The reason for this amendment is to add a specification and to provide a REVISED Statement of Work for the Wood Compactor requirement.

The added specification is as follow s: The machine must be able to run on a 208/120V pow er supply. The REVISED STATEMENT OF WORK is ATTACHED. All Amendments MUST be signed and returned w ith proposals in order for proposals to be considered. Vendor questions can be accepted until Wednesday, Dec. 2, 2020. Proposals w ill be accepted until Tuesday, Dec. 8, 2020. All other terms & conditions remain the same.

Statement of Work

Wood Compactor General Scope

This Statement of Work (SOW) is for the turn-key design, supply, delivery, assembly, installation and training of Wood Compactor to include the supply, delivery, installation, documentation and personnel training for a Wood Compactor. The machine will be located at Bldg. 66 and shall meet or exceed all applicable industrial, Federal, State, and Local standards and codes. The Contractor shall provide on-site machine training for operator personnel and maintenance personnel.

ACCESS TO INSTALLATION SITE (Bldg. 66): The site location for the compactor will be outside of Bldg. 66 in the Recycling Yard at Tobyhanna Army Depot.

SITE VISIT: Interested Contractors can request a site visit prior to submitting a proposal.

Wood Compactor Machine Specifications, Characteristics and Features

1. The compactor should be able to fit with accompanying container on 10’x 40’ concrete pad.

2. The machine must be fully capable of sitting outside in the open without any cover.

3. The machine must have be able to run on a 208/120V power supply.

4. The machine must be able to accept pallets of up to 60” x 96”in dimension, crates at least 48” x 48” x 60” in dimension and wood spools of up to 60” in diameter.

5. The machine must not directly use fossil fuels.

6. The machine should allow it to be fed primarily by forklift truck.

General Requirements

Delivery, Assembly, and Installation

1. It shall be the responsibility of the Contractor to contact the TYAD Fire Department to obtain a Burn

Permit prior to conducting welding, torch cutting, or soldering.

2. The Contractor shall be responsible to supply all labor, materials, supervision, tools and equipment to deliver, off-load, locate and place into position, and anchor all component pieces, materials and supplies necessary for the turn-key assembly and installation of the wood compactor. The Contractor shall be responsible to uncrate and unpack all supplied equipment. The Contractor shall be responsible to remove from TYAD all packing, packaging and crating materials.

3. All equipment, tools, materials, supplies and component pieces shall be delivered concurrently; no partial deliveries are acceptable.

4. The Contractor can perform all post-delivery work during normal 1st-shift hours: Monday—Friday, 7:00 AM—4:00 PM, but shall not interfere with normal production. Deliveries shall be coordinated by the Contractor with the COR 10 working days, minimum, in advance of delivery.

5. The Contractor shall adhere to all TYAD Environmental, Safety, and Fire standards, codes and regulations.

Furnishing of personal protective equipment (PPE) to Contractor personnel shall be the responsibility of the Contractor.

6. It shall be the Contractor’s responsibility to supply and deliver a dumpster for waste collection. Location for a dumpster shall be coordinated with the COR prior to its arrival. The Contractor shall remove the dumpster within 3 calendar days of work completion.

7. For all Contractor-owned electrical equipment that requires battery recharging it shall be the responsibility of the Contractor to supply his own battery-charging equipment. Connection of Contractor-supplied battery-charging equipment to TYAD power shall be the responsibility of the Contractor.

8. The Contractor shall have 180 days ARO to install the wood compactor and complete all operator and maintenance personnel training. The Contractor shall commence installation of wood compactor within 3 working days of arrival of the equipment.

9. The Contractor shall submit to the COR a project time line 10 working days ARO.

10. The Contractor shall be responsible for start-up and verification of system.

Warranty

1. The Contractor shall warrant wood compactor, all ancillary pieces of equipment, and installation parts and workmanship for a period of 4 calendar years following final acceptance by the COR. During this warranty period all parts costs, labor costs, transportation and shipping costs, travel costs, tool costs and per diem for all Contractor personnel shall be the responsibility of the Contractor. The Contractor field repair technician shall arrive on-site within 24 hours after being contacted by TYAD personnel to make warranty repairs during the warranty period. The Contractor field repair technician shall arrive on-site within 24 hours after being contacted by TYAD personnel to make repairs for the life of the machine at TYAD. The Contractor shall supply Contact Telephone Numbers and Points of Contact (POCs) for use by TYAD personnel to contact the Contractor for warranty, operation and maintenance questions and support.

2. All pieces of equipment that have a manufacturer’s warranty that exceeds the 4 year warranty period such warranties shall be provided by the Contractor to the Government at the expiration of the 4 year warranty period at which time it will become the responsibility of the Government to submit warranty claims.

Documentation

1. The Contractor shall provide to the COR 3 hardcopies each of the Operation and Maintenance (O&M)

Manuals and 3 hardcopies each of the Parts Manual. The Parts Manual shall include a list of parts— including consumables—that are recommended by the manufacturers to be kept on-hand at all times.

These parts shall be identified by nomenclature, part number, quantity, and pricing. The O&M manuals shall contain complete detailed instructions and schedules for system PM. All documentation shall be provided to the COR prior to final acceptance being given. O&M manuals shall contain a detailed description of the lock-out/tag-out procedures needed to bring the wood compactor to a zero mechanical state for maintenance and servicing. Procedures for returning the wood compactor to full operational capability shall also be provided.

Training

1. The Contractor shall provide machine operator training for 4 ea. Environmental Branch personnel.

Training shall be conducted by the Contractor’s qualified training representative. Training shall be done on-site and personnel shall be trained in the full operation and operator-recommended maintenance.

2. The Contractor shall provide maintenance training for 2 ea. Logistics Support Division (LSD) personnel.

Training shall be conducted by the Contractor’s qualified training representative. LSD personnel shall be trained in the full maintenance and PM of the wood compactor. LSD personnel training shall occur prior to installation of the wood compactor.

3. All materials and documentation used in the training of operator personnel and LSD personnel shall become the property of the Government.

General Acceptance

1. Upon completion by the Contractor of the installation of the wood compactor shall be the responsibility of the Contractor to verify and show the wood compactor, all ancillary components are operating within manufacturers’ specified parameters and specifications.

2. The Contractor or his qualified representative shall demonstrate the functioning of the wood compactor and ancillary components by successfully shredding/compacting a pallets of 60” x 96”in dimension, a crate at least 48” x 48” x 60” in dimension and a wood spool of 60” in diameter. The demonstration will be witnessed by Environmental Branch operators; the Environmental Branch supervisor; the COR; and a technical representative of the COR’s choosing. Demonstration shall be coordinated with the COR so as to permit all necessary personnel to be present. All defects encountered prior to or during the demonstration shall be immediately corrected by the Contractor. Only upon correction of the defect will the demonstration be allowed to continue.

3. Only following the successful demonstration of the wood compactor; conclusion of maintenance personnel and operator personnel training and furnishing of all manuals to the COR will general acceptance be granted to the Contractor.

Final Acceptance

1. Final acceptance will occur upon acceptance in Wide Area Workflow (WAWF) of the Contractor’s submitted invoice by the COR. The warranty periods associated with the wood compactor and ancillary components will begin on the date of final acceptance. Final acceptance will be granted only after general acceptance has been given to the Contractor.

Contractor Proposal Requirements to the Solicitation

A Contractor’s submittal to the solicitation shall contain explanatory descriptions to all aspects listed below and these descriptions will be used by the Government for technical review and evaluation of the submitted proposal. A single statement of Contractor compliance will not be accepted in lieu of an explanatory description and shall disqualify the Contractor for contract award consideration. A Contractor’s submittal not in compliance with all aspects of the Contractor Proposal Requirements to the Solicitation will not be considered and will result in the Contractor being disqualified from consideration for contract award.

1. A Contractor considering submission of a proposal to the solicitation may schedule a site visit, the site visit is highly recommended, but not required. All dimensions and dimension verifications are the responsibility of the Contractor.

2. The Contractor proposal shall indicate the training that will be provided for operator personnel and maintenance personnel as well as the training durations. The Contractor shall provide a detailed description of maintenance personnel and machine operator personnel training programs.

3. The Contractor proposal shall include 3 references of prior projects of similar scope and complexity along with points of contact (POCs) for each project and their telephone numbers.

4. A rough time line shall accompany the Contractor proposal.

Antiterrorism (AT)/Operations Security (OPSEC) Requirements

1. Antiterrorism (AT) Level I Training. All contractor employees, to include subcontractor employees, requiring access to Army installations, facilities and controlled access areas shall complete AT Level I awareness training within 30 calendar days after contract start date, and within 30 days of report date for any additional or new contractor employees, who start after that period. The contractor shall submit certificates of completion for each affected contractor employee and subcontractor employee, to the COR within 10 calendar days after completion of training by all employees and subcontractor personnel. AT level I awareness training is available at the following website: https://atlevel1.dtic.mil/at. All contractor employees must also complete annual AT awareness training. Upon request, the contractor will be provided one copy of the AT level I training slides for use with employees who may not have computer access.

TYAD will not be responsible for printing copies of slides for training purposes. TYAD reserves the right to provide instructor led training and will so advise the contractor within one week of the start of performance or at the time of annual training, if that will be the training approach to be utilized.

Completion of contractor employee training will be documented on ELTY form 538, Training Record, or contractor equivalent.

2. iWATCH Training. The contractor and all associated sub-contractors shall brief all employees on the local iWATCH program (training materials will be provided by the TYAD Antiterrorism Officer (ATO)).

This local developed training will be used to inform employees of the types of behavior to watch for and instruct employees to report suspicious activity to the COR. This training shall be completed within 30 calendar days after contract start date, and within 30 days of report date for any additional or new contractor employees, who start after that period. The contractor shall submit certificates of completion for each affected contractor employee and subcontractor employee, to the COR within 10 calendar days after completion of training by all employees and subcontractor personnel. TYAD reserves the right to provide instructor led training and will so advise the contractor within one week of the start of performance or at the time of annual training, if that will be the training approach to be utilized.

3. Operations Security (OPSEC) Training. Per AR 530-1, Operations Security, contractor employees must complete Level I OPSEC training within 30 calendar days after contract start date, and within 30 days of report date for any additional or new contractor employees, who start after that period. All contractor employees must also complete annual OPSEC awareness training. The contractor shall submit certificates of completion for each affected contractor employee and subcontractor employee, to the COR within 10 calendar days after completion of training by all employees and subcontractor personnel. The contractor will be provided a copy of the OPSEC training slides for use with employees who may not have computer access. TYAD will not be responsible for printing copies of slides for training purposes. TYAD reserves the right to provide instructor led training and will so advise the contractor within one week of the start of performance or at the time of annual training, if that will be the training approach to be utilized.

Completion of contractor employee training will be documented on ELTY form 538, Training Record or contractor equivalent.

4. Access and General Protection/Security Policy and Procedures. Contractor and all associated sub-contractors employees shall comply with applicable installation, facility and area commander installation/facility access and local security policies and procedures (provided by government representative). The contractor shall also provide all information required for background checks to meet installation access requirements to be accomplished by TYAD Security Division. Contractor workforce must comply with all personal identity verification requirements as directed by DOD, HQDA and/or local policy. In addition to the changes otherwise authorized by the changes clause of this contract, should the Force Protection Condition (FPCON) at any individual facility or installation change, the Government may require changes in contractor security matters or processes.

5. The contractor Facility Security Officer (FSO) will include Threat Awareness and Reporting Program (TARP) requirements in their initial and refresher training IAW AR 381-12, paragraph 1-14 and Chapter 2.
