STATEMENT OF WORK
B&F PROJECT 1D1C
REPLACE USP OVENS
FCC HAZELTON, WV
A. SUMMARY OF WORK & PLACE OF PERFORMANCE
The Federal Bureau of Prisons (FBOP), Federal Correctional Complex (FCC), located at 1640 Sky View Drive, Bruceton Mills, West Virginia intends to make a single award to a responsible entity that will provide all necessary equipment, labor, materials, and incidentals to install 5 Baxter single rack ovens, Model No. OV500G1EE at FCC Hazelton. It is the contractor’s responsibility to contract local authorities and obtain permitting, should it be required. The ovens are located at the United States Penitentiary (USP) Food Services Department.
B. SCHEDULE
a. All work is to be completed between the hours of 2:30 PM - 11:00 PM, Eastern Standard Time (EST) Monday - Friday, excluding weekends and federal holidays.
b. Any work required to be completed at any other time than noted above must be scheduled five (5) working days prior to the start of the work and approved by the Contracting Officer.
c. No work will be conducted on weekends, or federal holidays unless submitted in writing to the Contracting Officer five (5) working days prior to the start of the work and approval is granted.
C. CONTRACTOR RESPONSIBILITIES
a. Contractor shall be responsible for the procurement and delivery of all equipment
b. Contractor will be certified/authorized by the manufacturer to perform installations/repairs.
c. Contractor shall take all necessary precautions to ensure against any/all damage to the existing property. Any damaged items or property shall be returned to original condition by the Contractor via repaired and/or replacement at no additional cost to the Government.
d. The contractor will ensure all manufacturers recommendations are implemented for this project.
e. Contractor shall provide a designated Project Manager (PM) for administration of this project while installation is in progress.
f. Contractor will be permitted to wear jeans in the institution, however, green or khaki-color clothing will not be allowed. The clothes will be proper and suitable for the services that are being provided.
g. Lunches will be consumed at the work site. Contractor shall ensure they have everything they need for their lunch when they check-in first thing in the morning.
h. Smoking, vaping, and smokeless tobacco use on Federal property states that the previously mentioned actions are only allowed in designated areas. This policy will be strictly enforced.
i. The contractor will be responsible for notifying the COR if for any reason there will be an interruption of service.
j. The contractor is responsible for removal of all debris prior to final invoice submission.
k. The contractor is responsible for adhering to all safety regulations.
l. Contractors will NOT be allowed to bring cell phones or other electronic devices into FCC
Hazelton, exceptions to this rule would be if new equipment required an electronic device to set up new ovens.
m. Contractors and employees shall have and wear the required personal protective equipment (PPE) prescribed by industry standards at all times while on the job site. It is the responsibility of the contractor to provide all PPE equipment
D. GOVERNMENT RESPONSIBILITIES
a. FCC Hazelton shall ensure all energy sources needing to be secured to prevent personal injury and/or property damage will be locked-out and tagged-out with the appropriate safety hardware. All system isolations and Lockout/Tag out measures to be implemented, and/or removed, will require written verification by the Contractor, the Facility Manager, and the Safety Manager prior to doing so.
b. FCC Hazelton shall ensure a representative is present to allow access to all area’s requested by the Contractor as needed.
c. FCC Hazelton shall ensure that the Contractor is escorted at all times while within the institution.
E. SECURITY
1. DELIVERY OF MATERIALS
a. All materials/equipment required for work on this project is required to be processed by the Contractor and coordinated with the Contracting Officer Representative (COR) for temporary storage.
b. No additional construction related deliveries are permitted to be shipped to the contractor at FCC Hazelton.
c. Under no circumstances will the Federal Bureau of Prisons or its staff be responsible for the acceptance of any Contractor or company deliveries.
d. The Contractor shall be present during delivery to the Warehouse to ensure and accept equipment deliveries.
e. The Contractor shall use the institution’s USP rear loading dock ramp for the loading, unloading. The contractor will not impede/block the rear dock ramp at any time during or after the work shift. The contractor will not utilize the rear dock for storage of equipment.
2. STORAGE OF MATERIALS AND EQUIPMENT
a. The contractor will be allowed to store material/tools on site. Tools will be inventoried by front lobby officer upon initial entrance of the institution. The contractor will be allow to bring said tools in a secure rolling tool box, which will be inventoried by Facilities staff at the end of each shift and secured in the Facilities department each day until the project is complete.
b. The Federal Bureau of Prisons (FBOP) will not provide a secure lay-down area for storage of equipment and material from the Contractor. The FBOP does not take any responsibility for the security of such equipment and materials.
3. SECURITY CLEARANCE/CONTRACTOR BADGES
a. All contractors/sub-contractors (no exceptions) working on the grounds of FCC Hazelton will be required to conform to requirements set forth in the Homeland Security Presidential Directive (HSPD-12). All applicable HSPD-12 paperwork must be filled out in its entirety.
Once the contract has been awarded, all necessary HSPD-12 paperwork will be sent to the awarded contractor.
b. The prime contractor will be responsible to provide the necessary information for security clearance on the HSPD-12 paperwork for employees and sub-contractors. A Point of Contact will be designated after contract award. HSPD-12 documentation will not be accepted from a sub-contractor.
c. In the event a contractor’s employee does not pass the security clearance, it shall be the contractor’s responsibility to submit another employee for security clearance.
d. All security clearances shall be submitted to a designated person (Facilities Assistant), a minimum of ten working days after receiving the packets. Security clearances will be in effect for the term of up to one year, prior to the expiration period ending September 30 of each calendar year.
e. All contractors and sub-contractor employees will provide a check list of all employees on site-each day to the COR, verifying the number of workers on site each day and that all workers are pre-approved to be on the site.
F. UTILITIES
The FBOP will provide electricity service, restroom services and water.
G. INSTITUTION SECURITY
Contractor personnel shall wear clothes that are suitable for the work they are performing. The inmate population wears khaki and olive green clothes. For this reason, no Bureau of Prisons employees or contract personnel can wear these colors. No sleeveless shirts (tank tops) or camouflage clothing is allowed.
The Contractor will be required to be escorted at all times while entering and working within the institution. Items will be restricted to the equipment/tools required to complete the contract action only. Electronic devices or weapons of any kind shall not be permitted within the institution at any time. The Contractor shall be constantly aware of security and maintain a professional relationship with the inmates at all times if they come in contact with the inmate population.
H. GENERAL PROJECT REQUIREMENTS
Contractor shall unhook/detach all connections for the removal of 5 Baxter Ovens to the rear loading dock, in which FCC Hazelton will dispose of. Contractor will then move 5 new Baxter ovens Model# OV500G1EE into the building and erect in their final locations, and make final utility connections to machine to include electric, gas, and ventilation. The contractor will also be responsible for removing all packing debris to the loading dock for disposal by FCC Hazelton Staff. Work hours will be 2:30pm – 11:00pm, which will be straight time and not be considered overtime. Finally, contractor will start-up ovens, make final adjustments. Contractor shall be able to use institution dumpsters. FCC Hazelton will replace roof exhaust vents. FCC Hazelton will be responsible for any necessary repair to the existing floor beneath current ovens.
The Contractor will be escorted at all times starting at the USP rear gate to gain access to the USP rear loading dock. The contractor will be able to utilize the loading dock area to unload the equipment. From this point they will bring the equipment into the food service area using the roll up garage style door.
The Contractor is welcome to do a site visit prior to bringing any equipment to verify the route.
The successful Contractor shall provide labor, equipment, parts, materials and supplies to provide the following:
• Furnish and install to working order 5 Baxter Rotating Rack Ovens model OVS00GIEE, gas, curved front holds (1) single rack.
o 3 of these – Oven body shipped assembled. SHPAYD o 2 of these ovens – Oven body shipped split. SHPSPD
• HTS0N1 Natural gas burner, standard
• ELE0KG 440-480V/60/3ph, 2.2-2.4 amps, heating circuit, also requires 120v/60/1ph control circuit (USA)
• LFT00B "B" style lift, standard
• BACYES Manual backup control
• PRNYES1 Prison Safety Package, for single rack oven, includes tamper resistant hardware, lockable control cover with clear acrylic window, separate hasp for service panel access
• OV500T1-FLTKT1 Type 1 grease filter package for UL Listed hood – single rack units
• KOSHER-KIT Kosher control package
• One year parts and labor warranty with additional 9 years for heat exchanger tubes, standard
• Start up, commissioning and verification of the oven operation, including manufacturer acceptance report.
• The Contractor must meet or exceed the requirements in the SOW
I. FIELD QUALITY CONTROL
At the time of project completion, the contractor shall engage a factory authorized service representative to inspect field-assembled components and oven installation, including piping and electrical connections and perform start up services. The factory authorized service representative shall perform start up services and witness testing and/or operations as necessary to certify the system has been installed according to the manufacture’s recommendations. Reports showing results and all documentation associated with this verification shall be submitted to the Owner at completion. Operations / Maintenance manuals, manufacture warranty and construction warranty documents shall be given to the Owner at time of project completion.
