5. Description of Supplies/Services: USARIEM is requesting a contract to analyze carbohydrate tracer 6,6,-2H2-glucose turnover from biological samples collected in a recent study (18-09HC). Exogenous carbohydrate oxidation is diminished when unacclimatized lowlanders consume carbohydrate during exercise at acute high altitude (HA) compared to sea level (SL). Though several recent studies have reported similar results, it is unclear what the mechanism is that is causing diminished carbohydrate oxidation at HA. Obtaining a better understanding as to why exogenous carbohydrate oxidation is lowered at HA is important for the development of appropriate dietary recommendations and countermeasures to support acute HA military operations. The current study will require use primed/continuous infusions of the carbohydrate tracer 6,6,-2H2-glucose to measure glucose turnover, examining the rate of appearance and disappearance of exogenous and endogenous glucose in plasma.

6. Authority Cited: 41 U.S.C. 1901 OR 41 U.S.C. 1903

(a) FAR 13.106-1(b)(1)(i): Only one source reasonably available (e.g., urgency, exclusive licensing agreement, brand name or industrial mobilization); OR

(b) FAR 13.106-1(b)(1)(ii): Only one source can provide a portion of the purchase that is a particular brand-name item; OR

(c) FAR 13.501(a)(1)(i): Only one source reasonably available (including brand name); OR

(d) FAR 13.501(a)(1)(ii): Only one source can provide a portion of the purchase that is a particular brand-name item.

7. Reason for Authority Cited: USARIEM requires laboratory services to provide reliable and valid stable isotopically-derived kinetic assessments of glucose turnover, particularly skeletal muscle glucose uptake, and hepatic and skeletal muscle glucose release. The University of Arkansas Medical School (UAMS) possess the experience, expertise, and unique testing and validating abilities to meet this requirement, which will aid in USARIEM’s efforts to identify nutritional approaches to optimize physical performance during HA military operations. More specifically, UAMS is solely responsible for the origination, optimization, and validation of the most commonly used analytical and computational approaches to isotopically trace human substrate metabolism in response to disease, stress, exercise, and nutrition. UAMS has also provided stable isotope kinetic measurements services for USARIEM in the past, which have consistently yielded reliable and valid data necessary to address study hypotheses in order to advance military nutrition initiatives. UAMS’s demonstrated level of scientific expertise provides USARIEM unmatched analytical and interpretative capabilities and oversight to ensure that the data received is fundamentally sound and unquestionably valid for peer-review publication and evidence based nutrition policy recommendations.

As such, UAMS is the only institute with the laboratory possessing the knowledge, technical expertise, and facility required to complete this work, and is the only source available to meet the specific requirements.

8. Efforts to obtain Competition: A Request for Information (RFI)/Sources Sought (SS) notice was posted to Federal Business Opportunities (FBO) on 2 November 2018 for 15 days. UAMS was the only response received and provided their capability statement. The Capability Statement from UAM was reviewed by the technical office and was found to be acceptable on December 7th, 2018.

a. Effective competition. No effective competition is anticipated for this requirement. No other vendors provided capability statements to show they are capable of performing the requirement.

b. Subcontracting competition. There are no subcontracting possibilities with this requirement.

9. Actions to remove or overcome barriers Competition: USARIEM will continue to monitor market to identify sources that may provide the services in the future. An intent to sole source was posted on FBO for 15 days (12/06/18 – 12/20/18) with no responses received.

10. Market Survey: Market research was conducted using internet searches, literature review and discussion with colleagues. This service is offered by a select limited number of contactors to allow for third party data validation of analytical reports. No other analysis of carbohydrate tracer 6,6,-2H2-glucose turnover vendors were identified to have the capabilities to support this requirement.

11. Interested Sources: An Intent to Sole Source to the University of Arkansas Medical School was posted on FBO 6 December 2018 for 15 days. No responses were received as a result of the synopsis.
