SECTION A - GENERAL INFORMATION FOR OFFERORS

Paragraph 1 replaced with:

1. The scope of the contract consists of NSNs 2510015601159 and 2510015612540. Split Award Contracts are possible. Due to high quantity and repetitive demands on past buys, the Government may place a maximum of two awards. This initiative is for Indefinite Quantity Contract(s), which will provide DLA-Direct coverage (stocked by DLA) throughout the Continental United States—CONUS.

Paragraph 3 replaced with:

3. The proposals received in response to this solicitation will be evaluated under “Lowest Priced Technically Acceptable (LPTA)” source selection procedures in accordance with (IAW) FAR 15.101-2 as described in Section M.

Please note that all pages of the solicitation must be completed and returned to the Bid Room.

The Government may elect to make a split award by awarding two contracts under this solicitation in order to maintain multiple viable manufacturing sources to preserve competition. If a split award is made for these NSNs, the Government baseline requirements is to order quantities via the percentage allocations for each delivery order. In the event an LTC vendor is unable to perform on a delivery order in accordance with its applicable split award allocation, the Contracting Officer may issue a delivery order to the other split award LTC vendor that is in excess of that LTC. Compliance to the split allocation will be measured over the contract term on a yearly basis. Orders authorized by the Contracting Officer under this paragraph to a single (i.e., the other) LTC vendor shall not count towards the percentage allocation requirements of that LTC vendor’s contract.

SECTION B – SCHEDULE OF SUPPLIES

See Attachment B-1 Section B Pricing, Delivery, and Packaging Spreadsheet

EXPLANATION OF PRICING TAB Replaced with:

Pricing – Prices shall be FOB Origin and shall include applicable packaging costs. Price columns are set up for the First Year only. The base period consists of three years, but each of the years is individually priced.

Offeror’s Unit Prices: (Offeror fill-in) In the event an offeror opts to not propose on a specific percentage of ADQ, the offeror should so indicate by stating “No Bid” for the percentage ADQ. The proposal will not be evaluated for that split. An offerors decision to enter a no bid for a certain percentage ADQ will not preclude its proposal from being considered for an award. The pricing must include packaging as specified in PIDs (Procurement Item Descriptions) in this solicitation. Delivery is specified by clause DLAD 52.211-9054 (Delivery Tab on Section B spreadsheet).

YEAR 2-5 Percentages: (Offeror fill-in) Please propose a percentage increase/decrease for each of the

PAGE 2 OF 9 PAGESCONTINUATION SHEET REFERENCE NO. OF DOCUMENT BEING CONTINUED:

SPE7M115R0023-0001

subsequent years. This percentage is used to adjust the unit prices annually for each of the subsequent years.

After application of the percentage increases, the results will be rounded to the nearest cent. For the Year 2, these percentages will be applied to the Year 1 prices. For the Year 3, these percentages will be applied to the Year 2 prices, and so on.

COLUMN NAME - EXPLANATION

CLIN - Solicitation Line Item.

NSN - National Stock Number.

Nomenclature - Name of Item.

UI - Unit of Issue.

ADQ - Annual Demand Quantity OFFEROR’S UNIT PRICE (100% of ADQ) – Offeror’s price for the specified quantity of ADQ and the equivalent proportion of future Government demands for the NSN.

OFFEROR’S UNIT PRICE (60% of ADQ) – Offeror’s price for the specified quantity of ADQ and the equivalent proportion of future Government demands for the NSN.

OFFEROR’S UNIT PRICE (50% of ADQ) – Offeror’s price for the specified quantity of ADQ and the equivalent proportion of future Government demands for the NSN.

OFFEROR’S UNIT PRICE (40% of ADQ) – Offeror’s price for the specified quantity of ADQ and the equivalent proportion of future Government demands for the NSN.

Page 5 of Original Solicitation:

CONTRACT QUANTITY LIMITATIONS Replaced with:

CONTRACT QUANTITY LIMITATIONS

Split awards – The Government anticipates making split awards (awards for the same NSNs) as a result of this solicitation. If split awards are made, the contract minimum for each award during the contract period* will be a proportion of the overall minimum cited below. For example, if two awards are made at a 60/40 split the Government is obligated to purchase 60% of the minimum from the one awardee and 40% from the other. Regardless of the number of awards made under this solicitation, the Government may order, and each awardee is obligated to deliver, up to the maximum cited below:

The contract minimum will be $95,862.43, which is the total of the minimum estimates for all NSNs. The minimum estimate for each NSN is calculated as three(3) per cent of the estimated annual demand value for each item.

The contract maximum during the contract period* will be $31,954,143.60.

*Note: Contract period as defined in this clause means the extended contract shall be inclusive of any/all option periods.

The Government is obligated to order only the minimum quantity or dollar figure stated above.

SECTION L (Continued) (Attachment #4 to Original Solicitation)

INSTRUCTIONS TO OFFERORS

Entire Section replaced with:

Proposals will be evaluated on the basis of price and non-price evaluation factors in accordance with the evaluation criteria contained in Section M of this solicitation. Offerors should thoroughly read and

PAGE 5 OF 9 PAGESCONTINUATION SHEET REFERENCE NO. OF DOCUMENT BEING CONTINUED:

understand the terms and conditions contained in the solicitation. Failure to provide any information requested in the solicitation, may render the offeror’s proposal technically unacceptable and preclude it from any further consideration for contract award. Offerors should ensure that the information contained in their proposal is factual, accurate and complete. If the Government accepts the proposal, it will contractually bind the successful offeror to the terms and conditions of the solicitations.

In addition to price, the Government will evaluate non-price factors in accordance with the evaluation criteria contained in the solicitation. These factors are deemed essential in order to determine the offer that best meets the Government’s needs.

The Government is conducting a Lowest Price Technically Acceptable source selection in accordance with (IAW) FAR 15.101-2. All offers will be evaluated to determine technically acceptability. As these NSNs have been determined split award candidates, award decision will be made to the technically acceptable source(s) IAW the evaluation factors cited in Section M of this solicitation.

1. PRICE:

Offerors shall submit a pricing proposal for the item(s) identified in the request for proposal’s (RFP’s) supplies with the offeror’s insertions of its proposed prices for each contract line item on which the offeror is making a proposal.

2. PAST PERFORMANCE:

Performance Risk will be assessed based on delivery history and quality history obtained from the Past Performance Information Retrieval System Statistical Reporting (PPIRS-SR) for the applicable category of goods being procured using the Federal Supply Class (FSC) and/or the North American Industry Classification System (NAICS) code.

Past Performance Information Retrieval System – Statistical Reporting (PPIRS-SR):

PPIRS- SR provides past delivery and quality performance information. The sources of data include: the Department of Navy’s Product Data Reporting and Evaluation Program (PDREP), the Army's Logistics Modernization Program - Virtual Contracting Enterprise Reporting and Delinquency System (LMP-VCERADS), the Air Force’s JO18 (Delivery) and GO21 (Deficiency Reporting Information System), the Joint Deficiency Reporting System (JDRS) for joint Services aviation PQDRs, and DLA’s Supplier Relationship Management (SRM). PPIRS-SR classifications are based on the Federal Supply Class (FSC) or North American Industry Classification System (NAICS), are calculated monthly based upon 3 years of data, and are comprised of a Delivery Score and a Quality Color Ranking.

(a) Delivery Score The contractor’s delivery performance will be based on a weighted combination of the percentage of contract line items with on-time deliveries and the average days late for all deliveries.

(b) Quality Color Ranking Quality Color Ranking is based upon color/percentile groups. Contractor quality performance is based on a comparison among all contractors within an FSC. Contractor quality assessments are evaluated as follows:

Color Percent Group Dark Blue High Five Percent Purple Next 10 Percent Green Next 70 Percent Yellow Next 10 Percent Red Last Five Percent Dark Blue, Purple, and Green are evaluated approximately equal and more favorably than Yellow. Yellow is evaluated more favorably than Red

PAGE 6 OF 9 PAGESCONTINUATION SHEET REFERENCE NO. OF DOCUMENT BEING CONTINUED:

3. DELIVERY:

See Section F of the solicitation for time, place and method of delivery or performance.

SECTION M (continued) (Attachment #5 to Original Solicitation

EVALUATION FACTOR FOR AWARD

Entire Section replaced with:

General Basis for Award:

Single or Split Awards: More than one award may be made under this solicitation to maintain multiple viable manufacturing sources to enhance competition. The Government will award to the Lowest Priced Technically Acceptable (LPTA) or the Lowest Priced combination of technically acceptable offers in which both offers prices are determined fair and reasonable. All proposals will be evaluated to determine that the offered price(s) are fair and reasonable.

Evaluation Factors:

An offeror must meet or exceed the minimum requirements of the following factors to be eligible for award:

• Material Requirement Factor

• Delivery Requirement Factor

• Past Performance

1. PRICE:

A. The evaluated price for year one of the base period shall be arrived at by multiplying the Government’s estimated annual demand quantity (ADQ) by the offered unit price by the applicable portion of the split (i.e., if the total estimated ADQ for NSN is 100 units and Firm A offers on a 60% split, the estimated ADQ used to calculate Firm A’s evaluated price for the NSN will be 60 units).

B. The Government intends to make two awards to maintain multiple viable manufacturing sources to enhance competition based on the evaluation of the price combination of acceptable offerors totaling 100% of the requirement (i.e., either a 60%-40% split OR a 50%-50% split) that results in awards to two different offerors. The total price for the 60%-40% split combination will be determined by selecting the lowest priced offer for the 60% split and then from the remaining offers select the lowest price for the 40% split. The total price for the 50%-50% split combination will be determined by selecting the two lowest priced offers for that split. The 60/40 split combination and the 50/50 split combination will be compared and award will be made to the lowest priced combination of offers in which both offers are determined fair and reasonable.

C. Pricing for each split-award offer will be compared to the lowest price proposal for 100% of the requirement solely to evaluate whether it is reasonable for the Government to award split. If no combination of split-award proposals can be found fair and reasonably priced, the Government will award to the lowest priced proposal for 100% of the requirement.

D. Evaluation of Offers – Contractor First Article Testing: The cost to the Government to review the contractor’s first article test report shall be a factor in evaluation of the price. The Government’s review cost will be added to the offered price of the applicable item. Unless cited elsewhere in this solicitation, PAGE 7 OF 9 PAGESCONTINUATION SHEET REFERENCE NO. OF DOCUMENT BEING CONTINUED:

the costs are as follows: $200.00 for an internal review plus $950.00 for each engineering support activity (ESA) review. ESA reviews are required if the items being acquired are identified as critical application items or critical safety items.

E. The Government will evaluate offered prices on the basis of fairness and reasonableness.

F. The Government may determine that a proposal is unacceptable if the prices proposed on different splits (100%, 60%, 50%, or 40%) are materially unbalanced between one another. Unbalanced pricing exists when, despite an acceptable evaluated price at 100%, the price of one or more of the remaining splits is significantly overstated or understated as indicated by the application of cost or price analysis techniques.

An offer may not be considered for award of an NSN if the Contracting Officer determines that the offeror’s pricing for the NSN is unbalanced.

2. Subjective judgement on the part of the Contracting Officer during evaluations is implicit throughout this source selection. Each non-price factor will be assigned a rating of “Acceptable” or “Unacceptable” based on whether the proposal meets the basic minimum requirements delineated further. A rating of “Acceptable” shall be required for all non-price factors for a proposal to be considered for award.

“Acceptable” is defined as meeting the RFP requirements; “Unacceptable” is defined as not meeting the RFP requirements.

3. MATERIAL REQUIREMENT: The offeror must propose to the EXACT item required by the Government as indicated in the Procurement Item Description (PID). If the offeror is not proposing to provide the EXACT item requested by the Government; the offeror will not be eligible for award, unless the proposed item is added to the PID by amendment prior to the date set for final proposal revisions.

4. DELIVERY REQUIREMENT: The offeror must propose a delivery timeframe for the NSN that meets the Government’s delivery requirements as indicated in the Delivery Tab on the Section B spreadsheet. Any ramp up time requested, will not be counted against the company’s proposed delivery time frame. Failure to meet the Government’s delivery may preclude consideration of the proposal any further.

5. PAST PERFORMANCE: Past performance includes but is not limited to, the offeror’s record of conforming to specifications, to the standards of good workmanship, adherence to contract schedules, commitment to customer satisfaction, and cooperative behavior. The Government will use the Past Performance Information Retrieval System – Statistical Reporting (PPIRS-SR) for evaluation.

Past Performance Information Retrieval System – Statistical Reporting (PPIRS-SR):

The Government will use the offeror’s PPIRS-SR score/ranking to determine acceptability of Past Performance.

Evaluation of PPIRS-SR delivery assessments will be based upon a numerical scale ranging from 0 (low) to 100 (high). To be considered “Acceptable,” the offeror must have a delivery score of 70 or higher. Any delivery score below 70 will deem the offer technically unacceptable. If no delivery records are found in PPIRS-SR, no delivery score will be assigned to the offeror for that specific FSC. A color of green, purple or dark blue for Quality performance will be considered “Acceptable.” Quality ratings with colors yellow and red will deem the offer technically unacceptable.

In the case of an offeror without a record of relevant past performance or for whom information on past performance is not available in the PPIRS-SR, the offeror will be evaluated neither favorably nor unfavorably on past performance.

For further details concerning PPIRS-SR classification calculations and contractor data challenge procedures, see user guides under References at http://www.ppirs.gov/.
