About this file

This file contains the required submission forms for an RFP issued by the Howard County Office of Procurement and Contract Administration in Maryland for Solar Equipment Operations and Maintenance Services. The technical proposal cover page and affidavits establish the formal submission requirements and certifications that contractors must complete and submit as part of their bid response. While specific project details, quantities, timelines, and award dates are not detailed in this forms document, contractors are required to provide company information, tax identification numbers, delivery terms, and contact information for purchase order receipt.

The document establishes mandatory affidavits and certifications that bidders must execute, including attestations regarding non-collusion, conflict of interest, non-discrimination compliance, and Iran investment activities under Maryland law. Contractors must declare whether they are minority-, women-, or disabled-owned business enterprises and provide applicable certification information. The County specifies F.O.B. Destination with inside delivery as the delivery term and indicates a preference for email purchase order transmission. Contractors are required to accept Visa procurement cards without charging additional fees. Any signatory lacking the title of President or Vice President must provide verification of legal authority to sign on behalf of the contractor. The affidavits must be completed and signed by a company officer and submitted under penalty of perjury.
