Quality Assurance Surveillance Plan
Training and Technical Assistance to Native Connections Grantees

Contractor responds within 24 hours to COR inquiries 95% of the time.
COR & Contracting Officer monitoring deliverables, budget estimates, invoices and reports.
COR receives no more than 4 valid complaints per 12 months. No more than 2 invoices per 12 months requiring suspension or disallowance due to mistakes, incompleteness, or unallowable costs. If deficiencies are found, Contractor will provide COR solutions for implementation within 60 days.

Task 2
Deliver Training and Technical Assistance
| Timely and comprehensive training and technical assistance (TTA) to 112 Native Connections grantees to support conducting service delivery system analysis, developing community readiness assessments and strategic action plans, implementing strategic action plans, and other grant activities |

COR receives no more than four (4) valid complaints about TA from grantees; contractor meets delivery schedule; products are high quality and need few if any corrections. Task 3
Resource Development and Dissemination and Information Management
| Timely undertaking and completion of the development and dissemination of work products and resources, a grantee contact list and listserv, a SharePoint-type site, web page updates, and 508 compliance for all materials. |

Task 5
Transition Plan and Turnover at the End of Contract

| Contractor provides for the transition of all previous contract activities (e.g., website, training, and technical assistance) so there is no lapse in service. 100% of requested information, data and materials, is provided to COR. Transition activities are completed as specified in the SOW. |
Optional Task 6 Support for the Annual Native Connections Grantee Symposium
| Timely and comprehensive support the annual grantee symposium, including working with the evaluation contractor, planning with the COR/ACOR, and task leads, preparing all meeting materials, providing logistic support, and post-meeting debriefing and reports. |
