Sole Source Justification For Acquisitions under the Simplified Acquisition Threshold

1. Contracting Activity: U.S. Army Medical Command (MEDCOM), Health Readiness Contracting Office (Provisional), Joint Base San Antonio, Fort Sam Houston, Texas.

2. Description of Action:

3. Description of Supplies/Services to Include POP or Delivery Date:

This requirement is for a 1-year web access subscription to the APRISS Incorporated maintained Prescription Monitoring Program (PMP) InterConnect database and NarxCheck Plus Analytical tool.

The web access subscription will allow MEDCOM providers who make controlled substance prescribing decisions access to more than 30 states PMP databases for controlled substance prescriptions obtained through cash transactions information on beneficiaries who may be making cash transactions in other states for controlled substances. The information gained will mitigate the risk of Army providers over-prescribing medications.

(a) FAR 13.106-1(b)(1)(i): Only one source reasonably available (e.g., urgency, exclusive licensing agreement, brand name or industrial mobilization); OR

(b) FAR 13.106-1(b)(1)(ii): Only one source can provide a portion of the purchase that is a particular brand-name item; OR

(c) FAR 8.405-6(a)(1)(i)(A): An urgent and compelling need exists, and following the procedures would result in unacceptable delays; OR

(d) FAR 8.405-6(a)(1)(i)(B): Only one source is capable of providing the supplies or services required at the level of quality required because the supplies or services are unique or highly specialized; OR

(e) FAR 8.405-6(a)(1)(i)(C): In the interest of economy and efficiency, the new work is a logical follow-on to an original Federal Supply Schedule order not previously issued under sole-source or limited-sources procedures.

5. Reason for Authority Cited: Appriss Inc. built and operates the PMP InterConnect database along with the NarxCheck analytical tool. The PMP InterConnect database is a hub that allows states to share PMP data with other states. No outside party can access state PMP data unless they are connected to this database through Appriss; therefore, Appriss is the only vendor that maintains the database and is licensed to provide users access to connect to multiple state PMPs and provide analytics data through NarxCheck Plus. The information gained will help US Army providers mitigate the risk of over prescribing medications to its beneficiaries.

Currently, clinical pharmacists especially those supporting Warrior Transition Units (WTUs) and Army Medical Homes, frequently review patient medication profiles for appropriate medication management including monitoring of controlled substance use. Missing from the patient medication prescription profile are retail cash transactions for which beneficiaries may elect not to seek reimbursement;

creating a profile gap in the patients history inaccessible to providers when searching for controlled substance. Through the required system, providers, including clinical pharmacists, can review state PMP databases for controlled substance prescriptions obtained via cash transactions. Obtaining PMP access, a specific agreement between each provider end user and each state, can be an onerous process with limitations and challenges, including separate requests to access neighboring state databases. The administrative burden to obtain access to individual state PMPs, with varying requirements, would be daunting for even a small contingent of providers from each facility. Appriss, in collaboration with The National Association of Boards of Pharmacy (NABP) is connecting state PMPs through their PMP InterConnect interface. Currently, 30 states are connected with 32 projected by 2016 and an ultimate goal of connecting all State PMPs in the future. This interface provides wider access to state PMP databases, while eliminating the requirement to request access from multiple states. At this time, Appriss is the only vendor that maintains the database and is licensed to provide users access to connect to multiple state PMPs and provide analytics data through NarxCheck Plus.

6. Actions to Increase Competition: A pre-solicitation notice was posted to FEDBIZ Ops on 16 Sep 15 that stated HRCO (P) intends to solicit to Appriss Inc. only for a one-year subscription and that quotes or capability statements received from any other vendor would be considered by the Contracting Officer. The notice closed on 21 Sep 15 and no other quotes were received.

7. Market Survey: Research was conducted on the internet and it was found that the interface capability to connect to other state PMPs and controlled substance analytical data is only available thru Appriss Inc. in collaboration with the National Association of Boards of Pharmacy. This interface provides wider access to state PMP databases, while eliminating the requirement to request access from multiple states.

Subcontracting opportunities do not exist for the proposed contract action. This determination is based on the fact that Appriss has sufficient in-house capability to maintain the PMP Interface database and their personnel are employed by them and are not subcontractor employees.
