About this file
This is a Statement of Work (SOW) template document SPB0005 for COVID-19 Testing and Analysis services under the State of Wisconsin Department of Administration's contract vehicle. The SOW establishes the framework for agreements between the State of Wisconsin as the Authorized User and individual contractors, consolidating previously separate COVID-19 testing efforts from the Wisconsin State Emergency Operations Center, UW Madison, and UW Systems into a unified enterprise-wide solution.
The contract covers comprehensive COVID-19 testing and analysis services including test kits, testing services, laboratory services, specimen collection, and operational support for both PCR and antigen testing. Services are delivered through a regional pricing model with hourly and site fees that vary across five defined regions, with payment made monthly based on contractor rate cards from the master contract. The SOW has a term running until August 29, 2022, with costs determined by the contractor rate card or lower negotiated rates. Contractors are responsible for all materials, equipment, PPE, transportation, and access unless specifically provided by the state, and all travel expenses are included in the pricing with no reimbursement provided by the Authorized User.
