1.0 INTRODUCTION / BACKGROUND

The Wewoka Indian Health Center was originally constructed in 1978. This project will consist of the clinic building.

2.0 OBJECTIVE

To remove the current entry doors for the North entrance of the clinic building with automatic sliding doors and to add automatic glass doors throughout the clinical area.

3.0 SCOPE OF WORK

Schedule of work shall be as follows

A. Demo both doors that enter the vestibule on the clinic building north side B. Install sliding doors that match the south entry sliding doors with same brand and model

(Proslide 2003 belt drive system type SO-SX-SX-SO) or equal.

C. Install automatic break-away sliding glass doors on the north entry of the building, the north exterior set of doors needs to have a HSPD-12 PIV reader to unlock the door for entry. (The reader is already on existing door) shall have thresholds, (color of threshold shall match metal frame).

D. Provide an air-curtain inside of vestibule to the exterior sliding door to come on when the exterior door opens.

E. The air-curtain shall have heat capabilities for the winter time and have automatic on/off switch activation for when the door opens and closes.

F. Install two (2) automatic on/off switches for the south clinic sliding doors air curtain, and the Administration building sliding entry door air curtain that the air curtains are already installed.

G. The north entry vestibule shall have the tile removed and install new ceramic tile with grout to match south vestibule. Wewoka Indian Health Center will provide tile but not the grout.

H. Demo clinical entry door 414 corridor and install new metal frame with glass 4-0 (48” door).

Install door frame that the new door does not intrude into the 413 Corridor. This door shall have an automatic opener accessed with a PIV reader on the 413 corridor side with a touchless hand button for emergency egress access. (badge reader is already installed at this door)

I. Demo 502 corridor entry door and install a metal frame glass door (48”) in this area with an automatic opener and access will be controlled with a PIV reader and emergency egress to be controlled with a touchless hand motion switch( the touchless switch is not installed and will need to be purchased). This door may be moved back to allow the door to not intrude into the corridor, (bi-folding doors will not be an acceptable alternative)

J. Demo 517 door and install metal frame glass (48”) door with automatic opener, PIV badge reader ( is already installed at this door reuse) the reader will control access and have a touchless hand motion for emergency egress access ( the touchless switch is not installed and will need to be purchased), (bi-folding doors will not be an acceptable alternative)

K. Demo 516 door and install automatic sliding door, shall be a single door slider with a PIV badge reader installed (need to install a PIV reader that meets HSPD-12 HID reader along with programming into the symmetry system, there is one open slot on the reader board to add the door), along with a touchless hand motion (the touchless switch is not installed and will need to be purchased emergency egress release for the door as to keep the door from opening in a secure area. The slider door shall be Proslide 2003 belt drive system type single door or equal.

L. Demo door 408 and install a glass door (48”) metal frame with automatic opener, PIV (the reader is already installed at the door reuse the reader) badge reader shall control access and have a touchless hand motion for emergency egress access.

M. All doors shall be ADA compliant with logos on the all automatic doors.

3.1 Electrical:

A. There are electrical receptacles mounted above ceiling to supply power to each door.

B. There is a Junction Box mounted above north facility entry to supply power for the heated air curtain, (60 amp service) need to power the air curtain from junction box.

Equipment Installation (brand name or equal):

• Proslide 2003 belt drive system Type SO-SX-SX-SO four (4) sets

• Mars Air door low profile air curtain, with heat, 208 Voltage, 60 amp One (1) for the North Clinic door

• Dormakaba touchless switch for activation of automatic switch. (These switch needs to be mounted in a box and recessed into the wall were permitting, and surface mounted on block walls).

4.0 TASKS / REQUIREMENTS

Contractor shall be responsible for all aspects of preparing area of work for renovation, including demolition and removal of existing construction, and furnishing all transportation, labor, materials and incidentals to perform work as required:

4.1. General and Administrative

A. Contractor shall provide a full-time superintendent to be on site while supervising and scheduling work that is being performed at the Health Center by the general contractor or their subcontractors.

B. Contractor shall provide their own Personal Protective Equipment (PPE), Lockout Tag-out equipment, infection control barriers, vehicles barriers, and any other safety related supplies, materials, and equipment to perform this job.

C. Contractor shall provide a dumpster for the removal of debris, material, and waste for the duration of the project and dispose of according to State, Federal, and EPA regulations.

At least 50% of recyclable materials shall be recycled. Location of the dumpster shall be coordinated with the Facility Manager.

D. Contractor shall clean work areas daily. Keep work area, and public access areas free from accumulations of waste materials. Contractor shall remove trash daily.

Construction waste shall be transported in tightly covered containers to minimize spreading of dirt and debris associated with construction.

E. Contractor shall isolate HVAC system in area where work is being done to prevent contamination of duct system.

F. Contractor shall maintain a negative air pressure within the work site utilizing HEPA equipped air filtration units.

G. Contractor shall seal holes, pipes, conduits, and punctures.

H. Upon completion of the work, Contractor shall remove barrier material carefully to minimize the spreading of dirt and debris associated with construction.

I. Upon completion of work, Contractor shall vacuum work area with HEPA filtered vacuums, and wet mop area with cleaner/disinfectant. Contractor shall conduct a final clean and leave the work area in a clean, neat, and orderly condition.

J. Upon completion of work, Contractor shall restore HVAC system where work was performed.

4.1.1. Temporary Barrier

A. Contractor shall construct a temporary (<72 hours) fire-resistant plastic sheeting on the inside entry and outside access door for a sealed infection control barrier.

1) Barriers shall be installed with zipper doors, and ante room with sticky mats per barrier as required in order to meet Health Center infection control requirements for this project.

2) The plastic barrier can be removed once the work area is clean and free of debris.

4.2 CLEAN and REMOVAL OF BARRIERS

4.2.1 Contractor Final Clean

A. Contractor shall clean work area within the newly installed flooring and barrier wall spaces of work.

B. Contractor shall clean area within the secured construction route and exterior entry area.

C. Contractor shall conduct a final clean and leave the work area in a clean, neat, and orderly professional condition.

4.2.2 Demo Barrier

A. Contractor shall coordinate with the Facility Manager/COR for removal of barriers.

B. Contractor shall remove barrier material carefully to minimize the spreading of dirt and debris associated with construction.

C. Clean floor, walls and ceiling where hard barrier was located.

4.2.3 Final Clinic Clean

A. Coordinate with the Facility Manager/COR for guidance on final terminal clean.

5.0 DELIVERABLES

5.2 Submittals

Submittals may be shop drawings, product data, cut-sheets of equipment, material samples, sketches, manufacturer’s literature, catalog descriptions, or other description sufficient in detail for review and action to allow a decision by COR. COR shall respond within 7 business days.

Contractor shall maintain a submittal log with the date submitted for review, the date received back, and action taken. A copy of the submittal log will be provided at the monthly progress meeting for review.

Substitutions or deviations not approved by the COR risk rejection. Submittals and Safety Data Sheets (SDS) for chemicals and materials shall not be used until approved.

5.3 RFI

If the Contractor desires an explanation or interpretation of the contract or drawings, etc., the request shall be submitted in writing as a Request for Information (RFI) to the Facility Manager and CO via electronic format.

Contractor shall maintain a log, numbered sequentially, of all RFIs submitted, the date submitted, and the date an answer was provided. A copy of the RFI log will be provided at the monthly progress meeting for review.

5.4 Warranty

Warranty shall continue for a period of 1-year from the date of final acceptance of the work.

Special Manufacturers warranties shall be provided upon completion.

5.5 Transmittals

The contractor shall provide 2 hard copies of each deliverable, and one electronic version.

6.0 PROGRESS / COMPLIANCE / INSPECTION

The Government requires the following from contractors in order to monitor progress and ensure compliance:

6.1 Progress Meetings

COR will coordinate with Contractor in scheduling and administer the construction progress meetings throughout the Construction Phase.

Meetings shall be held on site, and will Weekly meetings shall be held with the COR on site to discuss weekly status, accomplishments, and planned activities for the week, and issues that need coordination.

Meetings will also cover topics pertinent to progress and completion of the project. COR shall draft reports of meeting and send to Contractor, CO, and other pertinent staff.

6.2 Inspections

Government shall inspect regarding quality assurance and remedies regarding non-conforming work.

6.3 Contractor – Signing In/Out

Contractor shall coordinate with the Health Center Facility Manager on the signing in/out and scheduling of Contractor’s staff and sub-contractors in order to maintain efficient use of traffic-flow, parking, and access to building and locked areas.

6.4 Life Safety and Infection Control

Before any construction or services are to be performed within the Wewoka Indian Health Center Clinic building the Contractor is 100% responsible for implementing and maintaining all Safety and Infection Control Measures during performance of their contract. This project requires that the entire construction area remain under negative pressure for infection control based on an ICRA (Infection Control Risk Assessment) performed by the Initial and periodic inspection shall be conducted by the Facility Manager, Safety Officer, and/or Infection Control Officer. Any life safety or infection control deficiencies will have to be immediately corrected by Contractor.

Contractor is responsible for implementing all safety and infection control measures to ensure these measures meet OSHA, NFPA Codes, ASHRE, and Joint Commission standards during the course of this project.

6.5 Video – Clinic Policy

Contractor shall review procedures with respect to Life Safety and Infection Control measures of the Health Center prior to beginning job. A film video will be made available to the Contractor to view prior to starting inspection to comply with infection control procedures during Health Center construction.

A. Video on “Infection Control during Construction”.

B. OSHA standard 1926.28 Personal Protective Equipment (PPE).

C. Interim Life Safety Hospital Measure (ILSM) Policy-Part A, Chapter 10, Section 4.

D. Guideline for environmental infection control in Health Care Facility 2003.

6.6 Risk Assessment – Health Center Policy

Contractor shall read and agree to all of the health safety regulations needed for this job.

A. Control risk assessment per type of project of activity per matrix of precaution for construction and renovation - Type 1-5. Facility Manager will provide a copy of policy “Infection Control during Construction and Renovation, Part H, Chapter 4, Section 7.0.

7 REFERENCES

7.1 Existing As-Builts

As-built prints for plumbing, sewer, HVAC, fire sprinkler system, fire alarm system, building construction, etc. are available for review. Coordinate with Health Center Facility Manager.

7.2 Quality of Workmanship

Contractor shall perform high quality trade and professional craft workmanship. All work must conform to, and comply with, the latest edition and version of ALL applicable codes including, but not limited to, OSHA, NFPA, IBC, FGI Guidelines for Design and Construction of Health Care Facilities 2014 edition, and Joint Commission Codes, Standards and Guidelines for Hospitals (CAMH).

8 SECURITY REQUIREMENTS

Contractor shall abide by all Safety, Security and Infection Control Policies and Procedures of the Wewoka Indian Health Center.

Contractor shall comply with security requirements including personal identification.

Contractor shall be responsible for providing each of their employee with an ID badge. ID badge shall be worn at all times when performing work during the course of this project.

9 LOCATION

The project location:

Wewoka Indian Health Center, 36640 Hwy 270 Wewoka, Oklahoma 74884

A. Contractor Work Hours:

This work shall be conducted after normal business hours from Monday thru Friday 5:30pm to Midnight, Saturday and Sunday 7:00 am to 10:00 pm.

Coordination and scheduling of work shall be coordinated with the Facility Manager on a daily basis.

B. Tobacco Free Campus:

The Wewoka Indian Clinic is a tobacco free campus. No smoking is permitted on the campus grounds.

10 PERIOD OF PERFORMANCE

The period of performance this scope of work is 90-calendar days after notice to proceed (NTP). Contractor shall utilize performance period for completion of submittals, approvals, color selections and acquisition of materials and supplies. On-site construction shall be limited to ninety (90) calendar days of on-site construction.

52.212-3 Offeror Representations and Certifications-Commercial Items. (Feb 2021)

The Offeror shall complete only paragraph (b) of this provision if the Offeror has completed the annual representations and certification electronically in the System for Award Management (SAM) accessed through https://www.sam.gov. If the Offeror has not completed the annual representations and certifications electronically, the Offeror shall complete only paragraphs (c) through (v)) of this provision.

(a) Definitions. As used in this provision—

"Covered telecommunications equipment or services" has the meaning provided in the clause 52.204-25, Prohibition on Contracting for Certain Telecommunications and Video Surveillance Services or Equipment.

Economically disadvantaged women-owned small business (EDWOSB) concern means a small business concern that is at least 51 percent directly and unconditionally owned by, and the management and daily business operations of which are controlled by, one or more women who are citizens of the United States and who are economically disadvantaged in accordance with 13 CFR part 127. It automatically qualifies as a women-owned small business eligible under the WOSB Program.

Forced or indentured child labor means all work or service—

(1) Exacted from any person under the age of 18 under the menace of any penalty for its nonperformance and for which the worker does not offer himself voluntarily; or

(2) Performed by any person under the age of 18 pursuant to a contract the enforcement of which can be accomplished by process or penalties.

Highest-level owner means the entity that owns or controls an immediate owner of the offeror, or that owns or controls one or more entities that control an immediate owner of the offeror. No entity owns or exercises control of the highest level owner.

Immediate owner means an entity, other than the offeror, that has direct control of the offeror. Indicators of control include, but are not limited to, one or more of the following: ownership or interlocking management, identity of interests among family members, shared facilities and equipment, and the common use of employees.

Inverted domestic corporation, means a foreign incorporated entity that meets the definition of an inverted domestic corporation under 6 U.S.C. 395(b), applied in accordance with the rules and definitions of 6 U.S.C. 395(c).

Manufactured end product means any end product in product and service codes (PSCs) 1000-9999, except—

(1) PSC 5510, Lumber and Related Basic Wood Materials;

(2) Product or Service Group (PSG) 87, Agricultural Supplies;

(3) PSG 88, Live Animals;

(4) PSG 89, Subsistence;

(5) PSC 9410, Crude Grades of Plant Materials;

(6) PSC 9430, Miscellaneous Crude Animal Products, Inedible;

(7) PSC 9440, Miscellaneous Crude Agricultural and Forestry Products;

(8) PSC 9610, Ores;

(9) PSC 9620, Minerals, Natural and Synthetic; and

(10) PSC 9630, Additive Metal Materials.

Place of manufacture means the place where an end product is assembled out of components, or otherwise made or processed from raw materials into the finished product that is to be provided to the Government. If a product is disassembled and reassembled, the place of reassembly is not the place of manufacture.

Predecessor means an entity that is replaced by a successor and includes any predecessors of the predecessor.

Reasonable inquiry has the meaning provided in the clause 52.204-25, Prohibition on Contracting for Certain Telecommunications and Video Surveillance Services or Equipment.

Restricted business operations means business operations in Sudan that include power production activities, mineral extraction activities, oil-related activities, or the production of military equipment, as those terms are defined in the Sudan Accountability and Divestment Act of 2007 (Pub. L. 110-174). Restricted business operations do not include business operations that the person (as that term is defined in Section 2 of the Sudan Accountability and Divestment Act of 2007) conducting the business can demonstrate—

(1) Are conducted under contract directly and exclusively with the regional government of southern Sudan;

(2) Are conducted pursuant to specific authorization from the Office of Foreign Assets Control in the Department of the Treasury, or are expressly exempted under Federal law from the requirement to be conducted under such authorization;

(3) Consist of providing goods or services to marginalized populations of Sudan;

(4) Consist of providing goods or services to an internationally recognized peacekeeping force or humanitarian organization;

(5) Consist of providing goods or services that are used only to promote health or education; or

(6) Have been voluntarily suspended. "Sensitive technology"—

Sensitive technology—

(1) Means hardware, software, telecommunications equipment, or any other technology that is to be used specifically—

(i) To restrict the free flow of unbiased information in Iran; or

(ii) To disrupt, monitor, or otherwise restrict speech of the people of Iran; and

(2) Does not include information or informational materials the export of which the President does not have the authority to regulate or prohibit pursuant to section 203(b)(3)of the International Emergency Economic Powers Act (50 U.S.C. 1702(b)(3)).

Service-disabled veteran-owned small business concern—

(1) Means a small business concern—

(i) Not less than 51 percent of which is owned by one or more service-disabled veterans or, in the case of any publicly owned business, not less than 51 percent of the stock of which is owned by one or more service-disabled veterans; and

(ii) The management and daily business operations of which are controlled by one or more service-disabled veteransor, in the case of a service-disabled veteran with permanent and severe disability, the spouse or permanent caregiver of such veteran.

(2) Service-disabled veteran means a veteran, as defined in 38 U.S.C. 101(2), with a disability that is service connected, as defined in 38 U.S.C. 101(16).

Small business concern—

(1) Means a concern, including its affiliates, that is independently owned and operated, not dominant in the field of operation in which it is bidding on Government contracts, and qualified as a small business under the criteria in 13 CFR part 121 and size standards in this solicitation.

(2) Affiliates, as used in this definition, means business concerns, one of whom directly or indirectly controls or has the power to control the others, or a third party or parties control or have the power to control the others.

(i) One or more socially disadvantaged (as defined at13 CFR 124.103) and economically disadvantaged (as defined at 13 CFR 124.104) individuals who are citizens of the United States; and

(ii) Each individual claiming economic disadvantage has a net worth not exceeding $750,000 after taking into account the applicable exclusions set forth at 13 CFR124.104(c)(2); and

Subsidiary means an entity in which more than 50 percent of the entity is owned—

(1) Directly by a parent corporation; or

(2) Through another subsidiary of a parent corporation

Successor means an entity that has replaced a predecessor by acquiring the assets and carrying out the affairs of the predecessor under a new name (often through acquisition or merger). The term "successor" does not include new offices/divisions of the same company or a company that only changes its name. The extent of the responsibility of the successor for the liabilities of the predecessor may vary, depending on State law and specific circumstances.

Veteran-owned small business concern means a small business concern—

(1) Not less than 51 percent of which is owned by one or more veterans (as defined at 38 U.S.C. 101(2)) or, in the case of any publicly owned business, not less than 51 percent of the stock of which is owned by one or more veterans; and
