Project Number: 550-13-102 Solicitation Number: VA69D-17-B-1750 Description: Renovate 58-5 Endoscopy Suite

AMENDMENT A00005

The following RFI questions have been submitted regarding this solicitation, representing all questions submitted to the Contracting Officer prior to the deadline established in Solicitation No VA69D-17-B-1750. Bidders are instructed to submit their bids in response to the solicitation “as-is”, taking into consideration all Amendments issued to date. Any remaining issues will be addressed with the successful bidder. Questions and responses are provided for bidder’s information and use.

RFI QUESTIONS/ANSWERS:

1. Drawing 58.AI105 Finish Legend gives Manufacturer Product Information with exception of VCT-1. Can you provide information on Manufacturer – Style-Color for this item. If this cannot be determined can you provide an acceptable alternate for this work.

RESPONSE: The intent of VCT selection is to match the existing corridor finish flooring. Armstrong imperial texture VCT or similar is acceptable. Provide product submittals for approval per specifications.

2. Please confirm flooring layout as reflected on drawing 58.AI105 - Waiting Room 507A where it reflects a transition VT-2. It looks like that needs to be in the doorway between the carpet and the vinyl flooring. If that is correct, then does the 2’ border get moved over and the infill piece get larger? Please explain what flooring goes in that area between VT-2 and the Consultation Room 507.

RESPONSE: Yes, the assumption is correct.

Extend field area shown in room 507A (Sheet 58.AI105) as LVT-1 to a location 2’-0” from wall shown between Waiting Room 507A and (adjacent) Consultation Room 507. Additional Clarification: 2’-0” dimension shown is typical for the border tile indicated.

2’-0” is measured from primary wall surface, not chases or bump-outs into room. Provide vinyl transition at the door opening (to 507) where the material changes.

3. The wall tile spec (CWT-1) calls for the color “Uptown Taupe” which is Dal Tile’s 4x4, 6x6 and 3x6 sizes. I am unable to locate tile in the 8” x 10” size. The bullnose and base spec are OK, just the field tile is the issue. Please advise.

RESPONSE: For the wall field tile: 6” x 6” is acceptable.

4. Please confirm the required material for Window Type A found on Sheet 58.AE701 and/or supply a Specification.

RESPONSE: Window type ‘A’ (also referenced by Interior Elevation 27 on Drawing 58.AE501) is a stainless-steel double hung pass-thru window. Basis of Design (B.O.D.) for this window is listed in Response #5.

5. Please provide detail for Window Type A as shown on drawing 58 AE701

RESPONSE: Basis of Design for Pass-Thru Window type ‘A’ is TBJ Incorporated, Pass-Thru window model SPD-44-38-PTW. Installation to comply with attachment ‘TBJ model-spd-44-38-ptw.pdf’.

6. Drawing 58.HA101 states to “remove and dispose of approximately 45 linear feet of 2” to 6”diameter asbestos-containing pipe insulation identified through the course of interior renovation/demolition...” Will any amount above and beyond the stated amount be addressed as an add in a contract modification per Spec Section 02 82 13.13 -1?

RESPONSE: Quantities of asbestos to be included with BID AMOUNT are shown by SPECIFICATION SECTION 02 82 13.13, GLOVEBAG ASBESTOS ABATEMENT and, SECTION 02 82 13.19, ASBESTOS FLOOR TILE AND MASTIC ABATEMENT.

Quantities of ACM for abatement shown by SPECIFICATIONS supersede those listed by drawings.

REGARDING GLOVE BAG ASBESTOS ABATEMENT QUANTITY

Refer to Specification SECTION 02 82 13.13, GLOVEBAG ASBESTOS ABATEMENT page 1, paragraph 1.1.2, EXTENT OF WORK.

Contractor is advised to include IN BID AMOUNT the quantities of ACM abatement Specified.

Contractor’s attention is also drawn to SECTION 02 82 13.13, GLOVEBAG ASBESTOS ABATEMENT, page 3, ASBESTOS CONTAINING MATERIAL UNIT PRICING LIST (GLOVE BAG TECHNIQUE).

Contractor is advised to include the referenced ASBESTOS CONTAINING MATERIAL UNIT PRICING LIST with Bid Documents.

REGARDING ASBESTOS FLOOR TILE AND MASTIC ABATEMENT QUANTITY

Refer to Specification SECTION 02 82 13.19, ASBESTOS FLOOR TILE AND MASTIC ABATEMENT SPECIFICCATION, page 1, paragraph 1.1.2, EXTENT OF WORK.

Contractor is advised to include IN BID AMOUNT the quantities of ACM abatement Specified.

7. Drawing 58.HA101, it states that “Contractor must field verify all site conditions and estimated quantities prior to the submission of a bid if preliminary demolition allows the asbestos-containing TSI to be quantified.” Who is to perform the preliminary demolition prior to the submission of the bid? When are the results of the above stated demolition to be posted prior to the submission of the bid?

RESPONSE: Pre-Bid asbestos survey (‘Field Verification’) is NOT to be performed.

8. Is the Contractor to include an asbestos survey prior to beginning work? Will ACM found during this survey be addressed as a contract modification?

RESPONSE: An asbestos survey prior to beginning abatement work is a Specified Requirement. Refer to ASBESTOS Specifications referenced above in response to RFI Question #6.

ACM discovered during demolition (or construction) activities – will be addressed as a contract modification – IF the amount discovered to need removal - varies from the quantities SPECIFIED.

9. 58.AD 104 – Note 1 – Denotes “signs.” What is note one intended to be? Please clarify.

RESPONSE: REMOVE ‘SIGNAGE PLAN NOTES’ indicated by Drawing 58.AD.104 and insert the following:

‘DEMOLTION PLAN NOTES

1. Remove existing ceiling system in area shown as required to access new plumbing work indicated for floor above. Coordinate work to occur with Plumbing and Electrical work indicated.’

Contractor’s attention is also drawn to related work including Drawing 58.GI204, FOURTH FLOOR INFECTION AND DUST CONTROL PLAN and Drawing 58.HA101, CLASS 1 ASBESTOS GLOVEBAG ABATEMENT – FOURTH FLOOR DEMOLITION.

10. What is the anticipated date of Award/NTP for this project?

RESPONSE: Estimated award date of 12/31/2017 Estimated Notice to Proceed issuance date will be 15-60 after award date.

11. What are the measurements from the drop ceiling to the deck in the area of renovation?

RESPONSE: The measurement from the ceiling grid to deck is 3’8” (floor to ceiling grid = 7’11”; floor to deck = 11’7”).

12. In analyzing the Hardware Schedule and on it, reads the Hardware Groups No. 11 & 12 is for the same opening (513C). According to that Hardware Schedule it reads that (513C Opening) is a pair in the Hardware group No. 12 but also lists it as a single opening in hardware group No. 11. According to the door schedule the only Pair of doors is the 508A opening, which is it? Also I don't have a Door type or Frame type for a Pair of doors listed/represented. Also is the 508A opening a Pair of two 4'" x 7' 0" and is their a frame or mullion in-between this Pair?

RESPONSE: Hardware Group #11 corresponds to Opening #513C.

Opening #508A is the only door pair in the S.O.W.

Each leaf of door pair #508A measures 2’-0” x 7’-0”. Door Schedule shown on Sheet 58.AE701 indicates opening #508A to be a wood door with hollow metal frame elevation ‘A’ and type 12 hardware.

13. Since revised drawings have been issued in Amendment 3, will the deadline for RFIs be extended?

RESPONSE: No, however if there are question(s) submitted prior to the bid opening, and the Government deems relevant, there will be an Addendum issues responding to the question(s).

14. On Drawing 58.AI205 Note 3, it states that there is to be a “room sign with slider,” however it is not placed on the wall. Is this sign intended to be placed on the wall just west of its current location? If not, please indicate where this sign is to be hung.

RESPONSE: Yes. It is to be placed on the wall, west of its current location.

15. What is the timeframe for government review and approval (or comment) of submittals? This information is needed for scheduling purposes.

RESPONSE: Refer to VHA MASTER SPECIFICATIONS: SECTION 01 33 23 and SECTION 01 33 24

16. Where will the laydown area and dumpster be located?

Response: See attached drawing titled “Laydown and Dumpster Location Aerial View”.

17. Please identify the proposed construction crane location.

RESPONSE: Following Contract Award – The Crane location is to be proposed by Crane Contractor – and approved by Contracting Officer’s Representative. Variables to be considered include the type and size of crane proposed for use, reach of proposed crane, duration of crane’s position on site, and available ground area.

18. Where are the SS-2 - 1/4" thick wall panels to be used in Room #513B, 513C and 513D? There is a conflict between the elevations on page 58.AE501 which show it on all the walls and the finish plan on page 58.AI105 which shows it on designated walls.

RESPONSE: Provide and install 1/4" thick Solid surface wall panels in Rooms 513B, 513C and 513D as shown by Interior Elevations, Sheet 58.AE501.

19. Informed by Corian rep that Pebble is not made in 1/4" thickness. The only colors made in 1/4" thickness are Bisque, Bone, Cameo White, Glacier White, Vanilla, Linen, Venaro White and Glacier Ice.

RESPONSE: Provide product submittal with standard color choices available for the specified 1/4" thick solid surface materials. Color will be selected/confirmed by the COR as part of the Submittal Process.

20. Sheet 58FA001 Fire Alarm Riser Diagram, what does the 15 & 16 stand for in the octagon box

RESPONSE: Please disregard the keyed notes (‘octagon box’) #15 and #16 shown by plan 58FA001.

21. Sheet 58EP101-M1 shows the Scale as being 1/8, is that correct or is it to be ¼

RESPONSE: The correct scale for Drawing 58EP101-M1 is 1/4” = 1’-0”

22. Sheet EY101 shows the Scale as being 1/8, is that correct or is it to be ¼

RESPONSE: The correct scale for Drawing 58.EY101 is 1/4” = 1’-0”

23. In reviewing the plans and specs for the VA project, it was discovered that drawing for the asbestos removal for fifth floor (58.HA104) shows only the removal of floor tile and mastic. The specification section (02 82 13.13, 1.1.2, B, 1) references the removal of pipe insulation 50 hangers on the fifth floor. Question is: Is there actually pipe insulation removal on the fifth floor and not shown on the drawing (58.HA104).

RESPONSE: It is possible that certain materials - including pipe hangers - located on the 5th floor are ACM.

Refer to Specification SECTION 02 82 13.13, GOVEBAG ASBESTOS ABATEMENT page 1, paragraph 1.1.2, EXTENT OF WORK for quantities to be included with Bid Amount.

Quantities of ACM for abatement shown by SPECIFICATIONS supersede those listed by drawings.

RESPONSE: Plumbing contractor.

25. Does RO water require insulation same as Domestic Cold Water, or is it to be left bare?

RESPONSE: Yes, it should be insulated the same as Domestic Cold Water.
