About this file
This document is a Single Source Justification form for a Simplified Acquisition Procedure (SAP) under the Simplified Acquisition Threshold (SAT) issued by the Department of Veterans Affairs. The document justifies a sole-source procurement of the Shukla screw removal system for the Baltimore VA Medical Center, with an estimated contract value of $36,175.50. The system is a universal surgical instrument designed to remove broken, stripped, and seized bone screws with minimal patient bone loss, specifically used in trauma and total joint surgeries.
Market research conducted on GSA, SAM, NAC, FPDS, and SBA/DSBS platforms revealed only two potential vendors: 1 large business (Shukla Medical) and 1 small business (Veterans Health Medical Supply). Extensive searches across multiple platforms using various keywords and manufacturer part numbers yielded no alternative systems that meet the specific requirements. The justification emphasizes the unique characteristics of the Shukla system, including surgeon familiarity, minimal additional training needed, and the absence of comparable universal screw removal systems in the market.

Acquisition Plan Action ID: 36C245-25-AP-2408, SHUKLA Surgical Instruments
1. Contracting Activity: Department of veterans Affairs, NCO5, located at 50 Irving
St. NWS, Washington, DC 20422. In support of the Baltimore VA Medical Center located at 10 North Greene Street Baltimore, MD 21201.
2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:
The Shukla screw removal system is a universal system designed to remove all broken, stripped, and seized bone screws with minimal bone loss to the patient. This system is a comprehensive screwdriver system designed to simplify the removal and insertion of large bone screws commonly found in both trauma and total joint surgeries. This will be used in the operating room and returned for sterile processing for reprocessing.
The estimated amount of the proposed action is $36,175.50.
3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:
The Shukla screw removal system is unique and limiting the requested requirement to Brand Name Only because it is a system that doctors are already familiar with at the Baltimore VA Medical Center. These surgeons have been trained with the previous Shukla system, so procuring this system will limit the training needed for an equal item. As well as standardization, this system fits with Brand Name Only Characteristics because after market research that was performed, no other systems could be found that satisfy the universal screws that can be used for removal during surgeries.
Based on the previous procurement of these items, no other system can provide the necessary measurements of the screw removal devices.
4. Description of market research conducted and results or statement why it was not conducted:
