1.3.1 Not Applicable.

2. REFERENCES:

2.1 Standard Items, (FY22)

2.2 COMFLTFORCOMINST 4790.3 Rev C, Joint Fleet Maintenance Manual (JFFM), Volume IV, Chapter 10

3. REQUIREMENTS:

3.1 Work on ship systems and components will be authorized and controlled by the Work Authorization Form (WAF) process in accordance with Attachments A, B and C. A WAF is required to authorize the start of work through completion, or until no longer requiring isolation or authorization. Work includes all maintenance, repairs, alterations modifications, and installation or removal of temporary support systems and equipment authorized for accomplishment pursuant to the Job Order. Non-intrusive work (e.g., painting, lagging, sheet metal work) that does not affect ship or personnel safety does not normally require a WAF. If there are any questions regarding whether a WAF is or is not required, NNSY Code 246 (CTE) has final authority.

3.1.1 Submit a filled out WAF for each required work authorization to the NNSY Code 246 representative. The NNSY Code 246 representative will assign a serial number. NNSY Code 246 together with the watch/duty officer will determine if adequate isolation and plant/system conditions exist to safely and properly conduct the work, authorize and hang tag out(s), and sign the WAF. The WAF is then authorized by NNSY Code 246 and the Ship's authorizing officer. The WAF will remain in the work authorization log until the work is complete and the WAF is cleared by the ship. Additional sign-off required by the WAF for work complete, testing, and closed out, shall be made as the work progresses.

3.1.2 Submit Technical Work Document(s) (TWD) (block 6 of the WAF) with the WAF when necessary to show or explain the job description.

3.1.3 For any revision or change to the WAF or TWD, verbally notify NNSY Code 246 at the time of revision or change. Submit a copy of the revision or change to NNSY Code 246.

3.1.4 Provide representative(s) on a daily basis to coordinate the requirements of this work item with NNSY Code 246. For large job orders (work in a main space or work in multiple spaces, compartments or areas, for 7 or more days), this shall be the representatives full time function; for small job orders (work in one location, generally for less than 7 days), this can be a collateral assignment of an individual.

3.1.5 Submit all requests for Work Authorization using the WAF, along with all supporting Technical Work Documents and Drawings to NNSY Test Code 246; drawings and documents will be returned to the contractor. (Code 246 is the central point of contact and liaison with Ship's Force (SF) for all Work Authorization and Tag-out issues.) Any subcontractors must submit a separate WAF even if work is on the same component. Do not contact SF directly for processing WAF or tagouts.

3.1.6 The Integrated Schedule Job Order and Key Op must be annotated on the top of the WAF. The Job Order and Key Op are the Project's job tracking numbers.

3.1.7 WAF's shall be submitted at least 14 days prior to the contractor's actual scheduled start date of each work item. Contractor completion dates shall be determined based on completion of all production work, all associated WAF(s) cleared and all shipboard tags removed (5 days after WAF is signed off). NOTE: Contractor "Need by Date" must be annotated on the top of the WAF. "Need by Date" must agree with Contractor's start date on the Contractor s Production Schedule.

3.1.8 A WAF shall be required for all contractor work items including installation of DC plugs, except as listed below. Work not normally requiring WAF controls:

3.1.8.1 Preventative maintenance, when integrity of a ship system is not being disturbed. Code 246 must be verbally informed prior to the accomplishment of the maintenance in order that system status may be checked.

3.1.8.2 Painting, cleaning or surface preparation for painting (including sandblasting) that is not in way of movable parts of ship's equipment (i.e., radar, SPM, gun turrets, sail planes, rudder) or adjacent to cathodic protection cells.

3.1.8.3 Non-intrusive work inside tanks (example: painting, inspections) when there is a previously approved WAF to open/close the tank.

3.1.8.4 Removal and/or installation of insulation, staging that is not in the removal, installation, or alteration of workbenches (not attached to a ship's system), lockers, non-structural bulkheads, furniture or joiner work.

3.1.8.5 Removal, installation or adjustment of label plates, wire way hangers and cable bands.

3.1.8.6 Installation of temporary systems not connected into ship systems and not installed for the maintenance of ship conditions.

3.1.8.7 Replacement of fluorescent lamps, regular screw in light bulbs, starters, and bayonet type sight glass light bulbs.

3.1.9 Code 246 shall act as Repair Activity Representative for all contractors and their subcontractors work requests. After the tagout preparation by SF has been completed, SF will notify Code 246. Code 246 will concur on the adequacy and accuracy of the proposed isolation and sign as Repair Activity Representative. After tagout actions have been completed, the SF authorizing officer and Code 246 will sign the WAF, and Code 246 will release the WAF to the contractor for accomplishment of work.

3.1.10 Components or equipment with Danger or Caution tags attached will not be worked, operated or removed. If a component to be worked has a Danger or Caution tag attached, contact Code 246 immediately for resolution.

3.1.11 Tags of color, size and similar shape to a Danger or Caution tag (NAVSHIPS forms 9890/8 and 9890/5 respectively) shall not be used by contractors or subcontractors.

3.1.12 Should a component which is Danger or Caution tagged (NAVSHIPS forms 9890/8 and 9890/5, respectively) be accidentally operated or moved, notify SF and Code 246 immediately so that any corrective action may be taken to protect personnel and equipment.

3.1.13 Do not start any work until you have received the official copy of the WAF from Code 246 to begin work. Blocks 13 and 14 must be signed at that point. If the isolation is electrical in nature, provide personnel to perform an electrical frisk and sign the Frisking Remark in the restrictions block prior to receiving the official copy of the WAF. Ensure all restrictions/precautions are noted in the restrictions/precautions block.

3.1.14 Maintain a copy of each WAF received from Code 246 at each shipboard location where a WAF is required.

3.1.15 Requirements of 009-24 of 2.1, which reference tagout functions for a contractor or their designated representative, are not applicable.

3.1.16 Upon completion of work, the contractor or their subcontractor shall contact their work authorization representative, who will contact NNSY Code 246 in order to sign the completion block of the original WAF. This signature signifies that all work specified by that WAF has been completed and that the contractor is ready for tags to be cleared.

3.1.17 Work requiring testing must have all associated WAFs signed as "Work Complete". Ensure the test boundary is in place and any collateral work, WAF and/or testing is complete. NNSY and SF concurrence must be obtained prior to test performance on the project's Daily Test Schedule or Daily Production Schedule.

3.1.18 Contractors and their subcontractors shall sign the contractor/vendor letter of understanding with Code 246 prior to starting work on the project. NNSY Code 246 will provide an initial training session (approximately one hour) for the contractor's key personnel (i.e., supervisors, work leaders, or designated training coordinators); key personnel will receive training material during the initial training session. The contractor will then be responsible to ensure that they provide the necessary training, including subsequent/refresher training as required, to any other employees/subcontractors subsequently assigned to perform work on ships for NNSY.

3.1.19 Submit 2 legible copies of a completed list of personnel trained, one copy each to NNSY Code 246 and the SUPERVISOR. List is due prior to commencement of work aboard the ship. Submit updates to the list each time personnel are added and trained, and when subsequent/refresher training is provided.

Contractors/vendors will not contact Ship’s Force directly to prepare WAFs, perform tag-outs or clear tags.

In the preparing of the WAF, the following conditions are applicable:

(1) If the contractor/vendor utilizes a serialized TWD (uniquely numbered or AIM entered work document) to accomplish work, the contractor/vendor shall provide Code 246 with a draft Attachment (1) WAF, TWD, and supporting documentation (e.g., drawings, Shipalt records, etc).

(2) If the contractor/vendor does not utilize a serialized technical work document to accomplish work, the contractor/vendor shall provide Code 246 with a completed Attachment (2) Contractor/vendor TWD and provide supporting documentation (e.g., drawings, Shipalt records, etc.). Code 246 will serialize the Contractor/Vendor TWD and process as any other TWD.

Note:
When the contractor/vendor submits a WAF form, Code 246 may, at CTE discretion, either utilize the contractor/vendor provided WAF or transfer the WAF information to a Contractor/Vendor TWD for processing.

c. Code 246 will evaluate the submitted TWD and supporting documentation to determine if the described work is intrusive or non-intrusive shipboard work.

(1) For nonintrusive work or non-shipboard work, the TWD will be annotated No WAF Required (“NWR”) in the ‘WORK RELEASE’ block and signed by Code 246.

d. The signed document will then be released to the contractor/vendor. Upon receipt of the annotated TWD, the contractor/vendor may commence work.

(2) For intrusive shipboard work, Code 246 may release the serialized TWD or Contractor/Vendor TWD by processing a new WAF isolating the work or by attaching the work to an existing WAF, as directed by reference (c).

(a) If a new WAF (generated by the contractor/vendor, Code 246, or in combination) is utilized, Code 246 shall process the WAF through Ship’s Force. Code 246 will indicate in the TWD the WAF isolating the work, sign as released and attach a copy of the WAF to the TWD.

(b) If the work is attached to an existing WAF, Code 246 shall process the WAF by adding the TWD to the WAF (adding the serial number to the TWD Line Item Record sheet). Code 246 will indicate on the TWD which WAF is isolating the work, sign as released, and attach a copy of the WAF to the TWD.

(c) The contractor/vendor shall receive the original TWD or a copy of the contractor/vendor TWD with Code 246 authorization signature. A copy of the WAF will be attached and shall be retained on the worksite while performing work. The contractor/vendor shall also maintain a copy of each WAF with the active Technical Work Document (TWD) and Drawings (or Contractor TWD as detailed in LOU) until the work is completed. Upon receipt of signed authorized documents, work may start.

(d) Should special requirements exist (i.e. work on energized gear, depressurization and draining of systems) the contractor/vendor will be prepared to assist Code 246 to meet the requirements of reference (c) prior to the release of the WAF. This may mean the contractor/vendor will complete applicable entries on forms, or indicate specific drainage/depressurization points to verify system status. Should any of these be required, the Code 246 Work Control personnel will inform the contractor/vendor at the earliest possible time.

(e) Code 246 will evaluate the proposed work and isolation to identify abnormal conditions, testing, concurrent work or test which could cause hazardous conditions to personnel or equipment. Should such conditions exist which would preclude expedient work release, Code 246 Work Control will inform the contractor/vendor of the condition and expected delay in processing.

(f) If work conditions change, such that a stoppage in work is required, Code 246 will issue a STOP WAF as per reference (c). The contractor/vendor will place the system being worked in a safe condition to stop work when issued a STOP WAF and sign the form. All listed work on the STOP WAF will be stopped until the STOP WAF is cleared. Code 246 may require the contractor/vendor to return the TWD and WAF isolating such work until the STOP WAF is cleared and authorization to resume work is granted.

When notified, the contractor/vendor will sign to release the STOP WAF and can then resume work.

6. Completion of Work:

a. Upon completion of the work the contractor/vendor shall supply Code 246 with a copy of the TWD signed indicating work is complete, or sign the Work Complete block on the original Contractor/Vendor TWD form. Code 246 personnel shall sign for work complete on the associated WAF after satisfactory review, as applicable.

b. Code 246 and Ship’s Force will complete the actions necessary to clear associated tags and line items. The contractor/vendor is advised clearing the contractor/vendor work may not clear all hanging tags, if tags referenced in other work documents for isolation. If notification of tags removal is desired, the contractor/vendor will have to check with Code 246 to find out if tags have been removed. Notification is not automatic.

7. APPROVAL FOR AUTHORIZED TESTING & OPERATION:

a. The contractor/vendor is advised that the operation of ship’s equipment is not authorized.

The operation of valves and components will be performed by Ship’s Force or Code 246 within system transfer boundaries when a system transfer WAF is in effect. The operation of any tagged valve or component will result in a critique offense without issuance of warning.

b. Authorized contractor/vendor shipboard testing requires the following actions:

(1) WAF clearance: The work requiring testing must have all associated TWDs signed as ‘Work Complete’. Code 246 will evaluate WAFs to determine if the system status is appropriate to support testing.

(2) Pretest Checks: Contractor/vendor test procedures must include prerequisite requirements to ensure the test boundary is intact and any collateral work, WAFs, and/or other required testing has been completed.

(3) JTG / NN Concurrence: When system / component testing is required, the contractor/vendor shall provide Code 246 with the applicable test procedure information. The test procedure unique number and title must be listed on the List of Authorized Tests (LAT) for authorization by the Joint Test Group (Non-Nuclear) (JTG / NN) prior to testing, if the JTG / NN is assigned. Only those tests listed on the LAT (concurred on by the JTG / NN, if assigned) are authorized for execution. The contractor/vendor may be requested to attend JTG / NN meetings to discuss the details of testing they desire to perform.

c. When system/component testing in accordance with a TWD or test procedure involves the operation of remotely controlled equipment where safety observers are required to be on station, the contractor/vendor shall fulfill the requirements of reference (d) regarding a Controlled Evolution.

(1) Examples of operations which would require a Controlled Evolution include: Operating torpedo tube muzzle doors, ejection pump doors, stern, fairwater & bow planes, rudders, vertical launch hatches, thin line/fat line towed array, secondary propulsion motors, main ballast tank vent valve/blow systems, anchors, antennas, radars, launchers, periscopes, and masts.

(2) Code 246 is the final authority in determining if a Controlled Evolution is required. The conduct of a Controlled Evolution involves completing a Controlled Evolution Form (available from Code 246 and in accordance with reference (d)), designating a Person In Charge (PIC), assigning safety observers and supplying a detailed description of the evolution.

d. Code 246 and Ship’s Force concurrence must be obtained prior to the start any testing evolution. The contractor/vendor is responsible for obtaining Ship’s Force support as needed for testing and/or Controlled Evolutions.

8. TRAINING:

a. Code 246 will provide the initial training for contractor/vendor personnel. Upon completion, the contractor/vendor will be provided the training materials to identify roles, responsibilities, and basic work requirements while at NNSY. The contractor/vendor will sign that such training has been completed. No work will be released or performed until this training is completed.
b. The contractor/vendor will be provided a copy of reference (c) and (d) and a copy is maintained at Code 246 Work Control. Copies of references (a) and (b) will be provided, if requested. This is not intended to be the sole training material for contractor/vendor employees but supplemental to their own training requirements.

c. The contractor/vendor is responsible to conduct training of all employees assigned to perform work in the requirements of references (a) through (d), and this document prior to performing shipboard work. The contractor/vendor is required to keep a record of employees trained and provide copies to Code 246 (Record of Training attached may be used).

d. Any questions regarding the processing of WAFs, control of work, tag-outs, or control of testing should be directed to NNSY Code 246.

e. Any contractor/vendor sponsored by and contracted to perform work on board the
