ATTACHMENT 6

Instructions for Submission of Quotation

(Request for Quotation - P17PS02555)

QUOTATIONS WILL ONLY BE ACCEPTED IF SUBMITTED ELECTRONICALLY BY E-MAIL.

1. Documents/Information To Be Submitted With Quotation: At a minimum, the following documents/information shall be completed/provided and submitted with your quotation.

Clauses and Provisions indicated below which are incorporated into the solicitation by reference rather than full text, can be accessed online, in full text, at:

a. Standard Form 1449 / Optional Form 336

(1) If Contractor is offering discount terms, complete Block 12.

(2) Complete Block 17a, also include DUNS #.

(3) Complete Blocks 30a, 30b, 30c.

(4) Enter the Total Quoted Price in the Amount Column of Line 00010.

b. Complete Attachment 3, Quoter’s Price Schedule, by entering the individual pricing for each location, the Total Price, completing all information, sign and date, and submit with quotation package.

c. Provide responses to the elements addressed in Attachment 4, Technical Approach.

d. Complete and submit Attachment 5, Technical and Financial Capability Statement along with the additional documents requested by that Attachment.

e. Refer to ‘Clauses and Provisions’ of Solicitation

(1) Complete and submit Provision 52.212-3 Offeror Representations and Certifications.

• If you have completed your Annual Representations and Certifications electronically via the System for Award Management (SAM) website at www.sam.gov, complete paragraph (b) of the 52.212-3 provision and submit with your quote.

• If you have not completed your Annual Representations and Certifications electronically via the System for Award Management (SAM) website at www.sam.gov, you must do so as soon as possible. See Paragraph 4 below.

(2) Complete and submit the information requested by Clause 52.223-3, Hazardous Material Identification and Material Safety Data (if applicable).

(3) Provision 1452.215-71 – Use and Disclosure of Proposal Information – Department of the Interior (if applicable)

(4) Provide any additional information requested by Evaluation Factors for Award paragraphs (c) (1) through (c)(4) if not called for elsewhere in these instructions or not already included in your completion of SF-1449, and Attachments 3, 4 and 5.

e. Any other information or documents requested by the solicitation that are applicable to your quotation. See FAR Clause 52.212-1. f. Acknowledgment of all Amendments which may be issued against this solicitation.

2. Points of Contact: The point of contact for this solicitation is Kathryn M. Logsdon, Contracting Officer, at telephone number (217) 391-3225, or by e-mail at kathryn_logsdon@nps.gov.

3. Amendments to Solicitation: It is the responsibility of the quoters to monitor the FBO and

FedConnect websites to download and acknowledge all amendments issued against the solicitation.

4. System for Award Management (SAM) and Online Representations and Certifications: All quoters are to have a Dun & Bradstreet number, be registered and active in the System for Award Management (SAM), be certified as a small business in the NAICS Code of 115310, and have completed their Online Representations and Certifications in order to be awarded a contract; therefore, if you have not yet completed these requirements, it is strongly advised that you do so at this time. Failure on the part of the successful quoter to have these requirements fulfilled by the time the Contracting Officer makes award may result in award of the contract being made to the next otherwise successful quoter. Access to the System for Award Management (SAM) website can be made at https://www.sam.gov

5. Explanation to Quoters:

a. Quoters may request an explanation or interpretation of the solicitation, or its attachments by contacting the Point of Contact named in paragraph 2 above.

b. When sending a question(s) via e-mail, prospective quoters are requested to include the solicitation number and contract title in the subject line of the e-mail and the Company’s full name and address within the body of the message. Questions are to be submitted no later than 10:00 a.m. central time on Thursday, August 24, 2017.

6. Delivery of Quotations:

a. Quotation packages are to be received by the Contracting Officer no later than

Wednesday, August 30, 2017 at 2:00 p.m. central time. Failure to submit your quotation package so that it is received by this time and date may result in the quotation not being considered.

Facsimile quotations will not be considered.

7. Notice of Pre-Award Survey:

a. No award shall be made unless the Contracting Officer can make an affirmative determination of responsibility of the prospective contractor as per the standards identified in FAR Subpart 9.1. Quoters are advised that this is accomplished through a pre-award survey which may require the prospective contractor to provide additional information or documentation as requested by the Contracting Officer. Areas of interest on this survey may include, but are not limited to the following, with regard to the prospective contractor:

(1) Have adequate financial resources to perform the contract, or the ability to obtain them.

(2) Be able to comply with the required delivery date, taking into consideration all existing commercial and governmental business commitments.

(3) Have a satisfactory performance record.

(4) Have a satisfactory record of integrity and business ethics.

(5) Have the necessary organization, experience, accounting and operational controls, and technical skills, or the ability to obtain them, including, as appropriate, such elements as production control procedures, property control systems, quality assurance measures, and safety programs applicable to materials to be produced or services to be performed by the prospective contractor and subcontractors.

(6) Have the necessary production, construction, and technical equipment and facilities, or the ability to obtain them.

(7) Be otherwise qualified and eligible to receive an award under applicable laws and regulations.

b. Quoters are advised that accomplishment of this survey is a part of the determination of responsibility process and is not to be construed that a request to the prospective contractor for additional information and documents is an indication that he/she will receive the resultant award.

8. Compliance: By submitting a quotation, the prospective quoter agrees to comply with all terms and conditions as set forth in the solicitation.
