QUALITY ASSURANCE SURVEILLANCE PLAN (QASP)

FOR

PROGRAM EXECUTIVE OFFICE,

ASSEMBLED CHEMICAL WEAPONS ALTERNATIVES (PEO ACWA)

Contract for Systemization, Operations, and Closure Support (SOCS)

1.0 INTRODUCTION

1.1 Purpose. The role of the Government in quality assurance is to ensure contract standards are achieved. The purpose of the Quality Assurance Surveillance Plan (QASP) is to identify the methods and procedures the Government will use to evaluate contractor actions while performing the requirements in the Performance Work Statement (PWS). It is designed to provide an effective surveillance method by monitoring contractor performance for each listed performance objective in the Performance Requirements Summary (PRS) of this document, as well as those listed in the PWS and Contract Data Requirements Lists (CDRL).

1.2 The QASP describes how the Government will observe, evaluate, and document the contractor’s performance in meeting the tasks identified in the contract. Specifically, the QASP will identify the method for monitoring performance and determining the quality level of services and products delivered under the contract. The intent is to establish and execute a QASP that will assist the Government in obtaining maximum quality for the work performed.

1.3 The QASP has been developed by the CORs and the requiring activities. It is designed to provide direction to personnel performing contract surveillance activities.

Personnel surveying the contract terms and conditions and PWS requirements will periodically review the QASP throughout the life of the contract.

2.0 ROLES AND RESPONSIBILITIES

The following Government resources shall have responsibility for the implementation of this QASP:

2.1 Contracting Officer (KO) - The KO will monitor performance of all necessary actions for effective contracting to ensure compliance with the terms of the contract and the PWS. The KO assures that the contractor receives impartial, fair, and equitable treatment under the contract. The KO is ultimately responsible for the final determination of the adequacy of the contractor’s performance.

2.1.1 The KO is responsible for safeguarding the interests of the United States Government in contractual relationships. Only the Contracting Officer is authorized to bind the Government and then, only to the extent of the authority delegated to them through the issuance of a warrant.

2.1.2 The Contracting Officer delegates authority for inspection and/or acceptance in accordance with the terms of the contract.

2.1.3 The Contracting Officer informs the contractor of the names, duties, and limitations of authority for all quality assurance personnel assigned to the contract.

2.2 Contracting Officer’s (KO) Representative (COR). The COR is responsible for technical administration of the contract and assures proper Government surveillance of the contractor’s performance. The COR is not empowered to make any contractual commitments or to authorize any changes on the Government’s behalf. Any changes that the contractor deems may affect contract price, terms, or conditions shall be referred to the Contracting Officer for action.

2.2.1 The COR is responsible for quality assurance guidance and ensuring that contract quality requirements, provisions, standards, and thresholds are defined, practical, enforceable, necessary, and verifiable.

2.2.2 The COR, relying on input from Task Managers and Subject Matter Experts (SME), evaluates and documents contractor performance in accordance with the QASP and PWS.

2.2.3 The COR notifies the KO of any significant performance deficiencies using a Contractor Discrepancy Report, with supporting documentation.

2.2.4 The COR maintains surveillance documentation.

2.2.5 The COR recommends improvements to the QASP and PWS throughout the life of the contract.

3.0 DESCRIPTION OF SERVICES (Scope of Work)

This contract provides integrated support services to PEO ACWA at the Headquarters (HQ), Aberdeen Proving Ground (APG), Edgewood, MD and three Government Field Offices (FO): the PCAPP, Pueblo, CO; the BGCAPP, Richmond, KY; and the Anniston Field Office (AFO), Anniston, AL. The support contractor shall provide to the Government, administrative and technical services necessary in support of the Government's management of pilot testing, systemization, operations, and closure of the chemical demilitarization facilities to destroy the chemical weapons stockpiles located at PCD, BGAD, and SDC operations (in Anniston, AL). Unless otherwise provided in this PWS, the support contractor’s scope is limited to providing administrative services and technical input to the Government. The support contractor shall provide support for the development of facts and provide a range of analyses or alternatives to the Government for timely, informed decision-making at the HQ and the FOs.

In executing this contract, the support contractor shall provide support, expertise, and experience in chemical demilitarization engineering technology over the various chemical demilitarization project phases (systemization, operations, and closure);

contract management; project controls (cost, schedule, budget, and performance data analysis); quality assurance; risk management; environmental regulation and permitting;

safety, occupational health and emergency preparedness; security and chemical surety;

information technology (IT) and cybersecurity; chemical laboratory sampling, analysis, and air monitoring; operational procedures; acquisition risk management and reporting;

property management; public affairs; records management; and administrative and training support in the aforementioned areas/disciplines to the respective HQ and FOs.

The contractor shall integrate all efforts across PEO ACWA. The Government reserves the right to require a draw down plan to be provided by the support contractor as the projects transition into closure and completion.

This work directly supports PEO ACWA and staff, and shall not duplicate work performed under any other contracts supporting PEO ACWA, to include systems contracts.

4.0 QUALITY ASSURANCE

4.1 Purpose. This section details the methods to be used in verifying contractor compliance with the contract requirements. The key elements of this process are the contractor’s quality control program and government-identified high risk and critical operational requirements.

4.2 Quality Control Program. The contractor, not the Government, is responsible for management and quality control actions to meet the terms of the contract.

4.3 High Risk and Critical Operational Requirements. The PRS contains only those items considered most important for mission accomplishment. Surveillance on these items will occur on a regular basis. CORs may choose to periodically inspect contract requirements not listed on the PRS. The government retains the right to inspect all requirements of the contract at any time.

5.0 PERFORMANCE MEASURES AND STANDARDS

5.1 PRS Performance Objectives, Standards, Performance Thresholds, and Surveillance Methods. The PRS summary identifies the services required by the PWS over which the Government will exercise the most surveillance. The services required by the contractor are summarized into performance that relates directly to mission essential items. The performance threshold briefly describes the minimum acceptable levels of service required for each objective. These thresholds are critical to mission success.

5.2 Cost, Schedule, and Performance. Primary risk considerations to be evaluated are:

cost, schedule (timeliness), and performance (quality, quantity, and effectiveness).

6.0 Surveillance

6.1 Surveillance Approach. The intent of the surveillance approach is to gain confidence in the contractor’s way of doing business and then adjust the level of oversight to a point that maintains that confidence. With this intent, the surveillance approach may not be one that stays the same throughout the duration of the contract.

All government evaluators should be prepared to periodically update the surveillance approach as required.

6.2 Surveillance Documentation.

6.2.1 General. Documentation is required to record, evaluate, and report contractor’s performance. This documentation provides the Procuring Contracting Officer (PCO) with contractor status as it applies to the performance criteria.

6.2.2 Performance Meetings. Performance meetings may be held periodically or as requested by the contracting officer, COR personnel, or the contractor. COR personnel are required to attend these meetings. Notes will be taken by the COR and will be sent to all parties present at the meeting within 3 workdays.

6.2.3 Monthly Surveillance. The COR will perform monthly assessments of the quality and effectiveness of the contractor’s support provided to effectively plan and manage contract requirements; e.g. meeting cost, performance, and schedule requirements of the contract. These assessments will be based the contractor’s data and product deliverables, as well as the contractor’s monthly reports and meetings at the contractor’s and subcontractors’ work sites and/or government work sites.

6.2.4 Monthly Surveillance Reports. COR personnel must document surveillances in the Wide Area Workflow (WAWF) COR Tracking Tool on a monthly basis.

Documenting surveillance is of major importance. The contracting officer cannot properly administer the contract without documentation of actual surveillance. All documentation resulting from surveillance becomes a permanent part of the contract file. COR personnel must provide all monthly surveillance documentation to the KO no later than 5 working days following the month in which the actual surveillance took place for inclusion in the official contract file. All surveillance documentation must be reviewed, signed, and dated by the COR prior to submission to the contracting officer via the WAWF COR Tracking Tool. The Business and Financial Management (BFM) Directorate at HQ PEO ACWA will provide further guidance on the contents of the monthly reports following discussions with all SOCS contract CORs. Items normally contained in this report are: the required service; PWS paragraph number; method of surveillance; and narrative formats for activities, details of critical requirements verifications, noteworthy actions, conversations, etc. and other items needed to document performance. The CORs may employ a “by exception” reporting technique in that only those items that are above or below satisfactory must be reported; all else is assumed to be satisfactory.

6.2.5 Performance Evaluation. Performance of a service will be evaluated to determine whether or not performance meets the performance threshold. The evaluation ratings are as follows:

a. Very Good - Performance meets contractual requirements and exceeds some to the Government’s benefit. The contractual performance of the task and sub-task being assessed was accomplished with some minor problems for which corrective actions taken by the contractor were effective.

b. Satisfactory - Performance meets contractual requirements. The contractual performance of the task and sub-task contain some minor problems for which corrective actions taken by the contractor appear or were satisfactory.

c. Marginal - Performance does not meet some contractual requirements. The contractual performance of the task and sub-task being assessed reflects a serious problem for which the contractor has not yet identified corrective actions.

The contractor’s proposed actions appear only marginally effective or were not fully implemented.

d. Unsatisfactory – Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance of the service(s) contains a serious problem(s) for which the contractor’s corrective actions appear or were ineffective.

6.2.5.1 Unacceptable Performance. Non-conformance with any contract requirement, whether specifically addressed in the PRS or not, is a “defect.” If the number of complaints/defects exceeds the performance threshold for any requirement, the COR will determine the possible cause of this unacceptable performance. Government-caused complaints/defects will not be counted against the contractor. The same applies to any other requirement of the contract when Government-caused complaints/defects are the cause of unacceptable contractor performance.

If the COR judges the contractor’s performance unacceptable for any requirement, the COR will inform the contractor’s on-site representative, and request his or her signature and date of surveillance on documentation acknowledging notification. If the on-site contractor representative refuses to sign, COR personnel shall annotate on the documentation the date and time of notification and name of the representative and his/her refusal to acknowledge. If the contractor disputes the results of surveillance, the COR will inform the contracting officer for resolution or other appropriate action in accordance with FAR 52.212.4, Contract Terms and Conditions-Commercial Items (May 1997) or the appropriate Inspection of Service clause. Significant performance deficiencies must be submitted to the Contracting Officer using a Contractor Discrepancy Report and include supporting documentation. Customer complaints must be validated.

Once the contractor representative has been notified of the unacceptable performance, the preferred course of action is for the COR to require the contractor to re-perform to correct the unacceptable performance, at no additional cost to the government.

Documentation may then be revised and resubmitted, at no additional cost to the government. The revised documentation shall include a written response as to why the performance threshold was not met, how performance will be returned to acceptable levels, and how recurrence of the cause will be prevented in the future.

If the contractor’s re-performance does not meet the threshold in the PRS or the quality stipulated in any other part of the contract, COR personnel would notify the contracting officer immediately. The Contracting Officer will notify the contractor on a monthly basis of any defects relating to cost, schedule, or performance, which must be addressed and corrected to ensure successful contract performance. The contractor shall provide a written response to the cause and the corrective action to prevent recurrence within ten

(10) calendar days after receipt of the Contracting Officer’s notification.

6.3 Surveillance Folder. The COR shall maintain an online COR file using the WAWF COR Tracking Tool. A surveillance folder must be developed and maintained to accomplish contract quality assurance for a performance requirement. The preferred method for maintaining the COR folder will be the WAWF COR Tracking Tool since this system provides back up of the data to preclude accidental loss and is accessible by the PCO. The surveillance folder must contain the following information, but may also contain any other information that the COR finds useful.

6.3.1 Contract.

6.3.2 Quality Assurance Surveillance Plan.

6.3.3 Contractor’s Contract Management Plan

6.3.4 Contractor’s Quality Control Plan.

6.3.5 COR Monthly Surveillance Report as described in paragraph 6.2.4.

6.3.6 Records. All documentation associated with contact quality assurance, e.g.

discrepancy reports (both active and resolved), Contractor’s monthly reports, Contractor’s trip reports, technical/study reports, DA Form 5477-R-Customer Complaint forms, correspondence, receiving report inputs, letters of interpretation from the contracting officer, etc.
