About this file
This is a Data Reporting Requirements Exhibit (Exhibit E) for a Client Services Agreement between Pierce County, Washington and a contractor providing behavioral health services funded by Pierce County's Behavioral Health Tax. The exhibit establishes the framework for contractors to submit client service data on a monthly basis using secure mechanisms and standardized formats established by the County. Contractors must report three primary data files: a Participant File containing demographic and enrollment information for all clients served; a Service File documenting all service interactions provided to clients; and a Client Satisfaction File containing survey responses administered quarterly to clients. Data submission requirements include specific field definitions, formatting standards (dates in YYYY-MM-DD format), and data classification as Protected Health Information (PHI) subject to federal and state confidentiality regulations. The County will use the reported data for planning, quality assurance, quality assessment, and quality improvement purposes, including matching client information with the All Payer Claims Database maintained by OnPoint for the Washington State Health Care Authority to evaluate treatment program effectiveness. The exhibit remains in effect for the contract term or not less than one year after the contractor receives final payment.
Data access is restricted to authorized County employees in the Research and Data Analysis team and Human Services Department, with County Council employees receiving only partially identifiable data from OnPoint. The County must protect all confidential information through reasonable security measures, including access restrictions to authorized personnel only and physical security of documents and electronic media. The County is prohibited from sharing data with third parties, including subcontractors, without the contractor's prior written approval and formal amendment to the agreement. At contract termination or upon request, client-level data must be securely destroyed, including destruction of paper records and deletion of electronic records, though aggregate data reports may be retained by the County. The County will provide the contractor with up to ten business days to review any public reporting of aggregate data prior to publication. The contractor bears no liability for data accuracy, and amendments to this exhibit require written signatures from authorized representatives of all parties.
