SP3300-18-Q-5029

NOTICE TO OFFERORS

1. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 – Streamlined Procedures for Evaluation and Solicitation for Commercial Items in conjunction with the Federal Acquisition Regulations (FAR) Subpart 13.5 Simplified Procedures for Certain Commercial Items. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This is a Request for Quote (RFQ) and the RFQ number is SP3300-18-Q-5029.

2. This notice incorporates provisions and clauses in effect through Federal Acquisition Circular (FAC) 2005-97 effective 24 Jan 2018 and Defense Federal Acquisition Regulation Supplement (DFARS) DPN 20180418 edition and Defense Logistics Acquisition Directive (DLAD) Current to Revision 5 and PROCLTR 2018-06. 3. This Request for Quotation (RFQ) is being issued to establish a Firm-Fixed Price (FFP) contract for the purchase and incidental installation of 2 Vertical Lift Modules (VLM) Storage Systems for Bldg. 1846, Bay 2, Storage facility at DLA Distribution Corpus Christi TX. The requirement covers all necessary incidental services, labor, material, equipment and control components required to supply and install 2 VLMs. A 1 year firm-fixed price maintenance contract after final government acceptance of the items will be issued for each VLM. Any award resulting from this RFQ will be issued on a Standard Form (SF) 1449.

4. This RFQ is solicited on an unrestricted basis. The North American Industry Classification System (NAICS) code for this project is 33922 and the size standard is 500 employees.

5. ALL UNIT PRICES SHALL BE QUOTED AS FOB DESTINATION PRICING.

6. PERIOD OF PERFORMANCE: Delivery shall be 270 days after date of contract award.

7. QUOTE DUE DATE, TIME, LOCATION: Quotes are due on June 15, 2018, at 3:00 PM EST. QUOTES MUST

8. System for Award Management (SAM): A prospective awardee shall be registered in the System for Award Management (SAM) database prior to award, during performance and through final payment of any contract resulting from this solicitation. Offerors may obtain information on registration and annual confirmation requirements via the SAM website at http://www.sam.gov.

9. INVOICING AND PAYMENT: Invoicing and Payment will be made via Wide Area WorkFlow. See DFARS Clause 252.232-7006.

10. A technical package is required from all offerors and shall be provided with the quotes.

Please pay special attention to the Instructions to Offerors. The instructions require various items of documentation to submit with your quote. The evaluation criteria explains how offeror’s quote will be evaluated for acceptability/unacceptability.

11. SYSTEM FOR AWARD MANAGEMENT – A prospective awardee shall be registered in the System for

Award Management (SAM) database prior to award, during performance and through final payment of any contract resulting from this solicitation. Offerors may obtain information on registration and annual confirmation requests via the Internet at http://www.sam.gov.

13. MARK CONTRACT NUMBER ON ALL CORRESPONDENCE

Contractor must mark the contract or purchase order number on all paperwork and shipments. The order number must appear on the exterior of the shipment. Failure to follow these instructions will hold up payment to you and could result in the return of merchandise at your expense.

14. INSURANCE: Reference FAR Clause 52.228-5 Insurance-Work on a Government Installation located in this solicitation. The minimum amount/kind of insurance required is shown below:

Workers’ Compensation and Employer’s Liability - $100,000 (except in states with exclusive or monopolistic funds that do not permit workers’ compensation to be written by private carriers).

General Liability- $500,000 per occurrence. Automobile Liability – Property damage $20,000 per occurrence, Bodily injury $200,000 per person and $500,000 per occurrence.

When requested by the Contracting Officer, the Contractor shall provide a copy of all subcontractors’ proofs of required insurance no later than five (5) days before each subcontractor commences work on the Government installation.

15. Site Visit: Date – May 30, 2018, Time - 8:00 am to 12:00 pm EST, Location - DLA Distribution Corpus Christi, TX Building 1846. Please provide the following information to joseph.crowley@dla.mil by no later than May 18, 2018 - Company Name, Employee Name, Employee Date of Birth, Employee Driver’s License Number, and State of Driver’s License. Visitors will be issued a visitor identification badge that will grant them access to Building 1846.

Failure to submit this information in the time-frame requested above may result in delays upon your arrival at the installation.

(d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph (a) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, and does not contain the clause at 52.215-2, Audit and Records -- Negotiation.

(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor’s directly pertinent records involving transactions related to this contract.

(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.

(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.

Cross-Referencing: To the greatest extent possible, each volume shall be written on a stand-alone basis so that its contents may be evaluated with the minimum of cross-referencing to other volumes of the quote. Information required for quote evaluation which is not found in its designated volume will be assumed to have been omitted from the quote. Cross-referencing within a quote volume is permitted where its use would conserve space without impairing clarity.

Except for the reproduced sections of the solicitation document and chart labels, the text size shall be no less than 10 point font. Use at least 1-inch margins on the top and bottom and ¾ inch side margins. Pages shall be numbered sequentially by volume. Legible tables, charts, graphs and figures shall be used wherever practical to depict organizations, systems and layout, implementation schedules, plans, etc. These displays shall not exceed 11 x 17 inches in size.

Foldout pages shall fold entirely within the volume, and may only be used for large tables, charts graphs, diagrams and schematics.

Distribution: Offerors are advised that the quote is “For Official Use Only” and are considered “Source Selection Information” – See FAR 2.101 & 3.104 and must be marked as such.

Table of Contents: A table of contents for each volume is required.

VOLUME I – PAST PERFORMANCE

The Offeror shall provide information on its Past Performance Record. This information will be utilized to determine the relevancy and quality of the Offeror’s past performance record to the proposed effort as outlined in the Technical Specification and the level of confidence the Government has for successful performance based upon the Offeror’s actions under previously awarded, relevant contracts.

Offerors should provide a detailed explanation of the relevance of the contracts or efforts to the requirements as stated in Attachment 1 Statement of Work. If major subcontractor (defined as those providing twenty percent (20%) of total dollar threshold AND/OR twenty-five percent (25%) of total man-hour effort) experience is provided as part of the past performance references, it will be given weight relative to the percentage of effort being provided by that particular subcontractor in the offer submitted. The references will be evaluated in the aggregate in order to allow Offerors who may not have the entire scope, magnitude, and complexity of the requirement under one individual contract to still be considered relevant if experience with the full scope, magnitude, and complexity of the requirement can be demonstrated within the allotted number of references as described above.

If an Offeror or its subcontractor determines they have no relevant past performance data, they shall provide the following statement in lieu of the information request below: “(Insert Offeror or subcontractor’s name) has determined that they possess no relevant past performance information IAW the RFQ”.

To be compliant with Factor 1, Offerors shall provide the following information for both the Offeror and proposed major subcontractors. The Offeror shall submit the information in separate, tabbed sections (1), (2), (3), (4), and (5), as outlined below.

(1) An index of Previous Contract Data for the Offeror. The following information must be submitted on all contracts valued at over $500,000 that the offeror has performed within the past 5 years or is performing for facilities similar in scope and size to the one described in the Government specifications. Experience can be with Government or commercial customers. Provide relevant past performance information requested as listed below in chronological order.

- Contract Number and Date .

- Procurement Agency/Purchaser.

- Name and telephone number of the Procuring Contracting Officer (PCO) and the

Administrative Contracting Officer (ACO).

- Brief work description and discussion of the similarities and differences between the previous experiences and the requirements of this solicitation.

- Identify subcontractors or partners used to complete significant portions, (25% or more), of the project.

- Original contract price.

- If completed: Include price at completion also, and explanation for each price change effected during the overall contract performance. Explanation should briefly explain the circumstances that created the need for change, the amount of the change, who was responsible for the change (i.e. purchaser or contractor) and the basis for that determination, and any corrective actions taken and their impact.

- If ongoing: Current revised contract price and explanation for each change as outlined in (a.) above.

- Original contract completion date and period of performance in months.

- If completed: The period of performance at completion in months, and explanation for each increase in performance time effected during overall contract performance. The explanation should briefly explain the circumstances that created the need for the increase in time, the amount of time increased, who was responsible (i.e. purchaser or contractor) for increases and basis for determination, and any corrective actions taken and their impact.

- If ongoing: Whether contract is on schedule (i.e. is it in compliance with its completion schedule). If not, provide explanation for each delay and for any increase in performance time as outlined in (a) above.

- Provide a brief description of all quality deficiencies experienced, waivers or deviations requested to requirements, and warranty actions taken. Describe resolutions (for example, corrective actions taken or approval/disapproval of requests) effected.

- Type of contract (Firm Fixed Price (FFP), etc.).

- All unusual contractual actions (Termination for Default, Termination for

Convenience, Show Causes, Cure Notices, etc.). Briefly describe the circumstances of these actions and outcomes.

- Any other pertinent information concerning performance under the contract.

(2) A narrative synopsis that shall describe the specific nature of the work, similarities with the proposed work and the relevant experience gained. The narrative portion shall also indicate any occurrence of cost growth and/or schedule delays encountered. The appearance of growths and/or schedule delays on past contracts must be explained.

(3) An index of Previous Contract Data for all major subcontractors containing the information listed in (1) above as it relates to relevant PRIME and SUBCONTRACTS that the major subcontractor is currently performing or has completed during the last five (5) years.

(4) Attachment 4, Past Performance Questionnaire. Offerors and all major subcontractors shall send this questionnaire to previous customers of relevant prime and subcontracts. All questionnaires shall be sent by the RFQ due date and time identified above. All previous contracts the Offeror utilizes for Questionnaire information must be contained in the Previous Contract Data index for the Offeror/major subcontractor. The Offeror/major subcontractor shall complete Part I of Attachment 6 prior to submitting the questionnaire to the customer. The Offeror/major subcontractor shall ask the customer to complete the questionnaire and return it directly to the Acquisition Specialist identified in Part I – RETURN INFORMATION of the questionnaire. Evaluation members may contact the named individuals to clarify, obtain additional information, or conduct a telephonic survey of present/past performance. As such, the Offeror/subcontractor shall provide a list of all Questionnaires submitted that provides the following information:

– Name of the customer/agency;

– Date submitted to customer/agency;

– Contract/Purchase Order Number

– Customer points of contact (minimum of two, provide name, telephone number, fax number and e-mail addresses) and;

– Whether the offeror/major subcontractor was a Prime or Subcontractor.

(5) Offerors and their subcontractors may describe any quality awards or certifications that indicate technical excellence in the service requested. Examples of such awards or certifications include, the Malcolm Aldridge Quality Award, other Government quality awards, and private sector awards or certifications (e.g. the automobile industries QS 9000, Semitic’s SSQA, or ANS/EIA- 599). Indicate what segment of the company (or division or the entire company) received the award or certification and the date. If the award or certification is over three years old, present evidence the qualifications still apply.

The Government may utilize the Past Performance Information Retrieval System (PPIRS), the Contractor Performance Assessment Reporting System (CPARS), Federal Awardee Performance and Integrity Information System (FAPIIS), Electronic Subcontract Reporting System (eSRS), or other databases; interviews with Program Managers, Contracting Officers, and Fee Determining Officials;

responses received from the Performance Risk Assessment Questionnaire; and the Defense Contract Management Agency DCMA to determine the quality and relevance of the Offeror’s past performance.
