About this file

This is a Service Level Agreements document for a Michigan Department of Corrections (MDOC) contract involving School Psychologists and Special Education Quality Assurance positions. The contract establishes performance metrics and compliance requirements that the contractor and subcontractor personnel must meet throughout the contract term. The agreement outlines five primary service level metrics: LEIN security clearance approval prior to work with MDOC offenders, timely and accurate monthly invoice submission within 10 calendar days of the reporting period, completion of MDOC-required training by September 30th of each training year, responsiveness to Corrective Action Plan requests within specified timeframes, and resolution of any reoccurring or unresolved deficiencies identified in Corrective Action Plans. The contractor must maintain 100 percent compliance across all metrics.

The agreement establishes a service credit system as the enforcement mechanism for non-compliance. Credits range from $125 per LEIN form not approved to $250 per occurrence for untimely invoices, $125 per staff member for incomplete training, $250 per business day for late Corrective Action Plan submissions, and escalating amounts from $250 to $1,000 for reoccurring deficiencies. The State may apply these credits toward amounts owed to the contractor or demand direct payment to Michigan within 10 calendar days. The MDOC Contract Manager may waive service credits in cases of extenuating circumstances. All contractor staff must submit completed LEIN Information Forms to MDOC-PMCD-FOA-LEINS@michigan.gov for approval prior to commencing contract work and annually thereafter.
