QUESTIONS AND ANSWERS

Q.1. Page 150 of 193, Paragraph 2.5: References service being required within 24 hours of calling in the service request. Should there be a response with pricing for cost of service 24 hours a day, 7 days a week? Or is this requirement only for weekdays?

A.1. At a minimum, the service plan should offer 24 hours/5 days per week, M-F. Offerors may elect to quote any service over this requirement at their discretion.

Q.2. Page 151 of 193, Paragraph 3.1: References initial on-site operator training to all technicians assigned and training for two (2) additional operators in subsequent years. Will it be acceptable to provide the additional training for subsequent years on-site as well?

A.2. Yes.

Q.3. Item No. 001AH, Simulated Whole Blood Control kit 4x6mL max quantity listed is 52 which only reflects the quantity for 1 analyzer.

A.3. CLIN 0001AH, 1001AH, 2001AH, 3001AH & 4001AH quantities are adjusted to 104 each on the solicitation.

Q.4. There appears to be conflicting language in the Solicitation: 1.1.2 states Each individual analyzer must have fully automated, discrete random access capabilities and perform all tests listed in the solicitation while the bid asks for an "all or none" response.

A.4. This statement is correct. Our requirement is that each analyzer must be fully automated with regards to each of the tests listed in the solicitation, and be able to perform discrete random access capabilities.

** No further questions will be accepted. **

The following have been modified:

ADDENDUM TO 52.212-1

Volume I. Administrative.

Provide the following items:

Completed SF 1449: Blocks 12, 17 (to include CAGE code and DUNS number), 30A - C Point of Contact Information (Name, Telephone Number, Fax Number, and Email) Statement that the offeror will hold its quote firm for at least 90 days from the due date for receipt of quotes, subject to any changes that may occur as a result of discussions if applicable.

Potential contractors must be registered in the System for Award Management (SAM) to be eligible for award. The SAM website is http://www.sam.gov. Offers are due by the date and time established in block 8 of the SF 1449 (4 October 2017, 4:00pm Central Standard Time). Offers received after the deadline will be considered untimely and will not be eligible for award. The deadline for questions is on 30 August 2017, 4:00 PM, Central Standard Time. All questions must be submitted in writing to gregory.s.knight.civ@mail.mil and michelle.m.bartkowski.civ@mail.mil. Offers shall be submitted via e-mail to gregory.s.knight.civ@mail.mil and michelle.m.bartkowski.civ@mail.mil.

Please provide completed provisions, as listed in the solicitation. Alternately, complete all provisions in

SAM and provide confirmation of completion with offer.

Electronic submissions with files larger than 20MB will need to be submitted in separate emails. All emails, regardless of the size, need to be received by the Government server, by the stated date and time. It is the offeror's responsibility to ensure the Government receives the entire quote.

Volume II. Technical Capability.

Factor 1. Technical Capability. Offerors shall demonstrate how their proposed items comply with the Government's Statement of Objectives via the submission of a written capability statement, product literature, or other materials, to include proof where applicable (e.g., Food and Drug Administration approval of equipment and assays).

Subfactor 1A. Instruments/Equipment and Reagents/Consumables.

Subfactor 1B. Service and Maintenance.

Subfactor 1C. Training.

Subfactor 1D. Delivery/Installation.

Volume III. Subcontracting Plan.

Factor 2. Subcontracting Plan. The offeror shall submit a Subcontracting Plan with their quote. If the offeror already has a Commercial Subcontracting Plan approved by a Government Contracting Officer, that plan (along with corresponding approval) is sufficient for submission. In lieu of a Subcontracting Plan, offerors may also submit a statement (with supporting documentation) that subcontracting opportunities do not exist.

Volume IV. Price.

Factor 3. Price. A unit price shall be submitted for each line item as well as the extended price (i.e., unit price multiplied by quantity). Offerors may submit in their own packaging, as long as the total amount proposed is equal to the amount listed in the solicitation. For example, if a CLIN description is for three (3) each 1 x 5 mL bottles (15 mL total), then the offeror may submit for five (5) each of 1 x 3 mL bottles (also 15 mL total). If submitting for other than the size (bottle or package) listed in the solictation, it is the offeror's responsibility to ensure it provides an explanation detailing how their proposed product meets the Government's requirements.

This is an "all or none" solicitation. Failure to provide a quote for any CLIN, unless it is specifically identified as "not separately priced" within the quote, will be construed to mean the offeror is not quoting that CLIN and thus, not eligible for award.

(m) The non-FAR Part 12 discretionary FAR and DFARS provisions included herein are incorporated into this solicitation either by reference or in full text. If incorporated by reference, see provision 52.252-1 herein for locations where full text can be found.
