The Contractor is to submit a cleaning schedule for approval to the OOD Designated Representative.

If necessary, damage should be brought to the attention of OOD Designated Representative immediately and documented in writing on work orders or job evaluation forms.

Normally this means liquid disposal into a sanitary sewer system especially when hazardous materials (HAZMAT) may be involved, at a licensed disposal facility.

g. The Contractor or employees should never rinse or dispose of wastewater from equipment in sinks that are used for food preparation or other sanitary purposes.

h. If waste disposal is not possible on location, the waste should be returned to the company facility for proper disposal.

i. Although commercial loop-pile carpet does not need grooming, any cut-pile carpet will require resetting of the pile using a carpet rake, broom, or mechanical brushing device immediately after cleaning or as part of the final cleaning step.

j. Upholstery Cleaning: If furniture and/or cubicle walls are requested to be cleaned, Contractor shall extend the above preconditioning services to furniture and/or cubicle walls.

k. Pre-treatment for upholstery stains required prior to actual cleaning of furniture.

l. The preferred method of upholstery cleaning is low moisture cleaning. However, if low moisture is not effective at removing a stain, then hot water extraction will be an acceptable alternative.

6. Floor Cleaning: The flooring is Vinyl Composite Tile (VCT). Cleanings must maintain a high appearance level on the most highly trafficked areas. The Contractor must coordinate the annual floor cleaning with the OOD designated representative.

a. Floor must be swept, or dust mopped prior to cleaning.

b. When the VCT has become dull, the wax will need to be stripped prior to new coatings of wax.

IV. ADDITIONAL FISCAL AND/OR BUDGET REQUIREMENTS:

A. Contractor will acknowledge the costs and timelines for janitorial services.

B. The Contractor is responsible for any travel-related expenses incurred during the execution of the services and requirements listed.

C. The Contractor will invoice OOD for janitorial services monthly unless mutually agreed upon otherwise.

D. Floor cleaning services (waxing and/or carpet cleaning) shall be invoiced separately from janitorial services, issued after services are rendered, and invoiced separately for each service.

E. Contractor is required to repay any funds expended under this Agreement in the event that OOD determines such costs to be unallowable.

V. MONITORING, EVALUATION AND REPORTING:

A. *The OOD Designated Representative may inspect the performance of the Contractor’s work each working day.

The Contractor shall have an on-site inspection/check-in by one of their supervisors a minimum of one (1) time per week for the purpose of quality control and shall furnish the OOD Designated Representative with copies of inspection reports and follow-up for deficiency corrections. * Documentation of all completed monthly, quarterly, semi-annual, and annual tasks will be provided via e-mail to the OOD Designated Representative within two (2) business days of their completion.

B. Contractor’s employees shall sign in and out with arrival and departure times as directed. Sign in/out sheet will be provided by OOD and retained by the OOD Designated Representative.

Update scope of work effective 07/01/2025.

C. *The Contractor is to inform the OOD Designated Representative via telephone or email of any resignation/termination of an employee. In the event of a workday infraction that demands an immediate termination, the Contractor must confirm either in person or telephone that the terminated employee has surrendered their PIV card (Personal Identification Verification) and OOD Identification Badge and been escorted from the premises. PIV cards and OOD Identification Badges for all ex-employees must be returned to the OOD Designated Representative.

VI. ADDITIONAL TERMS AND CONDITIONS:

It is important to note that the Social Security Administration (SSA) will pay the cost of this Agreement. As such, SSA must approve all employees and Contractor of the awarded Agreement by means of background checks.
