STATEMENT OF WORK

BUS RENTAL FOR BAYONET FOCUS 18-02 AT FORT HUNTER LIGGETT, CA

Location Fort Hunter Liggett, CA

Speci f ic Tasks The contractor shall provide eleven (11) 22-25 passenger shuttle buses, all maintenance & labor required to keep vehicles operational, supervision and quality control. Vehicles must be on-site and ready for use by 9:00am on 14 January 2018.

Specifications

• Vehicles shall utilize regular octane fuel as specified (DIESEL)

• All vehicles shall be leased to the government with standard insurance coverage for all military personnel

• Functioning heat and Air Conditioning, Fully safe and operational with all safety equipment on hand and functioning. All fluid levels topped off.

• Automatic transmissions, no manuals

• Manua1 or power locks/window

• Vehicle color irrelevant

Requirements

• Drop off date: 14th January 2018 NLT 9:00AM

• Turn ln date: 28th March 2018 at 5:00PM

• 11 Buses (22-25 Passenger shuttle buses)

• Contractor shall provide maintenance on vehicles

• Vehicles shall be made available on the avai1able date with full tanks of gas

• Government will make arrangements with contractor for pick up or delivery location(s)

Period of Performance and Operating Hours Period of performance for this contract shall be from 14 January thru 28 March 2018. Contractor is required to perform services during the term of the contract, to inc1ude during all Federal, State, and loca1 holidays.

Installation Hours of Operation: Unless otherwise specified, entrance and exit from the installation is no earlier than 07:30am and no later than 4:00pm.The government representative will coordinate gate entry with installation guards.

Any vehicle down time shall be deducted from the award's amount.

A government representative will be appointed to the awardee at the time of award. Coordinating instructions regarding delivery/pick up, maintenance and/or repair, and pickup be coordinated with the representative.

General Description

The contractor and government representative shall coordinate delivery or pick up of leased vehicles at the start and completion of the contract.

The leased vehicles shall be used during the Bayonet Focus 18-02 Training Exercises. The Government assumes responsibility for damages sustained during each exercise.

The contractor shall provide maintenance/repair service, as required, throughout the duration of the exercise.

A. Contractor shall provide a 24/7 phone number for service related issues. Contractor shall respond to service calls within 24 hours and comp1ete the service or replace the vehicles within 12 hrs.

B. Contractor shall provide recovery support for leased vehicles that become inoperable on the installation. Contractor shall provide all labor, material, equipment, transportation and supervision to provide repair or recovery, as required.

C. Transportation to the work site(s) for all contract employees shall be at contractor's expense.

D. The contractor shall provide all required scheduled and unscheduled maintenance to buses to include oil changes, brakes, batteries and tire(s) replacement.

E. All maintenance to the buses shall be coordinated as not to interfere with the Army mission requirements. If any bus is not avai1ab1e for any reason, the contractor shall provide a maintenance technician within 4 hours to perform repairs on the un-availab1e vehic1e.

F. The intended use of the vehicles is to transport soldiers to/from garrison and field training environments. The vehicles will be used to transport so1diers with their load bearing equipment to inc1ude rucksacks, and weapons over improved and unimproved roads.

G. Each bus shall have a current registration and license plates to operate on public roads and highways within the state of California. Each bus shall include insurance coverage for damages.

This coverage shall be without deductible, and shall cover only the damage to the buses. The contractor shall authorize unlimited mileage for each bus.

H. The contractor shall, replace un-available vehicles if it cannot be repaired within 24 hours or as requested by the government representative if continued problems arise. Replacement of damaged vehicles must occur within 24 hours. If a replacement vehicle is not made within 24 hours, a computed daily cost credit for each day of failure to make replacement shall be made.

I. Contractor submitted bid costs shall include transportation or delivery charges, if applicable.

J. Government Representative: Government will provide a representative who, on the date of delivery and pick up, will coordinate pick up times/locations and repair work, as required.

K. All vehicles shall undergo a joint inspection at the point of pick up and drop off by both a government and contractor appointed representative. Contractor shall ensure signed copies of delivery and pick up inspection results are provided or contractor shall assumes all liability at the point of contract termination.

L. The contractor shall authorize unlimited mileage for each bus.

M. The contractor shall provide the names and telephone numbers of the person or persons to contact in case of emergencies prior to contract award.

Training Area Conditions Training locations are on both paved and remote sites. Remote sites are considered unimproved land wherein personnel and equipment can be exposed to extremes in weather conditions (extreme temperatures, snow storms, heavy winds, and rain).

Conduct If repairs/replacement are necessary, contractor shall ensure sufficient management presence at the work site to ensure good order, discipline and safety is maintained. The contractor shall be responsible for the conduct of its emp1oyees at all times. All personne1 employed by the contractor in the performance of this contract, or any representative of the contractor entering the government reservation, shall abide by all regu1ations of the installation. The contractor shall be responsib1e for ensuring that any emp1oyee providing services under this contract conduct themselves, and perform services, in a professional, safe, and responsible manner and at all times they behave, act, and perform in a manner consistent with the Fort Hunter Liggett/Camp Roberts/Camp SLO and Camp Parks philosophy or IAW government directives, etc. Revocation of contractor personnel access to the Installation shall not relieve the contractor from the requirement to meet all the terms and conditions of the contract.

Quality Control Quality control is the responsibility of the Contractor. The Contractor is responsib1e for the de1ivery of quality services/supplies to the Government.

Quality Assurance The government will eva1uate the contractor's performance under this contract. When an observation indicates defective performance, the contractor shall correct the deficiency within 24 hours of the observation.

a. Measurable Performance Standards. The contractor shall be measured for compliance to include:

- Performance deadlines are met

- Damage free equipment

- Service calls

- All safety requirements are met

b. The contractor's success and failures in performance may be included in future past performance assessments such as the Past Performance Reporting System (PPIRS).

Safety Contractor personnel performing maintenance/repair/recovery shall not interfere with or impede Government activities. The contractor shall ensure their employees use the appropriate personal protective equipment required by statutory or regu1atory standards. Personal protective equipment shall be maintained in a safe, sanity and operational condition. Employees shall be trained in the use, care and maintenance of persona1 protective equipment.

The contractor shall perform all operations in accordance with the Occupational Safety and Health Act (OSHA) and all applicable Federal and National Standards. The contractor's safety program shall fully comply with the provisions of all Installation, Army and DOD safety regulations, to include, AR 12-9, TB MED 593, TB MED 530, The Army Radiation Safety Program; AR 385-10, Army Safety Program; AR 385-40, Accident Reporting and Records; AR 40-5, Preventive Medicine; Title 29 CFR 1910, 1926, and 1960 Occupational Safety and Health Administration (OSHA) regulation; EM 385-1- 1, copies of Engineers Safety Manual; TM 5-682, Facility Engineer/E1ectrical Facilities Safety and, app1icab1e federa1 state and loca1 standards shall apply. The above mentioned regulations may be accessed at www.usapa.army.mil. The contractor shall be responsible for all damages to persons or property that occur as a result of the contractor's fault or negligence, and shall take proper safety and health precautions to protect the work, the workers, the public and the property of others.

Emergency Medical Treatment: Medical services for contractor personnel shall be the responsibility of the contractor. However, the Government may provide, on an emergency basis, medical services for job related injuries, while an employee is performing under this contract if government facilities are available. Such emergency medical care shall be limited to those services necessary to prevent undue suffering or loss of life or limb and shall be provided only during the period of emergency. In such circumstances, action shall be taken to transfer employees of the contractor to a civilian health care provider as soon as the emergency period ends. The contractor shall reimburse the Government for any emergency medical care and/or MEDEVAC. Following the initial emergency care, the Government is not responsible for any continued or follow up care.

Security Requirements: Some areas are restricted to personnel who have not been properly cleared by federal law enforcement. Consequently, personnel without clearances shall be provided with a U.S.

Government escort during times when contractor personnel are performing work under this contract in a restricted area. To avoid unnecessary delays in the performance of service in restricted areas, the contractor shall arrange for a U.S. Government escort into those areas by coordinating a time/hour with the building/site coordinator. If the coordinator's alternate is not avai1able, the Contractor shall coordinate with the government representative. The Contractor shall notify the government representative when the Contractor's personnel are unable to perform work because escort personnel are not available.

Employee Vehicle Registration Any Privately Owned Vehicle (POV) used by an employee to enter the Installation shall be registered. Proof of employment, insurance and current vehicle registration shall be required. The contractor shall ensure each employee properly registers their vehicle according to the Installation requirement, regulations, and policies.
