About this file

This is an applicant work plan template document issued by the Virginia Department of Health (VDH) for the Virginia Rural Health Transformation Program (RHTP) Community Paramedicine & Mobile Integrated Healthcare initiative. The template is designed for organizations applying to implement Community Paramedicine (CP) and/or Mobile Integrated Healthcare (MIH) projects during an initial award period extending from the date of award through September 30, 2027. Applicants are required to submit comprehensive work plans that translate their implementation approach into practical project management tools, organizing activities in logical sequence with clearly identified milestones, deliverables, responsible parties, realistic timelines, and performance measures aligned with both core community paramedicine performance metrics and project-specific measures. The work plan must demonstrate consistency across the project narrative, budget, staffing approach, partnership structure, and proposed performance measures, with special attention to phased or multi-year projects that establish foundations for future implementation or expansion.

The template requires applicants to document goals and objectives, major activities and tasks, anticipated deliverables, implementation timelines, responsible parties and partners, and applicable performance metrics. Additionally, applicants must identify significant dependencies and assumptions that may affect implementation, including staffing and credentialing requirements, medical direction, partner agreements, referral workflows, technology needs, data access, and vehicle or equipment readiness. The work plan serves as both a planning document and an assessment tool for tracking progress throughout the initial award period, with the flexibility for organizations to use this template or submit a comparable format that includes all required information.
