PTM-CD REPORTING REQUIREMENTS / AGREEMENT DELIVERABLES:

1. MANAGEMENT/PROGRAM PLAN DOCUMENT.

Within 60 days after award of the Agreement, the Recipient will submit a program plan which includes: (1) Timeline chart for the total program including a break out for each subtask in the SOW, (2) A table of planned monthly man-hours and costs for each WBS level 3 task, and (3) A narrative description of the technical work with responsibilities and planned man-hours defined for each subtask in the SOW. Within 30 calendar days, the Government will provide any recommended changes/revisions. After making revisions, the Recipient will provide to the Agreement Administrator within 30 calendar days a revised program plan.

2. TECHNICAL REVIEW BRIEFING CHARTS

The Technical Review Briefing Charts will be provided or made available through electronic media, as agreed to by the Responsible Technical Agent, with sufficient documentation to support each Technical Review. This will include charts for the Initial Briefing, Monthly Status Updates, Preliminary Design Review, Detailed Design Review, Test Readiness Reviews and the Final Briefing. Charts will be provided at least three business days prior to each Technical Review.

3. INITIAL BRIEFING.

Within 90 days after award of the Agreement, the Recipient will host an initial (kick-off) briefing for the project. The briefing should described the goals, timelines, and expectations for the future work.

4. QUARTERLY REPORT.

On or before 90 calendar days after the effective date of the Agreement and quarterly thereafter throughout the term of the Agreement (and any extensions of the term) Recipient will submit or otherwise provide a Quarterly Cost, Performance and Progress Report to the Technical Agent and one copy to the Agreement Administrator and Administrative Agreement Officer / Contracting Officer. The report will have two major sections.

a. Technical Status Report. The technical status report will detail technical progress to date and report on all problems, technical issues or major developments during the reporting period. The technical status report will include a report on the progress made during the reporting period and plans for the next reporting period.

b. Business Status Report. The business status report will provide summarized details of the resource status of this Agreement, including the status of the contributions by the participants. This report will include a quarterly accounting of current expenditures by task, by quarter, and provide a comparison of these expenditures with the planned expenditures in the program plan. Any major deviations in schedule, resources or technical approach will be explained along with discussions of the proposed adjustment actions.

5. MONTHLY STATUS UPDATES.

A briefing will be provided each month to facilitate technical and programmatic communication, the detail of which will be agreed to by the responsible Technical Agent (s). The briefing will provide a summary of the current status of the project with any pertinent technical or business information, to include current progress, future work, issues, data, and as needed discussion.

6. DETAILED DESIGN REPORT.

The Detailed Design Report will be provided or made available on electronic media, as agreed to by the responsible Technical Agent (s). This report should be provided within three months of the completion of the detailed design, and include a thorough documentation of all design details (e.g. key defining parameters/graphs, materials, design illustrations with dimensions) and efforts (e.g. electrical, mechanical, and thermal) and any trade analyses of the component at the system level.

7. TEST PLANS/TEST REPORTS.

The Test Plans/Test Reports will be provided or made available on electronic media, as agreed to by the responsible Technical Agent (s), with sufficient documentation to support the testing required. Test plans and reports should be provided at a minimum for all component level demonstrations.

Recipient will provide, as a minimum, the following:

a. A description of the item/system undergoing test to include instrumentation locations

b. Objectives of the test

c. Description of test procedures

d. Performance requirements and acceptance/compliance limits

e. Test schedules

f. Test support requirements such as facilities, GFE, personnel, and consumables

g. Discussion of significant test results, observations, conclusions, and recommendations

h. Tables, graphs, illustrations, charts, and analyses as necessary

8. FINAL BRIEFING.

The Recipient will present a final briefing, detailing the results of program, within 30 days of the completion or termination of the Agreement. The location will be at Ft. Eustis, VA or another location specified by the Technology Development Directorate.

9. FINAL REPORT.

The Final Report will provide detailed discussion of all technical work accomplished and information gained in performance of the Agreement, pertinent observations, nature of problems, positive as well as negative results and design criteria established as applicable. The details of all technical work included will be sufficient to permit full understanding of the techniques and procedures used in evolving the technology or processes developed, conclusions reached, and recommendations made. The report will also include a summary of the task-by-task work flow as actually performed and a summary of the schedule as actually performed. The Recipient will submit a draft final report (2 versions) making full disclosure of all major developments within 60 calendar days of completion or termination of the Agreement. Within 30 calendar days, the Government will provide any recommended changes/revisions. After making revisions, the Recipient will provide to the Agreement Administrator within 30 calendar days one electronic copy, in word processing software acceptable to the Technical Agent, and four paper copies. One hard copy and one electronic copy will be submitted to the Defense Technical Information Center (DTIC) addressed to Building 5, Cameron Station, Alexandria, Virginia 22314, after Government approval.

The final report will include a SF 298. The Grants/Agreement Officer / Contracting Officer at the completion of the technical effort will provide the report number and completion instructions. The final report versions will be marked with appropriate distribution statement(s) to denote the extent of its availability for distribution, release, and disclosure without additional approvals or authorizations. The final report will be marked on the front page in a conspicuous place with the following marking:

Distribution Statement applicable to the version with technical data subject to unlimited rights.

DISTRIBUTION STATEMENT C

Distribution authorized to U.S. Government Agencies and their contractors, Critical Technology and Export Controlled, (Date). Other requests for this document will be referred to the Technology Development Directorate, Attn: FCDD-AMV-EPE, Fort Eustis, Virginia 23604-5577.

Applicable to the final report containing data with background data and will bear the following marking on the front page:

DISTRIBUTION STATEMENT B

Distribution authorized to U.S. Government Agencies only, Proprietary (DFARS Limited or Government Purpose), Export Controlled, and Critical Technology information, (Date). Other requests will be referred to the Technology Development Directorate, Attn: FCDD-AMV-EPE, Fort Eustis, Virginia 23604-5577.

Any deliverables or reports and any attachment/appendix thereto containing Export Controlled Data will also bear the following notice:
