Questions:

1.) Is the Army asking for only JV and Mentor- Protege type of relationship to respond or will they also accept a Prime and Subcontractor type of relationship?

2.) If Prime and Subcontractor relationship is formed via a signed teaming agreement can the subcontractor’s Past Performance be utilized and scored along with the Primes past performance to show team capability.

In order for the Government to gauge the experience of the quoter, the response to Subfactor 1E shall be the states in which services have been provided within the past three years. Clarification has been added to the solicitation.

6 Is there a form USMEDCOM would prefer the Quoter use for Factor 2:

Past and Present Performance? Or, should the quoter use their own format, so long as it addresses all of the information requested?
