About this file

This Quality Assurance Surveillance Plan (QASP) is for a Generator Preventative Maintenance contract at the Asheville Veterans Affairs Medical Center in Salem, VA, with contract number 36C24625Q0480. The document outlines the government's method for evaluating contractor performance through systematic surveillance, with three primary performance objectives: adherence to the Performance Work Statement, documentation and warranty requirements, and inspection specifications. The plan establishes an Acceptable Quality Level (AQL) of 95-100% for these objectives, with the Contracting Officer Harold L. McQuillia, Jr. overseeing the contract and having ultimate responsibility for performance determination.

The QASP includes a detailed surveillance approach using methods such as direct observation, complaint review, and user surveys, with a rating scale that classifies contractor performance as positive (no more than 1 incident of not meeting standards), neutral (no more than 3 incidents), or negative (more than 3 incidents). The government will document performance, with potential incentives including favorable performance evaluations and potential disincentives such as unfavorable evaluations or Contract Discrepancy Reports. Monthly performance assessment meetings are planned, and the document emphasizes the government's commitment to objective, fair, and consistent performance evaluation.
