Such code includes but is not limited to a computer virus, restrictive key, node lock, time-out or other function, whether implemented by electronic, mechanical, or other means, which limits or hinders the use or access to any computer software based on residency on a specific hardware configuration, frequency of duration of use, or other limiting criteria. If any such disabling code is present, the contractor agrees to indemnify the Government for all damages suffered as a result of a disabling caused by such code, and the contractor agrees to remove such code upon the Government’s request at no extra cost to the Government. Inability of the contractor to remove the disabling software code will be considered an inexcusable delay and a material breach of contract, and the Government may exercise its right to terminate for cause. In addition, the Government is permitted to remove the code as it deems appropriate and charge the Contractor for consideration for the time and effort expended in removing the code.

B.2 DESCRIPTION/SPECIFICATIONS/STATEMENT OF WORK
PERFORMANCE WORK STATEMENT (PWS)
DEPARTMENT OF VETERANS AFFAIRS
Veterans Health Administration (VHA) Infection Prevention and Control, Office of Quality, Safety and Value
Clinical Surveillance Software and Database Management System
Date: June 15, 2020
TAC-20-51539
PWS Version Number: 1.0
1.0 BACKGROUND
The mission of the Department of Veterans Affairs (VA), Veterans Health Administration (VHA) is to provide health care benefits and services to Veterans of the United States. In meeting this goal, VHA strives to provide high quality, effective, and efficient Medical Informatics systems to those responsible for providing care to Veterans in an effective, timely and compassionate manner. VHA depends on these systems to meet mission goals.
Federal requirements set forth by the VHA, VHA Directive 1031: Antimicrobial Stewardship Programs (ASP), and VHA Directive 1131: Management of Infectious Diseases and Infection Prevention and Control Programs (IP&C), were published to provide policy and guidance for emerging changes in the field of infectious diseases (IDs) infection prevention and control, antimicrobial stewardship, infectious diseases biosurveillance, and prevention programs. As a result, the ASP and IP&C programs at VA Palo Alto Health Care System (VAPAHCS) identified a critical need for the implementation and development of a comprehensive clinical surveillance system and data management system to reduce the incidence of hospital associated infections (HAIs), improve antimicrobial utilization, optimize clinical outcomes, improve patient safety, and minimize unintended or adverse outcomes, while reducing cost.
Current processes in place are manual, burdensome and redundant. ASP and IP&C are seeking solutions to streamline workflows, automate processes, create alerts, improve communication amongst providers and frontline staff, via filtering real time data, thus improving the time to perform intervention or action. The overarching goal is the ongoing quality improvement of processes, care, and outcomes for all patients receiving care at the VA Palo Alto Health Care System.

3.0 SCOPE OF WORK
The Contractor shall provide a Commercial Off-The-Shelf (COTS) solution that is capable of performing real time custom alerts, ongoing clinical surveillance/monitoring, extracts, report customization, and analytics services to support the following programs: Infection Prevention and Control (IPC), Pharmacy, Infectious Disease (ID), and Antimicrobial Stewardship Program (ASP) at the VAPAHCS. Each program requires user friendly customizable dashboards, as well as specific, detailed, customized reports and queries that can be modified and created ad hoc in a timely manner throughout the contract, incorporating Veterans Health Information Systems and Technology Architecture (VistA) and non-VistA domains. Data elements extracted will be used to create alerts for tracking specific events related to the occurrence of potential infections (specific organisms and critical laboratory results) and antimicrobial usage and trigger automatically when specific criteria established by the user are met. VistA domains required include, but are not limited to the following packages: microbiology lab and all laboratory data including, but not limited to, non-microbiology infection-related labs (e.g. lab-chem items like procalcitonin, Polymerase Chain Reaction (PCR)-based labs, serologic labs), Pharmacy and Bar Code Medication Administration (BCMA) data, Admission/Discharge/Transfer data (ADT) bed management, vital signs, procedures/imaging performed, Computerized Patient Record System (CPRS) text/notes, and Emergency Department and critical care data, outpatient laboratories/pharmacy/CPRS note data, and data from non-Palo Alto VA Palo Alto Health Care System (VAPAHCS) stations (e.g. community living/long-term care units at Menlo Park, Livermore VAs).
The COTS product shall interface with non-Vista domains including but not limited to: intensive care unit real-time database (PICIS Critical Care Suite), anesthesiology data in VistA/PICIS, Bed Management, and BCMA. This may also include the capability of scanning/accessing documents/data (e.g. VistA images, linked Portable Document Files (PDFs) from CPRS), such as those scanned in by the microbiology laboratory to record outside microbiology test results
The Contractor shall provide a solution that will use multiple VA data sources including and not limited to PICIS, VistA, CPRS, Corporate Data Warehouse (CDW), Healthcare-Associated Infections and Influenza Surveillance System (HAIISS) Data Warehouse (HDW) and other relational databases, as well as the ability to report to other agencies, such as Centers for Disease Control (CDC), National Healthcare Safety Network (NHSN), and Veterans Integrated Service Network (VISN) 21. This support solution shall allow multiple user groups, including clinicians and clinical and business analysts, the ability to create reports, analyze data, and develop effective patient care treatment plans and facilitate ongoing research and quality improvement efforts.
The Contractor shall also provide analytics/reporting support services, training, and help desk support services to identified VAPAHCS stakeholders.
The solution shall have the flexibility/scalability to interface with systems not yet established (e.g., Cerner) to provide real time data and predictive analytics to enhance clinical decision making. The system/products shall allow multiple users to simultaneously access and utilize multiple data resources to filter real-time data to create alerts, determine appropriate treatment or care algorithms, conduct queries, and pull a variety of information related to the following: on-going clinical surveillance and monitoring of healthcare associated infections, influenza, emerging infectious diseases and antibiotic resistance trends, daily alerts, trending, analysis, documenting, and reporting to support mission critical activities within the IPC, ID, Pharmacy, ASP and Quality and Safety programs (QS) at VAPAHCS.
