valves within the tank shall be factory installed. An ambient pressure compensated regulator shall be used so that during low water levels in the surge chamber excess air will not be diverted to the sludge holding tank. All air supply lines to the diffusers shall have a shut off valve on each line for adjustment to control the roll of aeration in both the surge chamber and the sludge holding tank. It shall be possible to completely cut off all air in the sludge holding tank to let solids settle when necessary to pump off clear fluids.

2.5. Electrical Circuitry and Control Panel. Furnish one NEMA 4X weatherproof electrical control panel and control circuitry for the duplex pump system. Enclosure shall be equipped with an inner door and interlock. The Government will construct a frame to hold the control panel, which will be located on the ground near the tank. Circuitry shall include all electrical controls to operate duplicate 1 hp pumps with 230 volt, 60 cycle, single phase service, and shall include breakers, start and run capacitors, relays, alternator, pump running lights, a pump motor moisture sensor light, and a high level alarm light. Circuitry shall be arranged so that the high level alarm light is activated when the lag pump comes on. "Hand-Off-Auto" switches shall be provided on the panel. A ground-fault circuit interrupter duplex receptacle shall be provided on the panel.

The control panel circuitry shall include starters with auxiliary contacts to operate both pumps on override conditions until the "stop pumping" level is reached. An alternating relay shall be provided to alternate operation of the pumps on each successive cycle. An interlock relay shall be provided to automatically reconnect the control circuit if a circuit breaker trips power to one of the pumps.

The high-level alarm light shall be a flashing type, weatherproof, 100 watts, housed in a red globe, and mounted on top of the control panel. The high-level alarm light is to continue to flash until manually reset, even though the level of liquid in the surge chamber returns to normal. Controllers shall be wired so that a pump will not operate if a thermal protection switch is activated. A moisture sensor system must be provided to detect the presence of water in the oil chamber of the pump. If this should occur, an amber warning light shall be activated. This light shall be mounted on top of the control panel, next to the red high level alarm light, in a waterproof globe similar to that for the high level alarm light.

Circuitry to control the blower, including the timer, breakers, etc., shall also be located in this same control panel.

All electrical equipment, materials, and electrical work must conform to the most recent version of National Electrical Code Standards. A detailed schematic wiring diagram of, and a bill of materials for the electrical control panel must be furnished to the Contracting Officer for approval before shipment is made. Supplier must guarantee that all materials and equipment are compatible with each other.

2.6. Float Switches. Three SJE 30SWINO normally open float switches shall be furnished, each with 30 foot cable leads. The floats shall have mercury switches encapsulated in a polypropylene housing and shall be weighted so that the switch is activated as the float swings around the weight. A Halliday Model J4A SS cable holder shall be provided. Float switches shall be installed in the equalizing flow box within the surge chamber.

3. DELIVERY.

Equipment and materials shall be delivered to Bulltown Campground, Burnsville Lake, 1183 Bulltown Campground Road, Napier, WV 26631. The Contractor shall contact Rick Pingley at 304-853-2371 at least one week before delivery is scheduled. The Government will have equipment onsite to offload the tank, so it is imperative that delivery of the tank is coordinated with the Government so the equipment to unload the tank is onsite at the same time the truck arrives with the tank. It may be necessary for the Contractor to delay delivery until the offloading equipment is available. Delivery of the tank without coordination will result in returning the tank to the Contractor for future delivery. Working hours are from 0730-1600 hours Monday through Friday. Delivery shall be made between 0800-1200 hours and shall not be made on a federal holiday.

4. WARRANTY.

The Contractor shall provide a full-coverage warranty for a minimum of one year on all components of the above system. There shall be no cost for parts or labor during the warranty period. If replacement parts are necessary, the Contractor shall ship the parts within 24 hours of notification of a deficiency. If a deficiency requires Contractor labor to correct the deficiency, the Contractor shall respond onsite within 24 hours (next business day) of notification of the deficiency.

5. SECURITY REQUIREMENTS.

For contractors who do not require CAC, but require access to a DoD facility or installation. Contractor and all associated sub-contractors employees shall comply with adjudication standards and procedures using the National Crime Information Center Interstate Identification Index (NCIC-III) and Terrorist Screening Database (TSDB) (Army Directive 2014- 05 / AR 190-13), applicable installation, facility and area commander installation/facility access and local security policies and procedures (provided by government representative, as NCIC and TSDB are available), or, at OCONUS locations, in accordance with status of forces agreements and other theater regulations.

Pre-screen candidates using E-Verify Program. 52.204-16 COMMERCIAL AND GOVERNMENT ENTITY CODE REPORTING (JUL 2015)

(a) Definition. As used in this provision--

Commercial and Government Entity (CAGE) code means—

(1) An identifier assigned to entities located in the United States or its outlying areas by the Defense Logistics Agency (DLA) Contractor and Government Entity (CAGE) Branch to identify a commercial or Government entity;

or

(2) An identifier assigned by a member of the North Atlantic treaty Organization (NATO) or by the NATO Support Agency (NSPA) to entities located outside the United States and its outlying areas that the DLA Contractor and Government Entity (CAGE) Branch records and maintains in the CAGE master file. This type of code is known as an NCAGE code.

(b) The Offeror shall enter its CAGE code in its offer with its name and address or otherwise include it prominently in its proposal. The CAGE code entered must be for that name and address. Enter ``CAGE'' before the number. The CAGE code is required prior to award.

(c) CAGE codes may be obtained via--

(1) Registration in the System for Award Management (SAM) at www.sam.gov. If the Offeror is located in the United States or its outlying areas and does not already have a CAGE code assigned, the DLA Contractor and Government Entity (CAGE) Branch will assign a CAGE code as a part of the SAM registration process. SAM registrants located outside the United States and its outlying areas shall obtain a NCAGE code prior to registration in SAM (see paragraph (c)(3) of this provision).

(2) The DLA Contractor and Government Entity (CAGE) Branch. If registration in SAM is not required for the subject procurement, and the offeror does not otherwise register in SAM, an offeror located in the United States or its outlying areas may request that a CAGE code be assigned by submitting a request at http://www.dlis.dla.mil/cage_welcome.asp.

(3) The appropriate country codification bureau. Entities located outside the United States and its outlying areas may obtain an NCAGE code by contacting the Codification Bureau in the foreign entity's country if that country is a member of NATO or a sponsored nation. NCAGE codes may be obtained from the NSPA if the foreign entity's country is not a member of NATO or a sponsored nation. Points of contact for codification bureaus and NSPA, as well as additional information on obtaining NCAGE codes, are available at http://www.dlis.dla.mil/nato/ObtainCAGE.asp.

(d) Additional guidance for establishing and maintaining CAGE codes is available at http://www.dlis.dla.mil/cage_welcome.asp.

(e) When a CAGE Code is required for the immediate owner and/or the highest-level owner by 52.204-17 or 52.212-3(p), the Offeror shall obtain the respective CAGE Code from that entity to supply the CAGE Code to the Government.

(f) Do not delay submission of the offer pending receipt of a CAGE code.

(End of Provision)

52.211-6 BRAND NAME OR EQUAL (AUG 1999)

(a) If an item in this solicitation is identified as “brand name or equal,” the purchase description reflects the characteristics and level of quality that will satisfy the Government's needs. The salient physical, functional, or performance characteristics that “equal” products must meet are specified in the solicitation.

(b) To be considered for award, offers of “equal” products, including “equal” products of the brand name manufacturer, must--

(1) Meet the salient physical, functional, or performance characteristic specified in this solicitation;

(2) Clearly identify the item by--

(i) Brand name, if any; and

(ii) Make or model number;

(3) Include descriptive literature such as illustrations, drawings, or a clear reference to previously furnished descriptive data or information available to the Contracting Officer; and

(4) Clearly describe any modifications the offeror plans to make in a product to make it conform to the solicitation requirements. Mark any descriptive material to clearly show the modifications.

(c) The Contracting Officer will evaluate “equal” products on the basis of information furnished by the offeror or identified in the offer and reasonably available to the Contracting Officer. The Contracting Officer is not responsible for locating or obtaining any information not identified in the offer.

(d) Unless the offeror clearly indicates in its offer that the product being offered is an “equal” product, the offeror shall provide the brand name product referenced in the solicitation.

(End of provision)

52.212-1 INSTRUCTIONS TO OFFERORS--COMMERCIAL ITEMS (OCT 2015)

(a) North American Industry Classification System (NAICS) code and small business size standard. The NAICS code and small business size standard for this acquisition appear in Block 10 of the solicitation cover sheet (SF 1449). However, the small business size standard for a concern which submits an offer in its own name, but which proposes to furnish an item which it did not itself manufacture, is 500 employees.

(b) Submission of offers. Submit signed and dated offers to the office specified in this solicitation at or before the exact time specified in this solicitation. Offers may be submitted on the SF 1449, letterhead stationery, or as otherwise specified in the solicitation. As a minimum, offers must show--

(1) The solicitation number;

(2) The time specified in the solicitation for receipt of offers;

(3) The name, address, and telephone number of the offeror;

(4) A technical description of the items being offered in sufficient detail to evaluate compliance with the requirements in the solicitation. This may include product literature, or other documents, if necessary;

(5) Terms of any express warranty;

(6) Price and any discount terms;

(7) "Remit to" address, if different than mailing address;

(8) A completed copy of the representations and certifications at FAR 52.212-3 (see FAR 52.212-3(b) for those representations and certifications that the offeror shall complete electronically);

(9) Acknowledgment of Solicitation Amendments;

(10) Past performance information, when included as an evaluation factor, to include recent and relevant contracts for the same or similar items and other references (including contract numbers, points of contact with telephone numbers and other relevant information); and

(11) If the offer is not submitted on the SF 1449, include a statement specifying the extent of agreement with all terms, conditions, and provisions included in the solicitation. Offers that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration.

(c) Period for acceptance of offers. The offeror agrees to hold the prices in its offer firm for 30 calendar days from the date specified for receipt of offers, unless another time period is specified in an addendum to the solicitation.

(d) Product samples. When required by the solicitation, product samples shall be submitted at or prior to the time specified for receipt of offers. Unless otherwise specified in this solicitation, these samples shall be submitted at no expense to the Government, and returned at the sender's request and expense, unless they are destroyed during preaward testing.

(e) Multiple offers. Offerors are encouraged to submit multiple offers presenting alternative terms and conditions or commercial items for satisfying the requirements of this solicitation. Each offer submitted will be evaluated separately.

(f) Late submissions, modifications, revisions, and withdrawals of offers:

(1) Offerors are responsible for submitting offers, and any modifications, revisions, or withdrawals, so as to reach the Government office designated in the solicitation by the time specified in the solicitation. If no time is specified in the solicitation, the time for receipt is 4:30 p.m., local time, for the designated Government office on the date that offers or revisions are due.

(2)(i) Any offer, modification, revision, or withdrawal of an offer received at the Government office designated in the solicitation after the exact time specified for receipt of offers is “late” and will not be considered unless it is received before award is made, the Contracting Officer determines that accepting the late offer would not unduly delay the acquisition; and--

(A) If it was transmitted through an electronic commerce method authorized by the solicitation, it was received at the initial point of entry to the Government infrastructure not later than 5:00 p.m. one working day prior to the date specified for receipt of offers; or

(B) There is acceptable evidence to establish that it was received at the Government installation designated for receipt of offers and was under the Government's control prior to the time set for receipt of offers; or
