Statement of Work
BLDG 8500 FLOORING
STATEMENT OF WORK
FOR
BLDG 8500, 30 SW/XP, 30 CONS Carpet Removal & Installation
17 JUN 2016
VANDENBERG AFB, CA
1.0 GENERAL
The 30th Civil Engineer Squadron (CES) has a requirement for install of new carpet and vinyl tile flooring in Bldg 8500, on Vandenberg AFB, CA (VAFB). New carpet and vinyl tile flooring will be installed in all interior rooms to include the 30SW/XP break rooms. Area is approximately 28,615 square feet. Measurements provided must be verified by successful offeror. Refer to the attached floor plan.
2.0 DECRIPTION OF WORK
The Contractor shall furnish all labor, supervision, materials, supplies, tools, equipment, transportation, and expertise necessary to perform the proper removal & disposal/recycling of existing flooring & base in addition to delivery & installation of the new porcelain tile flooring in accordance with the Statement of Work (SOW). The Contractor shall unload, uncrate, inspect and properly install all specified flooring in accordance with all applicable local, County, State, Federal, and industry standards, codes, laws, rules, and regulations. The Contractor shall work in coordination with the 30 CES Project Manager / Bldg Facility Manager to schedule delivery & installation of the new flooring. Any materials damaged en route or during install will not be accepted by the Government and shall be replaced by the Contractor at no additional cost to the Government.
3.0 OBJECTIVES
Obtain/perform all required permits, coordination, and documentation.
Properly remove and dispose/recycle existing flooring and base.
Install porcelain tile flooring and porcelain base in area shown on plan and described in this SOW.
Provide site clean-up.
Provide daily documentation to the 30CES project manager of completed work.
Minimize/coordinate impact to facilities, including working around the required usage of the facilities.
All Unified Facilities Criteria (UFC), Laws, Unified Facilities Guide Specifications
(UFGS) standards, etc. must be followed.
All VAFB safety requirements (Ground Safety 805-606-8874) will be met including Cal-
OSHA standards (http://www.dir.ca.gov/dosh/).
Participate in United States Air Force (USAF) Qualified Recycling Program (QRP) ensuring all standards/rules are followed.
Coordinate schedule and cooperate with the Facility Manager.
4.0 MATERIAL

5.0 ENVIRONMENTAL REQUIREMENTS
The Contractor shall be in compliance with all applicable local, County, State, and Federal environmental codes, laws, and regulations.
6.0 PERMITS, LICENSES AND CERTIFICATIONS
Unless otherwise specified in the contract, obtain necessary permits, licenses, and certifications;
give all required notices; and comply with applicable local, County, State, and Federal codes, laws, and regulations in the performance of the requirements of the contract. The Contractor shall maintain records of such requirements and make the documentation available to the Government for review.
7.0 SPECIAL QUALIFICATIONS
The contractor is required to ensure all personnel are fully trained, certified, & licensed to perform required services. The Contractor shall maintain records of such requirements and make the documentation available to the Government for review.
8.0 HOURS OF OPERATION
The Contractor shall perform work during normal duty hours. Normal duty hours are Monday through Friday, 7:00 a.m. to 3:30 p.m. PST, excluding recognized Federal Holidays (unless otherwise required in the contract or approved by the Contracting Officer). When the Contractor’s work schedule conflicts with an Air Force mission requirement, advise the Contracting Officer to reschedule the work to minimize disruption of operations.
8.1 RECOGNIZED FEDERAL HOLIDAYS
The Contractor is not required to perform work on recognized Federal Holidays, except in emergency situations. These holidays include the following:
New Year’s Day Labor Day Martin Luther King Day Columbus Day Washington’s Birthday Veteran’s Day Memorial Day Thanksgiving Day Independence Day Christmas Day
If a holiday falls on a Saturday, the preceding Friday will be observed. If a holiday falls on Sunday, the following Monday will be observed. If a holiday falls on a scheduled service day, the Contractor will be responsible for rescheduling services for the first day post the holiday observance.
8.2 BASE CLOSURES
Work scheduled but not accomplished because of base closure due to weather, exercises, or actual alert, will be accomplished as soon as possible after reopening the base.
9.0 SECURITY REQUIREMENTS
All Contractor employees shall comply with all Vandenberg AFB security requirements imposed by the Installation Commander at all times while on Vandenberg AFB premises. Contractor shall obtain all necessary passes, decals, badges, or other items required for access to perform contract services at Vandenberg AFB. The Contractor is responsible for accountability of all such media and these media shall be surrendered to the Security Forces Squadron Pass and Registration Section upon completion or termination of the Contract or upon termination of an individual’s employment for the Contractor. The Contractor shall comply with all VAFB policies regarding entry to the installation, rules of the road, contingency operations compliance, and photography prohibitions. Contractor employees are authorized entry to the installation for the purpose of work under this Contract only, and are not authorized in other facilities or areas without prior Government authorization. Due to safety/security requirements, Contractor personnel shall be required to fluently speak, understand, read, and write English.
9.1 BASE ACCESS
The Contractor needs to be aware that entry requirements are subject to change due to installation and world situations. In regards to permanent base access entry, all employees are subject to a background check through the FBI.
9.2 MAIN BASE ACCESS
Current security access restrictions at the Santa Maria Gate (Main Base entry gate) require all Contractor vehicles (which are the size of a standard bread delivery truck or larger) to enter through the Lompoc Gate only (gate is located at intersection of Santa Lucia Road and Pine Canyon Road). There is a vehicle check pull-off area and the Contractor may be required to pull-off and submit to a vehicle search prior to gaining entry to the base. The Contractor should plan and allow for delays as a result of increased and ever-changing security procedures.
9.2.1 The Contractor shall brief all employees that they shall immediately leave any area upon notification by the Contracting Officer, COR, constituted authority, or Security Forces, or when directed as occasional operational requirements, real world activities, or emergencies dictate.
9.2.2 Contractor Vehicles. Contractor vehicles must be maintained in a safe and operational condition at all times while on Vandenberg AFB.
10.0 INTERFERANCE WITH WORK
Do not unduly interfere with regularly scheduled Government operational activities in the performance of contract requirements. In the event Government personnel so requests, temporarily cease work in the area and report the instructions, to include name of the Government personnel involved, to the Contracting Officer immediately by the most expedient means. Notify the Contracting Officer verbally of disputes with customers or other base Contractors and follow-up in writing.
11.0 SAFETY REQUIREMENTS
The Contractor shall perform work in a safe manner as required by OSHA Safety and Health Requirements. The Contractor shall provide a verbal report to the Contracting Officer as soon as possible of each occurrence of damage to Government property or an accident resulting in death, injury, occupational disease, or adverse environmental impact. The Contractor shall also provide a completed copy of required Accident Investigation Reports to the Contracting Officer within five (5) calendar days of each occurrence.
11.1 SECURITY, FIRE, AND MEDICAL SERVICES
Emergency services may be obtained by calling 9-1-1 on Base phone or 805-734-4117 by cell phone. The Government will provide police and fire protection. In the event of a medical emergency, base ambulance service for transporting an injured employee to a local hospital is available on a cost reimbursement basis.
