About this file
This is a Service Level Agreements schedule document for the Michigan Department of Corrections (MDOC) Release Day Transportation RFP contract, outlining performance metrics and accountability measures for contracted staff and service providers. The document establishes five key performance metrics with specific service standards: mandatory MDOC training completion by September 30th annually with 100% compliance required; responsiveness to Corrective Action Plan requests within specified timeframes; completion of Law Enforcement Information Network (LEIN) clearance forms for staff entering facilities or handling records; timely and accurate monthly invoicing within 15 calendar days of each invoicing period; and proper adherence to referral procedures and timely fulfillment of coordinated transportation requests. Each metric includes defined data sources, audit methodologies, and acceptable standards, with the contract effective date, agency name, and contract number to be determined at execution.
Service level credits are assessed for non-compliance with the established metrics. Training violations incur $250 per non-compliant staff member with an additional $500 penalty if over 30% of staff fail to complete training by the deadline. Corrective action plan response delays result in $500 per business day penalties. LEIN clearance failures incur $250 per occurrence, invoice inaccuracies or late submissions result in $500 per occurrence, and referral process violations or no-shows are assessed at the full cost of the specific transportation referral. The MDOC Contract Manager may review extenuating circumstances before assessing credits, and credits may be applied against payments owed to the contractor or payable directly to the State of Michigan within 10 calendar days upon demand.
