About this file

This document includes a past performance questionnaire and a pre-solicitation notice for architectural and engineering services. The past performance questionnaire is a template for clients to evaluate a contractor's performance on a previous project. It requests information about contract details, quality, schedule, management, costs, and whether the client would hire the contractor again.

The pre-solicitation notice is seeking Standard Form 330 submissions from interested firms for a seismic study at the VA Medical Center in Columbia, Missouri. The scope involves investigative and design services to propose seismic retrofits for an eight-story concrete building. The contract is set aside for Service-Disabled Veteran-Owned Small Businesses. The selection criteria are provided to evaluate submissions, prioritizing professional qualifications, specialized experience, management plan, capacity, past performance, and geographic location. Responses are due by June 20, 2022. The period of performance for design is 120 days, with construction period services optional.
