SCOPE OF CONTRACT
This contract requires water testing services and related work in compliance with its terms, specifications, and provisions. This includes furnishing labor, materials, transportation, equipment, supervision, operating supplies, office support, and incidentals. Contractor shall be responsible for providing wide-mouth sample containers, testing, and providing test results as described above.
LOCATION AND DESCRIPTION

TECHNICAL SPECIFICATIONS
The lab conducting the testing services under this Contract shall be accredited and meet the requirements specified in the Oregon Health Authority public health website:
https://public.health.oregon.gov/HealthyEnvironments/DrinkingWater/Monitoring/Pages/labs.as px#lists.
All water testing shall be in accordance with all EPA and Oregon State Standard Methods, applicable regulations, Small System Compliance Technology List for the Surface Water
Treatment Rule and Total Coliform Rule (EPA-815-R-98-001), Total Coliform Rule, NELAC, and ORELAP Standards and Methods. A Handbook for Small Noncommunity Water Systems serving less than 3300 persons (EPA 816-13-06-001).
(vii) Date(s) and Place(s) of Delivery and Acceptance/Location
Contractor shall submit test results to the appropriate State entities as required, as well as submit all test results via email in clearly legible condition to RJ Shores richard.shores@usda.gov, AND
Craig Evans craig.evans@usda.gov.
Samples to be tested will be collected by qualified Forest Service personnel, but costs to pick up and transport samples shall be included in the bid price. The samples shall be picked up at 13:00 on the first Tuesday of every month at the Forest Service Office, 855 Highway 101, Reedsport, OR 97467 and transported to the testing lab in accordance with applicable State standards.
Occasionally an emergency may arise which will prevent Forest Service personnel from taking samples and having them ready for pick up at the arranged day / time. In those situations, the https://public.health.oregon.gov/HealthyEnvironments/DrinkingWater/Monitoring/Pages/labs.aspx#lists https://public.health.oregon.gov/HealthyEnvironments/DrinkingWater/Monitoring/Pages/labs.aspx#lists mailto:richard.shores@usda.gov mailto:craig.evans@usda.gov
Forest Service will notify the lab as soon as they realize that they will not have the samples ready for pick up, and the Forest Service will then be responsible for taking the samples at a later time and delivering them to the lab. The same goes for repeat samples – if test results come back positive and repeat samples are needed, then the Forest Service will be responsible for delivering them to the lab.
Additional testing will be required, and paid for on an as-needed basis, in the event that samples test positive for coliform or fecal matter.
DESIGNATION OF CONTRACTING OFFICER'S REPRESENTATIVE
The Contracting Officer designates the Contracting Officer's Representative (COR).
The COR is responsible for administering the performance of work under this contract. In no event, however, will any understanding, agreement, modification, change order, or other matter deviating from the terms of this contract be effective or binding upon the Government unless formalized by proper contractual documents executed by the Contracting Officer prior to completion of the contract.
The Contracting Officer should be informed as soon as possible of any actions or inactions by the Contractor or the Government, which will change the required delivery or completion times stated in the contract, and the contract will be modified accordingly. (This does not apply to individual task assignments which were issued by the COR provided they do not affect the delivery schedule or performance period stated in the contract.)
On all matters that pertain to the contract terms the Contractor must communicate with the
Contracting Officer. Whenever, in the opinion of the Contractor, the COR requests effort outside the scope of the contract, the Contractor should so advise the COR. If the COR persists and there still exists a disagreement as to proper contractual coverage, the
Contracting Officer should be notified immediately, preferably in writing if time permits.
Proceeding with work without proper contractual coverage could result in nonpayment or necessitate submittal of a contract claim.
