DEFENSE LOGISTICS AGENCY
AVIATION AT OGDEN
5851 f aVE, BUILDING 849 HILL Air Force Base, UTAH 84056
ATTACHMENT NO. 1
SUBJECT: Statement of Work for Shredder Service
1. General description of service: This specification establishes the minimum requirements for a recurring service to provide lockable containers, for paper and media, and monthly shredding of the material in those containers.
2. To be shredded: No classified data, only CUI, PII, LES or other sensitive but unclassified information will be shredded by the commercial shredding service.
3. Service to be performed:
The contractor will provide containers to be placed in DLA Aviation facilities on Hill AFB, Ogden, Utah. These containers will be emptied and shredded every four weeks, with an expected number of pickups to be 13 per year. Additionally, the contractor may be required to provide three out of cycle shredding’s per year as needed by the customer.
The contents within the containers will be shredded on site and destroyed in such a manner that would make the contents difficult to recognize or reconstruct the information. A DLA Aviation Security Representative will approve shredding methods and shredder output. The contractor will be liable for any information released due to improper or incomplete shredding as determined by the DLA Aviation Security Specialist. The Contractor or Sub-Contractor must Shred to “At Minimum” the National Association for Information Destruction certified 5/8-inch service. Personnel organizing the shred activity will monitor the complete operation to ensure that 100% of materials are shredded by the commercial shredder operator. Personnel must ensure the commercial shred operator will not read the contents inside the bins.
4. Inspection:
A thorough inspection by government personnel of the commercial vehicle may be conducted at the end of the shredding process before the vehicle is allowed to leave the onsite location. This inspection includes opening any compartment that allows access to the shred areas (except that of the loading doors) to ensure no un-shred material is found, i.e., the belt or hopper area. If un-shredded material is found, it must be re-shredded and re-inspected.
5. Shredding requirements:
The contractor will be responsible for bin installation, collection, shredding, and bin reallocation. Furthermore, contractor will be responsible for retrieving material from containers at all Aviation locations to include going up and down stairs if required. The shredding process will be scheduled (or rescheduled if necessary) by a DLA Aviation representative; the contractor must be able to meet the requested shred date within a period of 3 working days. All shredding will be accomplished on Hill AFB near the facilities where the containers are located. Containers will, at a minimum, be in buildings 5, 100, 225, 238, 265, 503, 507, 509, 843, 845, 847, 849, 1225. DLA Aviation will provide a map detailing locations of facilities. If bins are relocated, DLA Aviation will provide the new locations via a drawing or in person. The contractor will collect all bins, shred their material, and replace the bins within a maximum of 5 hours (or another reasonable time frame mutually agreed upon by both parties). The Contractor will be responsible for disposing of shredded material off Hill Air Force Base and not to sell the shred by-product to any company sending it to an overseas location. The entire shredding process will occur within DLA Aviation’s normal business hours; Monday through Friday 0700-1430, excluding government holidays. DLA Aviation shall provide escorts. The shredders will be able to shred paper bound with staples, paperclips, butterfly clips, folders and other items commonly associated with media. The shredders will be able to shred CDs and DVDs. DLA Aviation will not be responsible for any damage to the shredder during shredder operations.
6. Description and location of containers:
The contractor will provide approximately 14 each 64 gallon and 15 each 32 gallon (approximate size), to be placed in DLA Aviation facilities on Hill AFB. Each container will include a 1.5 to 2.5 inch by 12 inch slot in which personnel will deposit media along with a locking device. Each container will have wheels/casters and will be able to be relocated by one person. The containers will remain locked, DLA Aviation will be provided with a total of 26 master keys (each key capable of opening any container) in case a filling error occurs and media needs to be recovered. DLA Aviation may affix an 8.5 inch by 11 inch sleeve or sticker to each container. Contractor shall be responsible for cleaning up and removing all trash and discarded material. At locations where there is no elevator access and the bins would need to be moved down stairs, the Contractor shall substitute two approximately 32 gallons bins in lieu of the single 64-gallon bin. The purpose for this is so that the Contractor’s personnel can move the smaller bins while still holding onto a stair railing and reducing the chance of slips, trips, and falls. Contact the Project Manager to discuss alternative solutions as necessary.
7. Damaged Containers:
If any container becomes damaged or unusable due to normal wear and tear as determined by the Government, contractor is responsible for replacement of the containers. DLA Aviation will not be responsible for removal of the sleeve or sticker when the bin is no longer required.
In the event that damages occurred due to the Government’s negligence, the contractor will need to present to the Project Manager the damages for review. A final determination will be made by the contracting officer to see if the Government will be liable for the damage(s) and discuss appropriate action(s) to be taken.
In addition, the contractor shall be responsible for any damage to base facilities. Any damages requiring repairs will be at the contractor’s expense.
8. Special Requirements:
Contractor personnel, requiring access to Hill AFB, will be required to obtain contractor badges and controlled area badge. Personnel shall submit to a criminal background check and may be barred from entry to Hill AFB. The contractor-shredding vehicle is required to enter Hill AFB through the Commercial Gate (Roy Gate). Its driver is required to present the following information before being permitted on the base:
| * | Valid driver’s license |
| * | Proof of insurance |
| * | Vehicle registration and Tag Number |
There may be additional requirements for access to the Aircraft Maintenance Group (AMXG) Flight-line area and the Electronic Maintenance group (EMXG) facilities that is required. For AMXG, contractor drivers must complete Airfield Management Driving Training and be issued a driving competency card. DLA Aviation will provide this training. Airfield Management Driving Training and course tests are scheduled for four hours. For all locations and work being performed by the contractor, their personnel must be escorted at all times. The contractor will not be compensated for this training. At this time, this training is not required but when it is necessary, Government point of contact will notify the contractor.
