About this file

This document is a Single Source Justification for Simplified Acquisition Procedures (SAP) for the Department of Veterans Affairs, VISN 09 VA Medical Center in Memphis. The justification covers a one-month extension for offsite Ophthalmology surgery space and ancillary services from May 1-31, 2025, due to ongoing construction of the onsite surgery space. The procurement is with the Sole Source Affiliate (SSA), Memphis Surgery Center (MSC), which allows for continued patient care, lower costs, and residency training.

The document breaks down the estimated amount across multiple service lines: Cataract Bundled Rate, Glaucoma Bundles Rate, Oculoplastic Bundled Rate, and Unbundled Additional Supplies. Market research was limited, as this is a sole source requirement with a major affiliate. The justification notes that approximately 65% of eye surgery resident teaching for the Western Tennessee market is completed by the VAMC, and pricing is based on Medicare rates using an MSO CPT calculation template. The Contracting Officer has certified the purchase in accordance with FAR 13.106-1(b), approving the sole source procurement.

1. Contracting Activity:

Department of Veterans Affairs, VISN 09 VA Medical Center Memphis 1030 Jefferson Avenue Memphis, TN 38104-2127 2237:

2. Brief Description of Services required and the intended use/Estimated Amount:

The Memphis Veterans Affairs Medical Center (VAMC) has a requirement for Offsite Ophthalmology surgery space and ancillary services to continue providing care without a lapse in service. This is a request for a one-month extension that is Firm Fixed Price.

Total Estimated Amount: $ LIN 0001 Cataract Bundled Rate: $ cases) LIN 0002 Glaucoma Bundles Rate: $ ( cases) LIN 0003 Oculoplastic Bundled Rate: $ ( cases) LIN 0004 Unbundled Additional Supplies: $

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:

Due to the current onsite Ophthalmology surgery space being under construction, surgeries located at the Memphis VAMC must either be performed with the Sole Source Affiliate (SSA), Memphis Surgery Center (MSC), or be completed within the community. The Memphis VAMC Ophthalmology Department has a relationship with the SSA which allows for continued care, lower costs, and residency training. The long-term contract has been solicited, negotiated, and drafted.

Thus, a PO providing immediate, continued support is required.

Estimated POP: May 1-31, 2025 (one month)

4. Description of market research conducted and results or statement why it was not conducted:

This is an emergency sole source requirement to keep current Memphis VAMC services in place. Market Research for the per procedure price is based on the MSO CPT calculation template with current Medicare rates. This was used to determine the cost per procedure for the period of performance. Market Research for other contractors was limited as this is a sole source with a major affiliate. It is estimated that 65% of all eye surgery resident teaching is completed by the VAMC for the entire Western market of Tennessee.

Lastly, the BAA (FAR Part 25) does not apply to this acquisition as it is for services only.

5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
