This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.

(ii) This is Request for Quote (RFQ) 70RDAD21Q00000066.

(iii) Simplified acquisition procedures under FAR 13 apply to this solicitation. This solicitation includes all applicable provisions and clauses in effect through FAC 2021-05 / 3-10-2021 simplified procedures for commercial items.

(iv) This is a Small Business Set-Aside. NAICS Code: 423450 – Medical, Dental, and Hospital Equipment and Supplies Merchant Wholesalers.

Only one award will be made as a result of this solicitation. This will be awarded as a firm fixed price type contract.

I. Introduction/Background

Sudden cardiac arrest (SCA) can strike anybody, anytime, anyplace and often without warning. It is believed that increasing age and existing coronary problems lead to an increased risk. Unlike a heart attack in which blood flow to the heart muscle is temporarily blocked, the primary cause of SCA is ventricular fibrillation. This is a life-threatening condition in which the heart’s normal electrical signals become disorganized and erratic, causing the heart to cease pumping blood effectively.

Defibrillation or restoring the heart’s natural rhythm by applying an electrical shock is the only definitive treatment for SCA.

According to the American Red Cross, an AED is a device that analyzes the heart’s rhythm and, if necessary, may help the heart to reestablish an effective rhythm of its own. An AED is about the size of a laptop computer, is portable, fairly easy to use, and can vary in cost considerably based on features. The accessibility of AEDs is important when Emergency Medical Services (EMS) arrival times may be delayed due to traffic congestion or remoteness of location. In order for AEDs to assist in saving lives, a comprehensive AED use program must be in place to ensure trained and qualified AED operators are on site to use the cardiopulmonary resuscitation (CPR) and advanced life support techniques. Immediate contact of EMS by calling 9-1-1 or the local emergency services number must also occur when an AED is used. A preventive maintenance program, as well as medical oversight, must also be components of the AED use policy.

II. Objectives

To meet appropriate portions of the Federal Cardiac Arrest Survival Act and the requirements pertaining to AED deployment and use under District of Columbia AED Bill No. B13-0735.

III. Scope of Work

Provide the following AED services:

• Oversight. Conduct annual premium program management to include medical oversight subscription service for 251 (plus the 28 newly installed units, as they are installed) AED units that include:

- Maintaining the established AED program protocol document;

- Providing full medical leadership and oversight, guidance, mock drills, SCA event reports and reviews, consultation, and annual prescriptions (policy review). Mock drills can be included in the CPR/FA/AED training or conduct an exercise with trained CPR attendants annually.

- Providing updates to the protocol as needed by the Agency (including staff changes, site relocation, etc.);

- Ensuring the AED Program and each of the AEDs with the local Board of Health as required;

and

- Conducting ongoing management of the AED program; i.e., unit updates, updates to algorithms for CPR based on industry guidelines and other handbook materials and assist Agency and manufacturers with notification of recalls.

- Conduct a physical inspection of each unit at least once annually, as required by the manufacturer, and replace all durable goods (such as batteries and electrode pads) as they reach their expiration date. Also, as units are used, replace all items necessary to bring the unit back to operational condition with new pads, etc.

• Training. Conduct 12 Adult/Child CPR/AED/FA Combo classes including stairwell evacuation chair training (initial and/or recertification) classes of 8 to 12 students per class. Training shall be held on a monthly, quarterly, or continuous basis to certify responders’ skills are efficient to comply with the AED program requirements. The goal is to have four responders per AED unit. The Agency has the option to choose courses that range from CPR/AED/FA, CPR/AED/FA/BBP, CPR/AED/BBP, and CPR/AED.

The Oversight and Training portions are to be completed in the Base year and Option Periods.

The following applies to the Base Period only. Contractor shall procure and install the following AED products and supplies for the Regional Office Building that is located at 301 7th Street, Southwest, Washington, DC 20024:

• Twenty-eight (28) Physio-Control LifePak 1000 Automated External Defibrillators

(Brand name or equal) that include:

• Two Sets of Adult Electrode Pads;

• Battery and Spare Battery Packs (4-Year Lithium);

• Semi-Rigid Carrying Case;

• Quick Reference Card;

• User’s Manual and Quick-Start Guide;

• Rescuer CPR/AED Pack;

• Three-Dimensional AED Wall Sign;

• AED Check Tag;

• AED and CPR Guidelines Reference Poster; and

• Metal Surface Mount Cabinet with Audible Alarm and Strobe.

If an equal AED is provided as part of the quote, the equal must be compatible with the units that the Washington DC Fire and Emergency Services use. The Physio-Control LifePak 1000 Automated External Defibrillators is compatible.

IV. Additional Information

Provide products and services as described above. Install the AED wall cabinets, signage and AEDs at the St. Elizabeth’s Campus (2703 Martin Luther King, Jr. Avenue, Southeast, Washington, DC 20593) in accordance with the American with Disability Act requirements.

V. Period of Performance

Base Period 12-months from date of award with four 12-month Option Periods.

(d) Comptroller General Examination of Record. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. FAR 52.212-8 Option to Extend Services (Nov 1999)

The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. FAR 52.212-9 Option to Extend the Term of the Contract (Mar 2000)

(a) The Government may extend the term of this contract by written notice to the Contractor within 60 days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 15 days before the contract expires. The preliminary notice does not commit the Government to an extension.

(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.

(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 5 years.

VIII. Basis for Award

The provision at 52.212-2, Evaluation – Commercial Items (Oct 2014) applies to this acquisition.

Award will be made to the responsible quoter submitting a quote that is the most advantageous to the Government, price and non-price factors considered. Award will be made to the firm whose quote meets the Government's requirements and whose technical quote and price represents the Best Value – Tradeoff to the Government. The evaluation of quotes will be based on an evaluation of the vendor’s (1) Past performance; and (2) Price. Award will be made to the Quoter offering the Best Value to the Government by conducting a tradeoff of the factors.

Factors:

(1) Past Performance

The vendor has to provide 2 previous orders completed in the last three years in which similar services were provided to the satisfaction of the ordering organization. At a minimum, the vendor needs to provide: Date of award, contract number, and point of contact information.

The Quoter will be evaluated to determine the level of confidence that the past performance demonstrates the likelihood of successful performance in providing requirements similar in size, scope, and complexity of this solicitation. The evaluation will consider both the relevance and quality of past projects. The Government may choose to evaluate publicly available reports, and/or data from the Past Performance Information Retrieval System (PPIRS). The Government may use present and/or past performance data obtained from a variety of sources, not just those contracts identified by the Quoter.

(2) Price

The vendor shall provide the price to complete what is required under this RFQ.

The Government will not make an award at a significantly higher price to achieve only slightly superior (Technical and Past Performance) capabilities.

Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the options(s). The Government will evaluate FAR clause 52.217-8 Option to Extend Services (NOV 1999).

IX. Instructions on Submission of Quote (Amended sections of FAR 52.212-1, Instruction to

Offerors- Commercial Items)

The Government intends to evaluate quotations without discussions with the Quoter. Therefore, the Quoter’s initial quote should contain the Quoter’s best terms from a price and technical standpoint.

However, the Government reserves the right to conduct discussions if later determined by the Ordering Contracting Officer to be necessary. The Government may reject any or all quotes if such action is in the public interest.

Refer to RFQ 70RDAD21Q00000129 on all correspondence. Quotations shall be emailed to Pavi Tumbapura, Contract Specialist at Pavithra.Tumbapura@hq.dhs.gov and to Ronald Jean-Baptiste, Contracting Officer at Ronald.Jean-Baptiste@hq.dhs.gov and shall be received by 10:00 AM ET on June 16, 2021. Questions regarding this solicitation shall be received by 10:00 AM ET on June 11, 2021 and shall be submitted in writing to the emails above. The CO reserves the right to accept late submissions, before making an award, if it is in the best interest of the Government. In order to receive an award, contractors must be registered in the System for Award Management through sam.gov.

As a minimum, the quote must show:

1. The solicitation number;

2. The name, address, telephone number, and email address of the quoter;

3. Price and any discount terms of the items and/or services;

4. Acknowledgement of solicitation amendments, if any;

5. Terms of any express warranty;

6. If the terms and conditions of this solicitation are not fully accepted, a statement specifying the extent of agreement shall be included. Quoters that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration.

The quote shall be valid at a minimum 30 days from the specified due date of the quotes unless the quote clearly identifies a lesser period; however, the lesser period of validity of the quote shall not be for less than 15 days.
