About this file

This is a pricing schedule submission document for Request for Proposal No. 260000001734 issued by the Michigan Department of Health and Human Services (MDHHS) for Family Preservation - Family Assistance Program Services in Wexford and Missaukee Counties. The contract encompasses in-home family services delivered on an hourly basis, with bidders required to propose unit rates that reflect all direct and indirect costs, including mandated insurance. The contract term spans three years, running from October 1, 2026, through September 30, 2029, with an annual award amount of $100,000.00 for each year. Bidders must submit pricing schedules in modifiable formats and certify that proposed prices were developed independently without consultation with other contractors.

The pricing structure requires contractors to propose hourly rates for in-home family services while maintaining a maximum annual award cap of $100,000.00 across all cost components. A fixed allocation of $20,000.00 annually is designated for transportation expenses and should not be factored into the unit rate calculation. Travel cost reimbursement follows the State of Michigan premium rate or the contractor's standard reimbursement rate, whichever is lower, with mileage reimbursable for client transportation, travel to client homes, court hearings, and advocacy-related site visits. The document emphasizes that failure to complete the pricing schedule as requested may result in proposal disqualification, and all calculations must remain within the specified annual award limits.
