2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:

The Codonics Safe Labeling System is being requested to remove human error when anesthetics are being prescribed in a surgical setting. A recent medication error severely impacted patient safety and the procurement of these devices will alleviate those issues. The Codonics system is a Federal

Drug Administration (FDA) approved medical device that improves the safety and accuracy of medication management. The system uses barcode technology to read information from a drug container and electronically verify it against the hospital’s pharmacy-approved formulary database at the point of care.

Estimated Amount: $42,580.00

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:

The Codonics SLS system is the only system designed to work with the Omnicell medication cabinets. The Omnicell platform uses proprietary, trade secret, and patented technology to store and dispense medications. Codonics is the only authorized manufacturer to produce these items that will integrate with the Omnicell cabinets. They are FDA approved for use in a healthcare environment and designed to operate in accordance with Joint Commission standards.

4. Description of market research conducted and results or statement why it was not conducted:

Codonics is the only authorized manufacturer to produce these items that will integrate seamlessly with the Omnicell cabinets on already on site. They also confirmed that there are no authorized distributers for these items.
