Instructions to Offerors Red Sash/ Red Belt
All offerors interested in submitting a quote for this requirement need to follow these instructions. If an offeror does not comply with these instructions, they shall be excluded from consideration of a contract award.
All offerors shall send an email to Jarrett Baker at jarrett.j.baker2.civ@mail.mil by 12:00 pm on Febuary 7th, 2018. Any offeror may submit their information after Febuary 7th, 2018, however, the Government shall not be responsible for an offeror not having sufficient time to receive the Government Furnished Material, and produce a sample of one red belt and one sash, to submit with their quote by the solicitation closing date 02/26/18.
The email must contain the following information:

SAMPLE REQUIREMENT
Instructions to Offeror Red Sash/Red Belt Sample
All offerors interested in submitting a proposal for this requirement need to follow these instructions. If an offeror does not comply with these instructions, they shall be excluded from consideration of a contract award.
All offerors shall send one sample each of the Red Sash and Red Belt to Jarrett Baker at Bldg 681 Hardee Pl, RM-
104 West Point, NY 10996 jarrett.j.baker2.civ@mail.mil by close of Solicitation date by 26 Feb, 2018 at 12 pm
(Eastern Time). The mailed sample must contain the following information: Name of Company, Contact Name
(First & Last), Mailing Address, City, State, Zip Code, Direct Phone Number, and Email Address. Please include the
Solicitation Number (W911SD-18-T-0084) on package.
All samples must be submitted with the offerors proposal by the closing date of the solicitation. The Government shall make one contract award. Therefore, if an offeror doesn’t submit samples with their proposal, they shall not receive consideration for contract award. The Government shall not be liable for any lost or misdirected proposal and or samples.

USMA PURCHASE DESCRIPTION
RED BELT
Description: These belts are maroon in color and are worn around the waist in conjunction with a matching Red Sash by USMA Cadets in various dress uniforms. Belts are secured with pieces of Velcro loop and hook-tape. They may be produced in two sizes, Medium and Large.
a. Medium-length finished belts shall measure 46 ½” long (+/- 3/8”) to the pointed-end and shall be identified with a bar-coded size label indicating the local Stock Number
840501Y503876.
b. Large-length Belts shall be 50” long (+/- 3/8”) to the pointed end and shall be identified with a bar-coded size label indicating the local Stock Number 840501Y503877.
c. Finished width of either size Belt shall be 2 ¾” wide (+/- 1/8”).
Requirements:
1. The Government will provide cut Belt Shell-pieces, Lining-pieces, Buckram-pieces, bar-coded size labels.

2. The Government will provide sufficient lengths of 2” wide maroon Velcro Hook-tape and Loop-tape.
3. The Contractor shall cut Velcro Hook-tape and Loop-tapes Pieces as described below.
4. The Contractor shall sew all seams with thread matching the color of the Maroon shell cloth. Thread used in sewing shall be core-spun Cotton/Polyester size TKT 60/36, in a color to match the marron color and shade of the cloth such as or equivalent to Color:
Kir #3927 (Coats NA) or Color: Light Pimlico #11254 (A&E).
Cut Component Measurements:
1. The cut length of a Medium Belt Shell-piece (maroon cloth) will measure 48 ¾” long to its point and
4” wide. The cut length of a Large Belt Shell-piece will measure 52” long with same width as a
Medium Belt Shell piece.
2. All cut length and width measurements have a tolerance of +/- 1/8”.
3. A Medium Belt Lining piece (white cloth) will measure 46 ¾” long from the end cut with right-angles to the point of its pointed-end and 2 ¾” wide. A Large Belt Lining piece will measure 50” long with same width as a Medium Belt Lining piece.
4. A Medium Belt Buckram piece will measure 46 ½” long to its pointed-end and 2 ½” wide. A Large Belt
Buckram piece will measure 50” long with same width as a Medium Belt Buckram piece.
5. Velcro Loop-tape pieces shall be cut into two types of pieces for each Red Belt. Each Belt will require one each of the following and shall measure:
• 10” long by full 2” width of the Loop-tape.
• 2 ¾” long by full 2” width of the Loop-tape.
6. Velcro Hook-tape pieces shall be cut into a 5-sided polygon measuring 2” to its point (across the full 2” width of the Velcro Hook-tape), by 2 ¾” long, one for each belt. See Figure 1.
Manufacturing:
1. Position the 10” Velcro Loop-piece on the white lining piece, 3½” in from the point of the pointed-end of that lining piece. Center the 10” Velcro Loop-piece between the long-edges of the lining piece. Sew a lock-stitch seam around the parameter of the 10” Velcro Loop-piece approximately 1/16” in from its edges. Secure this seam with a 3/16 to 1/8” back-tack.
2. Position the Belt Lining piece face to face on the Belt Shell piece with the Belt Lining piece ¾” in from the squared end of the Belt Shell piece and even along one long cut- edge of the Belt Shell piece. See
Figure 2. Sew a ¼” lock-stitch seam along the edges of these pieces. As this seam is being sewn, insert the bar-code size label in this seam between the shell piece and the lining piece at a position 1½ to 2” away from the end of the 10” Velcro Loop-piece towards the middle of the belt. The label must be positioned and sewn in a manner in which will allow the printed information on the label to be read after pressing the seams. Approximately ½ or more of the label will be sewn into the seam. See Pressing instructions in item #8.a. below and Figure 4. Secure this seam with a 3/16 to 1/8” back-tack.
3. Position the other long-edges of the Belt Lining and Belt Shell pieces in the same manner and sew them together with a ¼” lock-stitch seam following the edges of those pieces. Secure this seam with a 3/16 to 1/8” back-tack.
4. Turn the Belt face out.
5. Insert the Buckram-piece into the Belt and align it within the shell piece so that the Shell is evenly divided on the back of the belt and so that the squared end of the Buckram is even with the squared end of the lining piece. See Figure 3.
• Operations 4 and 5 may be accomplished together.
6. With the Belt laying face-down, with the Lining facing up, fold the squared end of the Belt Shell-piece in a manner in which will hide the ends and cut-edges of the shell and lining pieces from view when looking at the face of the Belt; and position the rectangular 2 ¾” Velcro Loop-piece evenly from the three edges at the end of the Belt. Sew a rectangular lock-stitch seam 1/8” in from all four edges of the
Velcro Loop- piece. The bobbin thread of this seam will show on the face of the Belt. Secure this seam with a 3/16 to 1/8” back-tack.
7. With the Belt laying in the same manner, fold the pointed-end of the Belt Shell in a manner in which will hide the ends and edges of the shell and lining pieces from view looking at the face of the Belt and position the Velcro Hook-piece evenly from the side and point-edges of the Belt. Securely sew the
Velcro Hook-piece with a seam 1/8” in from all five edges of the Velcro Hook-piece. The bobbin thread of this seam will also show on the face of the Belt. Secure this seam with a 3/16 to 1/8” back-tack.
NOTE: No seams other than those found in manufacturing instructions 6 and 7 above shall be visible on the face of the Belt.
8. Press the Belt in a manner with places an easy crease along the edges of the belt, centers the white lining piece between the long edges of the belt and folds the legible potion of the label towards the center of the belt.
• The label shall not be visible when viewing the face of the Belt after pressing.
Packaging:
1. Finished Belts shall be packed into sturdy cardboard boxes (cases). Boxes shall be strong enough to support full cases of Belts being stacked 3 high without deforming the boxes from the weight of three cases.
2. Only one size of belt shall be packaged in any case. Cases mixed with differing sizes of Belts are not acceptable.
3. Boxes shall be clearly marked with:
• The words “RED BELT” in lettering 2” tall (+/-1/4”),
• The size of the Belt (Either Medium or Large),
• The respective Quantity of Belts contained in that particular case, and
• The calendar year in which the Belts were sewn.
Contractor shall return all samples and excess Velcro to: U.S.M.A.

USMA PURCHASE DESCRIPTION
SASH, RED
Description: Vertically draping burgundy sash with two tassels worn by USMA Cadets in
Dress Uniform in conjunction with a matching Red Belt.
1. Finished length shall be 24” long (+/- 3/8”) not including tassel.
2. Finished width shall be 3” wide (+/- 1/8”).
Requirements:
1. The Government will provide cut cloth and buckram pieces.
2. The Government will provide lengths of sewn belt-looping and lengths of 2” wide
Velcro Hook-tape.
3. The Government will provide Tassels (2 each) and 5/8” Flat Washers (2 each) 1/16” thick and 11/16” OD for Sashes.
4. Contractor shall cut Belt-looping to 4” segments and Velcro Hook-tape into 27/8” lengths.
5. The Contractor shall sew all seams with thread matching the color of the Sash. Thread used in sewing shall be core-spun Cotton/Polyester size TKT 60/36, in a color to match the marron color and shade of the cloth such as or equivalent to Color: Kir #3927 (Coats
NA) or Color: Light Pimlico #11254 (A&E).
Cut Component Measurements:
1. Sash-piece will measure 52” x 6¾” (+/- 1/8”).
2. Velcro & Belt-loop Cover will measure 4” x 6¾” (+/- 1/8”).
3. Buckram Pieces will measure 3” x 3” (+/- 1/16”).
Manufacturing:
1. Make Velcro & Belt-loop Piece.
a. Fold Velcro & Belt-loop Piece Cover face to face matching both 4” sides evenly and sew a ¼” lock-stitch seam to join.
b. Secure all seams with a 3/16-¼” back-tack.
c. All seams shall be lock-stitch seams.
d. Press 4” seam open and turn Velcro & Belt Loop Piece Cover face out.
e. Press Velcro & Belt-loop Piece Cover flat with seam centered on back.
f. Insert 3” x 3” buckram piece inside the Velcro & Belt-loop Piece Cover and position it in the center (left to right and top to bottom) keeping the seam centered on the back.
g. Fold approximately ½” of one front edge of the Cover over the edge of the buckram and inside the Cover while keeping buckram centered as in 4.e.
above.
h. Fold approximately ½” of the back edge (edge with seam) of the Cover inside the Cover and even with the fold of front edge while keeping buckram centered as in 4.e.
i. Top Stitch the folds closing the end of the Cover with a 1/8” seam.
j. Repeat steps 4.e, f, and g for the other side of the Velcro & Belt-loop
Piece Cover.

Evaluation Factors for Award
d. Basis for Contract Award. This is a best value source selection conducted in accordance with Federal
Acquisition Regulation (FAR) 13, Simplified Acquisition Procedures, as supplemented by the Defense Federal
Acquisition Regulation Supplement (DFARS), and the Army Federal Acquisition Regulation Supplement
(AFARS). The test program authorized under FAR Subpart 13.5 is applicable to this acquisition. Award will be made to a single offeror who is deemed responsible in accordance with the Federal Acquisition Regulation
(FAR), whose offer is technically acceptable, conforms to the solicitation requirements, and whose offer, judged by an overall assessment of the evaluation criteria and other considerations specified in this solicitation, represents the lowest price.
e. Award for All of the Work. The Government intends to award one contract as a result of this solicitation. The Government intends to evaluate proposals and award a contract without discussions with offerors. Therefore, the offeror's initial proposal should contain the offeror's best terms from a price and technical standpoint. The Government reserves the right to conduct discussions if the Contracting Officer later determines them to be necessary.
f. Evaluation Criteria. Product samples. One Red Sash and one Red Belt are required at or prior to the close of solicitation. These samples shall be submitted at no expense to the Government. Samples will be evaluated in accordance with the evaluation criteria listed below. Evaluation criteria consist of factors.
The proposals will be evaluated under three (3) evaluation factors: Sample, Past Performance, and Price.
Factor 1 – Sample. The Sample will receive a rating defined below.

In order to receive an acceptable rating, the sample submissions must not contain any major defects as stated in the evaluation criteria listed below.
Factor 2 – Past Performance. The Past Performance Factor will receive one of the ratings defined below.

In order to receive an acceptable rating, the Past Performance submissions must show evidence of successfully performing work same or similar to the work outlined in the solicitation or meet the criteria for
“unknown” record of past performance. Offerors without a record of relevant past performance, the offeror may not be evaluated favorably or unfavorably on past performance. In the context of acceptability/unacceptability, “unknown” shall be considered “acceptable.” Past Performance must be both recent and relevant. To be recent, the effort must be ongoing or must have been performed during the past three (3) years from the date of issuance of this solicitation. To be relevant, the effort must be similar in nature of work, quantity, and complexity.
Factor 3 – Price. Price will not be scored or rated. Evaluation of price will be performed using one or more of the price analysis techniques in FAR 13.106-3(a). Through these techniques the Government will determine whether prices are reasonable.
Offerors must submit Past Performance on the attached sheets.
d. A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.

52.203-17 CONTRACTOR EMPLOYEE WHISTLEBLOWER RIGHTS AND REQUIREMENT TO INFORM
EMPLOYEES OF WHISTLEBLOWER RIGHTS (APR 2014)
(a) This contract and employees working on this contract will be subject to the whistleblower rights and remedies in the pilot program on Contractor employee whistleblower protections established at 41 U.S.C. 4712 by section 828 of the National Defense Authorization Act for Fiscal Year 2013 (Pub. L. 112-239) and FAR 3.908.
(b) The Contractor shall inform its employees in writing, in the predominant language of the workforce, of employee whistleblower rights and protections under 41 U.S.C. 4712, as described in section 3.908 of the Federal Acquisition
Regulation.
(c) The Contractor shall insert the substance of this clause, including this paragraph (c), in all subcontracts over the simplified acquisition threshold.

52.204-3 TAXPAYER IDENTIFICATION (OCT 1998)
(a) Definitions.
Common parent, as used in this provision, means that corporate entity that owns or controls an affiliated group of corporations that files its Federal income tax returns on a consolidated basis, and of which the offeror is a member.
Taxpayer Identification Number (TIN), as used in this provision, means the number required by the Internal
Revenue Service (IRS) to be used by the offeror in reporting income tax and other returns. The TIN may be either a
Social Security Number or an Employer Identification Number.
(b) All offerors must submit the information required in paragraphs (d) through (f) of this provision to comply with debt collection requirements of 31 U.S.C. 7701(c) and 3325(d), reporting requirements of 26 U.S.C. 6041, 6041A, and 6050M, and implementing regulations issued by the IRS. If the resulting contract is subject to the payment reporting requirements described in Federal Acquisition Regulation (FAR) 4.904, the failure or refusal by the offeror to furnish the information may result in a 31 percent reduction of payments otherwise due under the contract.
(c) The TIN may be used by the Government to collect and report on any delinquent amounts arising out of the offeror's relationship with the Government (31 U.S.C. 7701(c)(3)). If the resulting contract is subject to the payment reporting requirements described in FAR 4.904, the TIN provided hereunder may be matched with IRS records to verify the accuracy of the offeror's TIN.

FELONY CONVICTION UNDER ANY FEDERAL LAW (FEB 2016)
(a) As required by sections 744 and 745 of Division E of the Consolidated and Further Continuing Appropriations
Act, 2015 (Pub. L. 113-235), and similar provisions, if contained in subsequent appropriations acts, the Government will not enter into a contract with any corporation that--
(1) Has any unpaid Federal tax liability that has been assessed, for which all judicial and administrative remedies have been exhausted or have lapsed, and that is not being paid in a timely manner pursuant to an agreement with the authority responsible for collecting the tax liability, where the awarding agency is aware of the unpaid tax liability, unless an agency has considered suspension or debarment of the corporation and made a determination that suspension or debarment is not necessary to protect the interests of the Government; or
(2) Was convicted of a felony criminal violation under any Federal law within the preceding 24 months, where the awarding agency is aware of the conviction, unless an agency has considered suspension or debarment of the corporation and made a determination that this action is not necessary to protect the interests of the Government.

The Offeror shall complete only paragraph (b) of this provision if the Offeror has completed the annual representations and certification electronically via the System for Award Management (SAM) Web site located at https://www.sam.gov/portal. (a) Definitions. As used in this provision --

“Place of manufacture” means the place where an end product is assembled out of components, or otherwise made or processed from raw materials into the finished product that is to be provided to the Government. If a product is disassembled and reassembled, the place of reassembly is not the place of manufacture.
“Predecessor” means an entity that is replaced by a successor and includes any predecessors of the predecessor.
“Restricted business operations” means business operations in Sudan that include power production activities, mineral extraction activities, oil-related activities, or the production of military equipment, as those terms are defined in the Sudan Accountability and Divestment Act of 2007 (Pub. L. 110-174). Restricted business operations do not include business operations that the person (as that term is defined in Section 2 of the Sudan Accountability and Divestment Act of 2007) conducting the business can demonstrate--
(1) Are conducted under contract directly and exclusively with the regional government of southern Sudan;
(2) Are conducted pursuant to specific authorization from the Office of Foreign Assets Control in the Department of the Treasury, or are expressly exempted under Federal law from the requirement to be conducted under such authorization;
(3) Consist of providing goods or services to marginalized populations of Sudan;
(4) Consist of providing goods or services to an internationally recognized peacekeeping force or humanitarian organization;

“Sensitive technology”--
(1) Means hardware, software, telecommunications equipment, or any other technology that is to be used specifically--
(i) To restrict the free flow of unbiased information in Iran; or
(ii) To disrupt, monitor, or otherwise restrict speech of the people of Iran; and
(2) Does not include information or informational materials the export of which the President does not have the authority to regulate or prohibit pursuant to section 203(b)(3) of the International Emergency Economic Powers Act

“Service-disabled veteran-owned small business concern”--
(1) Means a small business concern--
(i) Not less than 51 percent of which is owned by one or more service-disabled veterans or, in the case of any publicly owned business, not less than 51 percent of the stock of which is owned by one or more service-disabled veterans; and
(ii) The management and daily business operations of which are controlled by one or more service-disabled veterans or, in the case of a service-disabled veteran with permanent and severe disability, the spouse or permanent caregiver of such veteran.
(2) Service-disabled veteran means a veteran, as defined in 38 U.S.C. 101(2), with a disability that is service-connected, as defined in 38 U.S.C. 101(16).
“Small business concern” means a concern, including its affiliates, that is independently owned and operated, not dominant in the field of operation in which it is bidding on Government contracts, and qualified as a small business under the criteria in 13 CFR Part 121 and size standards in this solicitation.
“Small disadvantaged business concern”, consistent with 13 CFR 124.1002, means a small business concern under the size standard applicable to the acquisition, that--
(1) Is at least 51 percent unconditionally and directly owned (as defined at 13 CFR 124.105) by--
(i) One or more socially disadvantaged (as defined at 13 CFR 124.103) and economically disadvantaged (as defined at 13 CFR 124.104) individuals who are citizens of the United States; and
(ii) Each individual claiming economic disadvantage has a net worth not exceeding $750,000 after taking into account the applicable exclusions set forth at 13 CFR 124.104(c)(2); and
(2) The management and daily business operations of which are controlled (as defined at 13.CFR 124.106) by individuals, who meet the criteria in paragraphs (1)(i) and (ii) of this definition.
“Subsidiary” means an entity in which more than 50 percent of the entity is owned--
(1) Directly by a parent corporation; or
(2) Through another subsidiary of a parent corporation.
“Successor” means an entity that has replaced a predecessor by acquiring the assets and carrying out the affairs of the predecessor under a new name (often through acquisition or merger). The term
“successor” does not include new offices/divisions of the same company or a company that only changes its name.
The extent of the responsibility of the successor for the liabilities of the predecessor may vary, depending on State law and specific circumstances.
“Veteran-owned small business concern” means a small business concern--
(1) Not less than 51 percent of which is owned by one or more veterans (as defined at 38 U.S.C. 101(2)) or, in the case of any publicly owned business, not less than 51 percent of the stock of which is owned by one or more veterans; and
(2) The management and daily business operations of which are controlled by one or more veterans.
