1. PROJECT DESCRIPTION and INTENT
For several years the Seattle US Courthouse (USCH) has been plagued with various leaks and short term fixes. In 2014 and 15 minor stop gap repairs were made, weep hole extensions were installed as one of the more economical solutions to correct deficiencies identified in the 2014 report. In 2018 the skylights were re-caulked again, but the leaks have persisted and traveled by location. These leaks seem to appear only with high winds, and not with a deluge of straight down rain. As the water intrusion seemed to becoming progressively worse, a roofing contractor was hired to conduct further investigations. The roofing contractor believed that the membrane was leaking and not the curbs, and recommended destructive testing. On February 27 and March 1, 2019 representatives from ECH (Architecture), Wetherholt (Building Envelope Consultant), and Pacific Rainier Roofing (Sheet Metal/Roofing Contractor) conducted preliminary site visits for initial reviews and observations to confirm the curbs were not the only problem.
The Scope of Work plans for the full replacement of the existing roof installed in 2004, with a new "Styrene-Butadiene-Styrene (SBS) Modified Bituminous Membrane Roofing.", per the plans dated February 28, 2020 and specifications dated 28 February 2020 by ECH Architecture.
The intent is as follows:
1.1 Replacement and repairs of existing roof system at USCH Seattle main entrance roof.
This work will include replacement of current existing roofing with a "Styrene- Butadiene-Styrene (SBS) Modified Bituminous Membrane Roofing.", replacement of existing skylights, installation of new structural members as required to raise skylights to include the associated flashing, installation of pavers and the replacement and repairs to interior finishes in the skylight wells below.
1.2 Mechanical, electrical, and security equipment will be shut down and/or temporarily removed by the contractor for the sole purpose of installing the new roofing system. All equipment will be reinstalled per current conditions.
1.3 Reference the contract documents 100% contract drawing and specifications dated February 28th, 2020. And all drawing notes for additional information and restrictions.
1.3.1 Reference 01 23 00 – Alternates part 3 (A), (B), and (C) for the Base Bid design and alternate’s #1 and #2.
1.3.2 Reference section 07 52 16, 2.3.A.1/2.4.A.1 for the "Styrene-Butadiene-
Styrene (SBS) Modified Bituminous Membrane Roofing." And Drawing A503 detail 8-10.
1.3.3 Reference the contract documents stated in 1.3 for additional work restrictions and contractor plans required.
1.4 Reference the contract documents stated in 1.3 for all the items to be protected, re-used, and or stored offsite.
1.5 Reference the contract documents stated in 1.3. Art work, furniture, and all ancillary finishes that cannot be sufficiently protected per your plan may need to be removed and stored off site. The contractor shall coordinate with GSA to properly document, record, and communicate their planned submittal for review.
1.6 Reference the contract documents stated in 1.3. The contractor shall provide GSA with crane pick plans, crane certifications and crane operator certificates prior to any lifting on site.
1.6.1 The Contractor shall provide all City of Seattle street closure and parking permits as needed to execute their work, to include fees and documentation.
1.7 The contractor shall provide complete waste removal:
1.7.1 A chute(s) with dust control measures shall be approved by the GSA PM.
1.7.2 Any dumpster(s) aloud to remain onsite per/pending your approved plan shall be closed with a hard lid, lockable and secured when not in active use.
1.7.3 Any toilet facilities aloud to remain onsite per/pending your approved plan shall be lockable and secured when not in active use.
2. PERIOD OF PERFORMANCE
2.1. Performance period for all work is 180 calendar days from the receipt of the initial Notice to Proceed (NTP), to include:
2.1.1 30 calendar days for administrative requirements, security processing and submittals
120 calendar days for construction
30 calendar days for closeout
2.2. Reference the contract documents stated in 1.3. Government Occupied Hours:
Government personnel are scheduled to occupy the building during the following hours on weekdays, Monday through Friday, except for established Government Holidays: 8am to 5pm. All work shall be performed during Government Unoccupied Hours. Exceptions to the stated work days or hours shall have prior written approval by the Contracting Officer (CO) with the concurrence of the Contracting Officers Representative (COR), 5 days in advance of the request.
2.2.1 Weekday’s access to the inside of the building must be prior to 4:00 pm for overnight work. During overnight work, leaving and re-entering the building will not be allowed. Coordinate access with the GSA Property/Project Manager at least 5 days prior to overnight request.
3. PROJECT REQUIREMENTS
The specified items of the project are described as follows:
3.1. Construction
3.1.1. The contractor shall provide all materials, equipment, tools, supplies, labor, and supervision necessary to construct the approved designs, per the plans dated February
28, 2020 and specifications dated 28 February 2020 by ECH Architecture, for General Services Administration, known as the Contract Documents (CD’s). All work shall be in accordance with the CD’s, this SOW, PBS Key Sustainable Products (when applicable, see section 6), GSA and Industry Standards, to include, but not limited to:
a. The Contractor shall remove from the Courthouse all waste and dispose per GSA waste diversion policy. See Specifications section Division 01 74 19 and waste policy in GSA GreenConstruction.pdf
b. The contractor shall provide all testing necessary for the complete installation of the design, with manufacturer approval;
c. The Contractor shall procure the services of third party entities to remove, install, test, balance and commission the reinstalled HVAC, electrical and security installations
d. The Contractor shall provide training for the Property Management Office (PMO) and Operations and Maintenance (O&M) Staff on roof maintenance requirements and schedules.
3.1.2 General construction expectations:
The contractor shall:
a. Work shall be completed in a workmanlike manner by individuals experienced in the trades they are performing.
b. Employ safe work practices and control environmental pollutions, including but not limited, dust and odors, for building users and occupants.
c. Maintain safe, clean, orderly work site during all operations.
d. Comply with and maintain the security of the building, to protect the tenant spaces, occupants and the public.
4. GENERAL REQUIREMENTS and EXPECTATIONS
4.1 Pre-Construction Meeting. GSA will host an administrative meeting within ten (10) working days from the contract award date to review contractual and technical issues, objectives of the project and communication protocols. This meeting will include the discussion of schedules, submittal dates, site conditions and requirements and establish points of contact prior to the commencement of work. At the meeting, the contractor shall provide evidence that all employees and subs are in the process of being badged (See Security section 4.10) and provide for approval any revisions in the Project Schedule or PMP, per section 6 below. As a minimum, the following items shall be discussed:
a. Confirm all preliminary documentation and badging is in process
b. Clarify outstanding questions
c. Review security and safety issues
d. Review the Initial RO List and identify POC
e. Schedule, phasing, integration, and coordination
f. Schedule site visits and additional discussions
g. Site conditions and rules
h. Section 5 submittals
4.2 Progress Meetings. The Contractor shall be available to meet with the GSA Project Manager upon request to present deliverables, discuss progress, exchange information and resolve emergent technical problems and issues. These meetings shall take place on site or a location deemed convenient for all parties. The Contractor shall keep GSA informed, through meetings or reports, onsite discussions or emails of progress and potential problems with the project. The Contractor shall keep an open dialog with the COR, particularly in areas relating to the project, schedule slippage, or any potential conflicts regarding project standards. The Contractor shall bring such matters to the attention of the COR as soon as he/she is aware of the issues.
4.3 Building Tenants. The building will be occupied by federal staff and the public during the construction phase. Minimize disruptions and ensure safety to building tenants and the public, to include, but not limited to: Securely store tools and materials out of public or non-public areas; keep supplies and materials in their staging areas. Provide dust and odor containment; ensure noisy or vibrating work is planned to ensure no disruption to Government operations and work.
4.4 Daily Cleanup. Contractor personnel shall clean the area open to public or the tenant and remove dirt and debris from worksite and building, as a minimum, at the end of each day. The building disposal system may not be used for construction waste. Contractor shall provide waste removal for construction debris.
4.5 Utilities. Minimal water and electric service will be provided to the Contractor at no cost.
4.5.1 1 hose bib and 1, 110 outlet; see contract documents for additional requirements.
4.6 Protection. The Contractor shall provide all engineering controls to protect all areas being worked on or around from the migration of dirt, dust, oils and spray, and shall return all areas to the original condition, to include the landscape.
4.6.1 The Contractor shall provide all labor, material, and equipment necessary for the protection of personnel, landscaping, equipment, or building structure from damage.
The Contractor shall replace or repair to the original condition any items damaged due to work performed under this contract, at no additional cost to the Government.
4.7 Work Site Access. Entrance to the worksite shall be determined depending on the material and accessibility to bring in. No equipment will be aloud inside the building without prior approval/load study. Contractor must provide temporary covering to protect floors, walks, and outside/inside features. Refer to specifications section 01 50 00 Part 3 Execution.
4.8 Safeguard of Equipment, Supplies, and Materials. The Government is not responsible for any loss or damage to materials, supplies, tools, or equipment on the site, including materials in which salvage value may have been reflected in bid.
4.9 Site Closeout Requirements. All items under this paragraph shall be completed prior to submitting the final invoice for payment.
4.9.1 Clean, patch, or repair any all surfaces the contractor disturbed to the condition expected in a normal, commercial cleaning and maintenance program.
Ensure cleaning covers path of entry noted above in 4.7.
4.9.2 Construction debris, contractor tools, equipment, and materials shall be removed from the jobsite.
4.10 Security:
4.10.1 All workers shall pass a background investigation prior to the start of the work.
a. The GSA Project Manager will provide the successful Offeror with a Requesting Official (RO) Contractor Approval List (RO List) for completion by the contractor for all personnel to be employed at the work site. This list shall be returned to the Project Manager/COR within 10 days of contract award. The Contractor shall provide the following information on this list for approval:
i. Full legal name
ii. Position title
iii. Dates (Duration) required on the project
iv. Subcontractor name
v. Escorting information (if allowed by GSA)
vi. Type of security investigation requested
b. The RO List is a living document and shall be updated as needed by the contractor’s Person of Contact (POC), identified by the contractor during RO List preparation and submittal. If an individual is not on the RO list, that person will not be considered for an investigation. It is the contractor’s responsibility to maintain and amend the RO List.
4.9.2 Following review and approval of the RO List, the Project Manager/COR will prepare a Master, Contractor Information Worksheet (MCIW) and forward the RO List and MCIW to GSA’s Zonal Help Desk, to initiate the security clearance process.
a. The MCIW will be partially completed by the Project Manage/COR; The
Contractor will provide all information in section 1 and, in section 2, as needed (POC information and subcontractor Information);
b. The Zonal Help Desk will provide the contractor with the electronic MCIW and form OF 306.
c. The Zonal Help Desk will provide training to the contractor’s POC;
d. Paper copies of CIW’s will not be accepted. The CIW is required to be submitted in MS Word document (Version 4 12/2010 Scanned PDF documents will not be accepted.)
e. Form OF 306 shall be signed and submitted in PDF format;
f. All documents provided to GSA shall be encrypted;
g. Submit both the completed CIW’s and signed OF 306 electronically to: zoned-hspd12@gsa.gov:
i. Do not submit CIW and OF 306 forms for individuals not previously identified on the Requesting Official Contractor Approval List. If additional employees are required, contractor shall provide an updated Requesting Official Contractor Approval List to the GSA Project Manager/COR for approval.
ii. Following receipt of a validated and legible CIW and OF 306 by the Zone D Help Desk, the information from the CIW will be entered into GSA’s database, and the contractor will be sponsored. Once sponsored and notified via email, the contractor employee(s) shall go on-line and make an appointment at any shared service credentialing office to get their fingerprints taken. iii. Once an employee is approved, an adjudication email will be sent to the contractor and employee stating that OPM FSEM has determined that this individual is fit to provide 6 months of contractor support to the General Services Administration mailto:hspd12@gsa.gov mailto:hspd12@gsa.gov http://www.fedidcard.gov/centerlocator.aspx
(GSA) starting on (TBD) .
iv. All workers shall have a photo ID and a copy of their OPM adjudication email with them at all times when they are on the work site.
v. All workers shall confine operations to areas permitted.
4.11 The Contractor shall not perform work on the jobsite until the investigation process is complete.
4.12 The Contractor shall maintain building security at all times. Provide temporary closures, barricades and signage to prevent unauthorized entry during non-working hours.
5. SUBMITTALS
5.1 Project Management Plan (PMP). The contractor shall submit one (1) electronic draft copy of the PMP to the COR for approval, five (5) calendar days from receipt of contract award. The COR will approve or provide comments why the PMP was not approved at the Pre-Construction meeting. If required, the Contractor shall submit the revised PMP within two (2) calendar days after receiving the disapproved PMP comments. The Contractor shall submit updates within three (3) calendar days of any changes to the PMP. Contractor shall include the following components in the Project Management Plan:
5.1.1 Project Schedule. Submit proposed Project Schedule showing all project milestones and dates for Government meetings, reviews and inspections. Daily project reports shall be submitted within a time frame designated by the COR at the pre-construction meeting and shall include any unplanned disturbance of existing building finishes, materials, and fabrics must be reported.
5.1.2 Project Directory. Submit a list identifying all participants, affiliation, title, address, telephone number, cell phone number, fax number, email address, and badging status.
5.1.3 Key Personnel. The On-Site Superintendent is hereby designated as Key Personnel under this Task Order. Identify key personnel, experience, and specific role of each in this project. The contractor shall not replace key personnel without acknowledgement of the GSA Contracting Officer (CO). Before replacing any individual designated as KEY by the Government, the Contractor shall notify the CO no less than 15 calendar days in advance, submit written justification for replacement, and provide the name and qualifications of any proposed substitute(s). Contractor shall notify the CO within 24 hours upon receiving notice of KEY individual submits resignation. All proposed substitutes shall possess qualifications equal to or superior to those of the KEY person replaced. The Contractor shall not replace KEY personnel without acknowledgement from the CO. The Government may designate additional Contractor personnel as KEY at the time of award.
5.1.4. Project Work Plan (PWP). Submit in MS Word, a management approach to the project and contingencies for on-site conditions, should any occur. Include the site layout plan, hazardous material management plan, survey report and waste recycling plans.
5.1.5. Quality Control Plan (QCP). The contractor shall submit a viable, documented plan for self-monitoring of the project quality. GSA will perform random Quality Assurance inspections throughout the duration of the project to assure compliance with the QCP. QCP shall include, but not limited to the following as applicable:
a. Demonstration of standards;
b. Finish samples;
c. Material submittals with log;
d. Mock-up demonstrations with log;
e. Schedules to be approved by the COR, indicating dates for GSA review, evaluation and acceptance prior to closure or work progress;
f. Testing, certification, programming, commissioning and documentation.
5.2. Submittals and logs. The Contractor shall prepare for GSA approval and acceptance, all product submittals prior to use of any materials on the jobsite, per the submittal schedule and procedures in specifications section 01 33 00, attachment 1 and to include but not limited to:
a. Submit one (1) electronic copy of all products SDS and Cut Sheets or as mock-up of the installation or item.
b. Submittals must be accompanied by a letter of transmittal from the Contractor to the Government.
c. All products shall consider the environment, both interior and exterior, and recycled or “green” technology shall be priority, per Presidential directives.
1. All products shall be low or no emitting, volatile organic compounds (VOC), unless no other products are available for installations. If no other products are available, the Contractor shall provide a plan to mitigate short and long term exposure risks to building occupants, the Public and the environment.
1. Upon approval of submittals, which require approval prior to installation, one set will be retained at the job site until the project is complete. The Contractor shall comply with all submittals scheduled in the plans, specifications and the SOW.
5.3. Project Closeout Submittals: The Contractor shall submit prior to final invoice submission:
5.3.1 Release of Claims: One (1) electronic and one (1) hard copy.
5.3.2 One (2) year warranty letter. (075216, 1.11 B (1)) Contractor shall provide GSA with manufacturer or labor warranties that cover periods exceeding the 2-year warranty period, one (1) electronic and one (1) hard copy;
5.3.3 As-built drawings and “redline” set. As-built drawings shall be in CAD format meeting GSA file standards.
5.3.4 Testing, balancing, certification, programming, commissioning and testing documents, to include the Manufacturer’s Representative inspection reports.
5.3.5 O&M Manuals
6. INSPECTION AND ACCEPTANCE CRITERIA
6.1. The Contractor shall notify the PM via e-mail a minimum of five (5) calendar days prior to the following construction milestones, as applicable:
a. Protection measures and crane set up
b. Delivery of materials or samples, for GSA review;
c. Progress and/or final inspections;
d. Contractor QC inspections and results;
g. Substantial Completion. The contractor shall formally initiate and request Substantial Completion review and acceptance five (5) days prior to the anticipated Substantial Completion date. The contractor shall submit with this request a punch-list identifying the remaining work to be performed and the dates these items shall be complete.
h. Defects and Omissions list: generation and closeout.
6.2. Contractor shall submit Defects and Omissions List to PM at least five (5) working days prior to schedule close out meeting/completion of project. Upon receipt of the punch list, GSA will inspect the jobsite. Based on the Contractor’s punch list and GSA’s inspection, GSA will provide the Contractor with any additions or comments within two (2) days.
7. REGULATIONS AND STANDARDS
7.1 Applicable Regulations, Standards, Codes, Manuals. The contractor shall perform the work in accordance with the following, including, but not limited to:
